| Feb 2022 | VEHICLE SPEED CONTROL,FORWARD COLLISION AVOIDANCE | Cruise was control set to 70 mph while I was driving on a highway. There were no cars in front of me, there was a van following behind at a safe distance. As I drove up a hill on the highway my car suddenly decelerated. I did not get to see what my speed reduced to, but it was very dramatic, and I would estimate a slow down of 20 mph in a very short distance. | Crash: No Fire: No | Unknown |
| Feb 2022 | VEHICLE SPEED CONTROL,FORWARD COLLISION AVOIDANCE | Driving down the road on Cruise control and car will slam on the brakes and you almost get rear ended every time. Scary to say the least. | Crash: No Fire: No | Unknown |
| Feb 2022 | VEHICLE SPEED CONTROL,SERVICE BRAKES,FORWARD COLLISION AVOIDANCE | The cruise control system malfunctioned during the very first highway use (within several weeks after receiving the vehicle). While driving at a high speed on the open highway, the car would break hard for no clear reason, even when I was not following closely behind any other car. The nearest car in my lane was least 20 car lengths ahead, if at all. This experience was quite traumatizing to both me and my wife, and made us feel unsafe on the highway. Luckily, I follow the speed limits in our state, which reduces the likelihood of a rear-end collision (all others drive at least 10 MPH faster than the limit and zip past me). However, this "phantom braking" (as others have dubbed it) is very hard and sudden, often dropping the car down from 70 to 50 MPH in seconds, without warning. When I contacted Tesla via the app, they texted me to reset the cameras (do a hard reboot). There has been no follow-up by them. I do not think that this has fixed it, but have not given it a full test on a long trip. I wonder if this has something to do with the removal of the radar guidance system by the manufacturer, as this system was discontinued immediately before my Tesla was built. This experience has been so disturbing that we are seriously considering selling the Tesla and buying another brand of EV. It made our initial 200 mile trip in the car one of the worst driving experiences that I have had in decades. I had to hold the steering wheel with an iron grip out of fear for when the car would next capriciously decide to jam on the brakes. Needless to say, I was physically exhausted and emotionally drained by the time that we reached our destination. To specifically address your questions: the car is available for inspection; it put my and my wife's safety at risk; the "dealer" has not specifically confirmed the problem; nobody has inspected the vehicle; there was no warning prior to the problems. | Crash: No Fire: No | Unknown |
| Feb 2022 | UNKNOWN OR OTHER | The rear undercarriage body panel was not fully attached. The front bolts were missing causing a scooping of air and scraping of the panel against pavement during higher speeds. | Crash: No Fire: No | Unknown |
| Feb 2022 | SERVICE BRAKES,FORWARD COLLISION AVOIDANCE | I purchased the vehicle on 2/12/2022. In the first six days of ownership, the vehicle suddenly braked on two occasions while using cruise control. | Crash: No Fire: No | Unknown |
| Feb 2022 | FORWARD COLLISION AVOIDANCE | When driving on a two lane highway (US 131 and US 115) at speeds of 55-70mph in clear weather with adaptive cruise control engaged over a 4 day period and covering ~500 miles, this car starts emergency braking (warning chime and sudden deceleration) EVERY time the car interacts with an oncoming tractor trailer going the opposite direction. It will sometimes do this for multiple, closely following cars coming the opposite direction. It will additionally slow down when overtaking tractor trailers on the expressway (I-75) traveling at speeds of 70-85mpg when passing on the left and rounding gradual curves. | Crash: No Fire: No | Unknown |
| Feb 2022 | FORWARD COLLISION AVOIDANCE | Vehicle has phantom braking when auto is in cruise control on a two-lane road. Usually when a semi-truck or other large vehicle is coming towards us, car will sound an alarm and break heavy, reducing speed by 20-30 mph. This is not an issue when on the Inter-state highway. This has been an ongoing problem since we picked up the vehicle in Mid-December of 2021 | Crash: No Fire: No | Unknown |
