Jul 2024 FORWARD COLLISION AVOIDANCE The FSD is not fully matured and I have tried multiple times and all times and the experience is weird and very hard braking and no control on parking lots. So not sure why this is called Fully self driving capability by Tesla? I have paid 12k for this capability in my car and unfortunately it's not fully functional. Hope some safety precautions will be taken for the safety of the drivers and passengers. Happy to provide more details if needed. Thank you

Jun 2024 VISIBILITY/WIPER Tesla broke Automatic wiper function with a software update in 2023. Models other than Model Y have or have had this issue. There is no setting or adjustment I can make and Tesla has not made it a priority to fix this. They spend all of their resources doing other things. Nobody at Tesla is listening. New feature after new feature and screen changes are released but no fix for wipers. This is a conic years long recurring issue with Tesla cars. Tesla service can not fix software issues. It seems Tesla will never fix the issue. It creates a safety problem when driving because you must take your attention off the road and interact with the screen or try and remember the various voice commands to control them, or look at the screen while scrolling a wheel (you must pre-configure ahead of time to do this) while looking at the screen to see what gets selected on the screen, or do everything manually on the center screen. Attention is pulled off the road frequently regardless of weather. Currently in dry weather the wipers turn on unpredictably and often. Hour after hour on a long trip and you battle them all day long. Wipers are switched to Auto every time you use a driver assistance system such as Autopilot or Full Self Driving. You repeatedly disable auto wipers using your choice of method all day long. You are ready to rip wipers off the car by the end of the day. In wet weather auto wipers don't turn off when the rain has ended temporarily or becomes a mist needing only an occasional wipe by pressing the single wipe button. If you disable Auto wipers when it is raining you are constantly adjusting the speed between off, low, medium, and high which is eyes off the road screen intensive using scroll wheel or touching screen directly. Better if you can remember the speed adjustment voice commands which I can't because I rarely will drive in the rain because of wipers. I can remember disable because I use that constantly nearly every time I drive depending on the sky.

Jun 2024 UNKNOWN OR OTHER,FORWARD COLLISION AVOIDANCE Tesla will not fix: Tesla Pushed a Spring Software update which introduces risk to pedestrians and risk of accident including rear-ending. In the past an image of the car is on the left side of the screen and a large map is on the right side of the screen. With this update, the map is removed from the right side of the screen when not in gear. If the car is in gear it remains on the screen. To setup ADAS destination it now works best if you have the car in gear. If the car is in park you have only full screen car image and extra steps to get the large map. If you voice command destination you may never see the map until driving on road. You should be driving at that point not interacting with the screen. Accident and pedestrians at risk while using screen. For instance if you get in the car and issue a voice command to destination with only one location the tiny map image in the far upper right corner of the screen displays a route so small and completely without detail it could be wrong but you just assume it is correct because it will take extra user interface interaction to make it large enough to tell what it is actually going to do. You put the car in reverse and back into the street, put the car in drive and now you get the full size map, route, destination for the first time and route is wrong. When in park, interacting with the map you don't really know what will cause the map to go away and be replaced by the car image. Can happen if you change destination entry. The map is removed from the screen every time you put the car in park. Now a big problem. If you are driving and decide maybe I should go to a different destination first and you pull over and put the car in park the map is removed from the screen again and the destination remains unchanged. You either fight the propensity to replace the map with the car image or don't put the car in park. To prevent the car screen from replacing the map unpredictably you have to keep the car in gear.

Jun 2024 SERVICE BRAKES,FORWARD COLLISION AVOIDANCE Safety issue with Tesla's auto pilot braking without cause in the middle of freeway. Many times accidents were about to happen when I intervened and press onto the gas pedal. It seems like there are other complaints and not sure why NHTSA is not doing about it? Is Tesla that powerful that government does not want to disturb anything?

Jun 2024 VEHICLE SPEED CONTROL,SERVICE BRAKES,FORWARD COLLISION AVOIDANCE Phantom braking occurred when the adaptive cruise was engaged when a truck entered the road from the right side. The car abruptly slowed when the truck crossed my lane although there was no eminent danger. The car behind us nearly collided with us due to the sudden braking incident. This is the third sudden braking incident; however, the others were related to a bridge or overpass.

Jun 2024 STEERING,LANE DEPARTURE,FORWARD COLLISION AVOIDANCE Two incidents: 1) Non-accident: My wife was slowing to make a right turn at a stop light and the steering locked up for about 3 seconds, where she wasn't able to regain manual steering control. We did not have full self-driving mode at the time. Luckily, she was only going about 10 mph, and was able to brake before running into oncoming traffic. 2) Accident: Our son was driving on a country road, along a section with a slight curve. We had a free trial for full self-driving mode during the previous month, but it ended two weeks before the accident, so he could not have had it in self-driving mode. It appears that he didn't make the curve, but as evidenced by a single long tire skid mark, continued almost perfectly straight, across the oncoming lane, and ran into a redwood tree. The car rolled and ended up upside down, against the bank back on the right side of the road, with the lithium batteries on fire. Our son and his passenger sustained severe injuries and neither has a memory of what caused the accident. Naturally we suspect the possibility that the steering locked up again. But it also seems likely that the AEB and ABS were not functioning correctly, as if they were, there should have been several short skid marks for the tires on both sides of the car, rather than only a single long skid mark. Additionally, there's no shoulder on that road, and there is a clump of bushes that come very close to the white line, so it's possible that a steering auto-correct initiated the incident. We have not yet been able to recover EDR or log data from the vehicle.

Jun 2024 UNKNOWN OR OTHER The (Part:REAR AERO SHIELD(1498771-00-E) SCREW - HEX -M6X16(1484170-00-A) and also the (Part:DIFFUSER REAR FASCIA(1494007-00-B) SCREW - HEX - M6X16(1484170-00-A) RVT PUSH - PULL 8X18X20.0). On June,05,2024 there was a storm and there was flood areas. I drove the vehicle while passing these parts came out because water underneath removed them. The manufacturer did not tight the screws correctly and water caused detachment underneath behind the electric battery where the Rear electric motor is located. I reported this and schedule a service appointment but Tesla the manufacturer said they will not cover this under warranty. This is not fair. This should be secure and safe and not detached on its own when water hits it. This should be cover under the manufacturer warranty.

Jun 2024 UNKNOWN OR OTHER On two occasions, the Tesla hijacked 911 calls during emergencies. One time I was inside someone's house talking to 911. Suddenly I was cut off. The 911 call was hijacked by the Tesla and the person in the car did not understand how to respond to the emergency. On the second occasion, I was outside the vehicle, but the windows were open. I called 911 and was talking to them, but then the Tesla hijacked the call. I tried to continue the conversation through the open windows of the car, but this route of communication suddenly stopped. I changed to speakerphone and was able to complete the call through my phone. Tesla states that all car systems that use Bluetooth (Tesla, Apple Carplay) have this feature and that all is operating normally. (I guess if all employees steal from their employer, all is normal when someone steals from their employer.) Tesla says that before making a call, someone would have to disconnect Bluetooth or unpair the phone from the car. But that is not the solution. Tesla should use, for example, the in vehicle camera to determine if someone is in the vehicle before hijacking the phone call through Bluetooth and there should be an active prompt for Tesla to take over the call. Since Bluetooth is needed in order to unlock the vehicle, it is not a good solution to force users to turn Bluetooth on and off on the phone. Tesla indicated that they would log the complaint, but kept saying that since every other car did this they would probably not take corrective action.

May 2024 WHEELS My Tesla model Y performance 2022 was purchased on September 26, 2022. I found that the left rear wheel was leaking in July 2023. After other tire repair shops failed to find the cause of the leak, I went to the Alhambra Tesla service center for further inspection. They found that my wheels rims had cracks that caused the tire to leak very slowly. These cracks are invisible to the naked eye (I still have this wheel rim at home). During my negotiation with this Tesla service center, they told me that it was my use that caused the wheels rims to crack, and they would not be responsible for the wheels rims. I disagreed with their decision and bought a new WHEEL RIM at my own expense. I use this car for work. I have never driven this car for strenuous exercise or to steep mountains. This is my vehicle usage scenario. In February 2024, the same situation occurred on my right rear wheel. My wheel began to leak slowly and I couldn't find the reason. After multiple inspections, Tesla service center found that there was a very small crack in the right rear wheel rim. The conclusion of TESLA SERVICE CENTER is still that my use caused the crack of the wheel rims. TESLA MODEL Y PERFORMANCE is a high-performance sports car. The wheels rims are specially made 22 inches and are produced by the original factory. There have been many cases of wheels rims cracking in 18 months. I hope to attract your attention and conduct product quality inspections, which is very important for safety.