| Feb 2022 | FORWARD COLLISION AVOIDANCE | While using adaptive cruise control at highway speeds, the car suddenly engaged the brakes. This occurred both on a multilane highway (with no direct oncoming traffic) as well as on a smaller single lane highway with oncoming traffic in the adjacent lane. When it happened on a multilane highway, there were no cars ahead of me at the time. I was passing a large exit sign located on the side of the road. When it happened on the single lane highway, there was a semi truck headed towards me in the adjacent lane. On the multilane highway, it was fortunate that there was nobody behind me as the brake application was forceful and could have resulted in me being rear-ended. On the single lane highway, it was a terrifying experience because the brakes engaged rapidly as a truck was approaching at highway speeds. The problem has not been confirmed by a dealer, I have an appointment for service pending. It has not been inspected by any party. The collision warning alarms did not go off when during either event, and no other alarms were activated. I turned off the cruise control after the second episode and have not used it since. | Crash: No Fire: No | Unknown |
| Feb 2022 | VEHICLE SPEED CONTROL,SERVICE BRAKES,FORWARD COLLISION AVOIDANCE | The vehicle will brake while utilizing the AutoSteer and Cruise Control features. I do not have the Advanced AutoPilot feature enabled on my vehicle. The vehicle will brake suddenly at highway speeds requiring me to take control of the vehicle immediately. This is concerning and May result in an rear end collision. | Crash: No Fire: No | Unknown |
| Feb 2022 | ELECTRICAL SYSTEM,FORWARD COLLISION AVOIDANCE | Phantom braking using traffic-aware cruise control during 205 mile trip. Car brakes suddenly on two lane roads, especially for large oncoming semi trucks, but also randomly with no traffic. I have to keep foot on gas pedal to keep cars behind me from rear-ending my car. Also, one time the computer screen locked up and went black. I could still drive, but there was no indication of speed. The PC rebooted and went back to normal approximately five minutes later. | Crash: No Fire: No | Unknown |
| Feb 2022 | FORWARD COLLISION AVOIDANCE | Bought this car three weeks back. It has the issue oh phantom braking. Where car start braking out of nowhere when on autopilot. No object is detected during this | Crash: No Fire: No | Unknown |
| Feb 2022 | ELECTRICAL SYSTEM | Phantom braking. When car is on autopilot the car suddenly brakes. It’s happening a lot when I’m driving back from work. I drive 72 miles one way to work on Mondays and some Fridays. On one day it did breaks 3 times with no object in sight. Glad I’m not rear ended | Crash: No Fire: No | Unknown |
| Feb 2022 | FORWARD COLLISION AVOIDANCE | While traveling down a two lane road using adaptive cruise control and no one in front of me, the car rapidly decelerating replicating the often reported phantom braking event. This happened three times in roughly a mile before I disengaged autopilot. | Crash: No Fire: No | Unknown |
| Feb 2022 | SERVICE BRAKES,FORWARD COLLISION AVOIDANCE | Application of emergency braking without warning when TACC cruise control system engaged. Yes, vehicle is available for inspection upon request. The vehicle was driven yesterday for its 1st longer mileage trip since purchase in February, 66 miles on two lane highway and interstate. When the Tesla TACC system (cruise control) was engaged the braking system abruptly applied with no warning and without reason. This occurred approximately 6 times during the drive and only while cruise control was active. Two instances appeared to have been caused by on coming vehicles in opposing direction in the other lane. One at lower speed, 25 mph, a bus in the opposite lane. Our vehicle applied the brakes hard enough that the car almost came to a complete stop. There was no vehicle directly behind but if there were we would likely have been hit. The 2nd was an oncoming semi truck on a two lane highway at approx 65 mph. Our vehicle again with no warning slammed on the brakes. At this speed we felt some difficulty gaining