May 2024 ELECTRICAL SYSTEM,STRUCTURE,UNKNOWN OR OTHER I just got T-Boned in my 2022 Model Y performance. I got out of the vehicle which wasn't in terrible condition, in fact the left side of the vehicle was still perfect and the doors worked fine. I had my dog with me and everyone was okay, but I opened my door and the back passenger door to grab a jacket and check on my dog. I shut the doors to make sure he didn't run out and was a bit out of it because of the accident impact so I took a moment to compose myself.. All of the car's lights flickered and then the electric cut off leaving the doors completely locked and inoperable even though they had just been used moments before and were not damaged in the accident. I had to climb in through a shattered glass window to get my dog as all the doors wouldn't open from the outside and I was unable to access the glove box, nor pry it open because the electricity was out and the impact caused that door to make the side panel to unlock the glove box manually accessible. If the car DID catch fire or an occupant were bleeding out: Anyone inside, unconscious, would be unable to use the manually release and would be burned alive while people pull on doors that won't open that were undamaged in the accident and working 60 seconds before. From what I have read the only solution would be to jump the car using the 12V battery in the Frunk of which the Tow company and Emergency Responders were not familiar with.

May 2024 FORWARD COLLISION AVOIDANCE The adaptive cruise control doesn’t function correctly. I was on the freeway, which was 70 MPH, and all of a sudden the vehicle breaker abruptly while in autopilot. I saw that it registered the max speed as 40 MPH. It almost caused a collision.

May 2024 SUSPENSION,WHEELS,UNKNOWN OR OTHER I was sideswiped by another vehicle which just joined our direction of traffic coming out of a cutout. As soon as I saw the vehicle moving fast from inside the cutout to join our direction and into my lane, I immediately decided to move to the lane next to me. I was at the leftmost lane at the time. Halfway into the lane I just felt the violent impact of the other vehicle hitting us. My wife and I were shaken so bad, we were shocked and terrified at what just happened and what else might happen to us. I felt like we were travelling on the front wheels with our rear end up in the air. Next thing we knew, the car stopped in front of a tree. Our iphones and the our car's phone called 911. We were pulled out of the car and taken immediately into an ambulance and Orange Global Hospital, where we stayed for about 2 days. X rays, CT Scan and MRI were taken. NOTE: A ROD STICKING OUT FROM THE FLOOR HIT MY WIFE during the accident. This rod was connected to the wheel and was not supposed to be stick out inside the car during an accident! I BELIEVE THAT A DEFAULT IN CAR DESIGN CAUSED THIS ROD TO HIT MY WIFE , WHICH CAUSED SO MUCH PAIN, DOCTORS COMPARED HER BRUISED RIBS TO BROKEN RIBS!!

May 2024 FORWARD COLLISION AVOIDANCE,LANE DEPARTURE I've had Enhanced autopilot since June 24 2022. Since that time I've accumulated approximately 15,000 miles, with approximately 80% being auto pilot miles. On May 14th my car was upgraded to "Full Self Driving (Supervised)" without my consent. I was given 30 days of FSD trial, with no way to opt out. I was presented with a software upgrade that looked like a normal upgrade, however after the upgrade I was told I had FSD. I immediately disabled FSD, only enabling autopilot (Highway only), and attempted a drive from Fairfield, CA to Concord CA along [XXX]. During the drive, the car was not able to maintain lane control, and would suddenly apply hard braking randomly. On two occasions the car accelerated towards stopped traffic, and I had to disengage autopilot. I ended up manually driving the car as it was unsafe to use to assistive systems. I contacted Tesla, who said they where unable to remove the FSD trial, and I was stuck with the upgrade, even though it made the car unsafe to drive. May 2024 SUSPENSION The screws of front left lower control arm my Model Y came off. There was initially a clicking noise and the noise turned into a larger "boom" noise and when control arm came completely detached from my car. There were two large bolts found below the car. The car was relatively new and never been in an kind of accident or had any type of impact. Luckily, I was backing the car out of my driveway when this occurred, or this could have been a more severe accident, if it occurred while driving on the road. The entire hood of the car landed on the front left tire and there was no way for the car to move. It was towed to the Tesla service center, where they fixed the car. It was covered under my warranty as the car only has 16,700 miles on it. The service center was unable to diagnose the root cause of this issue. The service center indicated that they have seen similar issues to this, but in very rare instances. They said the car is now safe to drive, but this was something I have never had happen with any other car in 30 years of driving. However, I felt that I had to notify someone of this issue.

Apr 2024 VEHICLE SPEED CONTROL,FORWARD COLLISION AVOIDANCE I was performing a 3-point turn on a residential street. While reversing during the turn the Automatic Emergency Braking enabled and slammed on the brakes, bringing the car to a complete halt instantly. This caused my head to hit the headrest with enough force that I have headaches and brain fog the next day, indicating a concussion. I was 10 feet from the nearest parked vehicle when reversing; the AEB kicked in erroneously. This is not the first time that Tesla's AEB has erroneously and dangerously engaged while I was driving.

Apr 2024 STEERING,FORWARD COLLISION AVOIDANCE Incident Report: Model Y Autopilot Anomaly Incident Details: Date and Time: April 18th, 2024, approximately 11:44 AM PST Location: I-405 Northbound Vehicle: 2022 Model Y Autopilot Setting: Full Autopilot engaged at 65 miles per hour Description: While driving my Model Y on I-405 Northbound, I experienced a sudden and unexpected event. As I reached for a bag containing an apple, the car abruptly veered left toward the middle concrete divider that separates the Southbound and Northbound lanes. The impact resulted in a noticeable scratch sound as the car brushed against the divider. Challenging Autopilot Behavior During this incident, I felt like I was wrestling with the Autopilot system. It took approximately 7 seconds of back-and-forth struggle before I regained control of the vehicle. Fortunately, there were no other cars nearby, and both the car and I remained unharmed. The outcome felt like a true miracle. Summary In summary, the Autopilot system executed a sudden maneuver without any prior warning. The experience left me shaken, and I continue to have nightmares about what had happened. I have saved 4 videos which recorded the accident.

The contact stated that the vehicle was locked and she was unable to exit the vehicle. The contact received assistance from a bystander who opened the door from the outside. After that, the contact stated she was able to drive to her destination. The contact stated she contacted the dealer and they advised her to take the vehicle for diagnosis. The vehicle was towed to the dealer. The contact stated that the vehicle was repaired. The contact stated that she experienced another incident with the vehicle. The vehicle was parked in front of a veterinary clinic when suddenly the vehicle accelerated on its own and she was unable to stop it which led her to crash into the veterinary clinic building. The contact didn't experience any injuries. No police report failed. The vehicle was not repaired they related the incident with an operator error malfunction. Apr 2024 FORWARD COLLISION AVOIDANCE Autopilot began beeping with messages to pay attention while on highway. Moving the wheel did not remove message to the point car was shaking in lane due to turning the wheel. Warning beeps and messages got louder and more distracting and would not go away until brake petal was used at highway speeds to disengage autopilot.

Apr 2024 ELECTRICAL SYSTEM A Tesla Model Y has one screen which is all instruments, warning indicators and reverse camera. On March 3rd, 2024 I had just completed about a 40 mile drive. I parked the vehicle for approximately three minutes and upon returning to the vehicle the screen failed to turn on. The vehicle ran and drove perfectly normally, but I had zero instrumentation or warning indicators. I drove home about two miles this way without incident. Upon exiting the car and closing the door (which is the equivalent to turning off the key in a traditional vehicle), the screen turned back on as normal and has since not repeated this issue. I contacted Tesla service about it and even sent them a video showing the vehicle operating with the screen not functioning and I was told to just reboot the car and they brushed it off and would not see the vehicle over this incident.