control of the vehicle. Again, no vehicles were directly behind so we avoided another possible collision. The remaining 4 instances presented no possible reasons for application of the brakes. Each time this occurred without warning. This drive sees relatively low traffic levels thankfully, as this issue would likely cause collision with following vehicles not being able to stop fast enough from random hard braking at highway speeds. Our regular driving occurs on more traffic filled roads. Due to this issue the system cannot be used as an accident would be eminent. Vehicle has not yet been inspected by the dealer or independent service center. The vehicle has not yet been inspected by the manufacturer, police, or insurance representatives. No warning lights or messages occur prior to this failure. | Crash: No Fire: No | Unknown |
| Feb 2022 | UNKNOWN OR OTHER,SERVICE BRAKES,FORWARD COLLISION AVOIDANCE | I was driving with the autopilot engaged and the car unexpectedly slammed on the brakes. This happen 2 more times and I disengaged the autopilot and did not turn it back on. | Crash: No Fire: No | Unknown |
| Feb 2022 | FORWARD COLLISION AVOIDANCE | Phantom braking on 2 lane roads when using adaptive cruise control or autopilot. | Crash: No Fire: No | Unknown |
| Feb 2022 | SERVICE BRAKES,FORWARD COLLISION AVOIDANCE | WHEN IN AUTO PILOT MODE, THE CAR WOULD SUDDENLY BRAKE FOR NO APPARENT REASON. I WAS AFRAID I WOULD GET HIT FROM BEHIND. I REPORTED THIS TO THE LOCAL TESLA DEALER. HE ACKNOWLEDGED OTHERS HAVE HAD THE SAME PROBLEM. THERE IS NOTHING THEY COULD DO NOW. | Crash: No Fire: No | Unknown |
| Feb 2022 | SERVICE BRAKES,FORWARD COLLISION AVOIDANCE | Today I was driving to work using the adaptive cruise control and the car applied brakes for no valid reason, this sudden deceleration could have caused an accident. | Crash: No Fire: No | Unknown |
| Feb 2022 | FORWARD COLLISION AVOIDANCE | The contact owns a 2022 Tesla Model Y. The contact stated while driving 65 MPH with the cruise control engaged, while another vehicle was approaching on the left, the vehicle started to decelerate independently. The contact stated that an unknown warning light was illuminated. The contact was able to continue driving but experienced the failure to the extent of manually braking and stopping the vehicle. The contact scheduled an appointment for the dealer to inspect the vehicle. The contact became aware that the Tesla App detected no failure and canceled the appointment. The contact stated that the Tesla App had canceled appointments on four different occasions. The vehicle was not diagnosed or repaired. The manufacturer had been informed of the failure by the Tesla App. The failure mileage was approximately 300. | Crash: No Fire: No | 300 |
| Feb 2022 | UNKNOWN OR OTHER | The driver reported while slowly turning right into a parking stall, the vehicle accelerated and collided into a building despite efforts to stop the vehicle. The driver stated she was applying the brakes as the vehicle accelerated. | Crash: Yes Fire: No Towed | Unknown |
| Feb 2022 | FORWARD COLLISION AVOIDANCE | The contact owns a 2022 Tesla Model Y. The contact stated while driving approximately 73 MPH, the vehicle experienced “Phantom Braking” while the cruise control was activated and the vehicle slowed to 20 MPH. The vehicle was not taken to a local dealer. The manufacturer was informed of the failure. The contact was informed that the remedy for NHTSA Campaign Number: 22V063000 (Electrical System, Forward Collision-Avoidance) was not yet available. The failure mileage was 3,338. | Crash: No Fire: No | 3,338 |
| Feb 2022 | FORWARD COLLISION AVOIDANCE | I am experiencing regular "phantom braking" by the Autopilot adaptive cruise control. This creates sudden and dangerous hard braking at highway speeds between 55 and 65 mph with abrupt deceleration to the low 40's and has nearly resulted in rear end collisions with trailing vehicles. I received a notice that my vehicle was being built with camera "Tesla Vision" sensors only and does not have the radar sensors of previous Model Y vehicles. Several over the air software updates since my delivery Jan 3 have failed to improve reliability of this system. | Crash: No Fire: No | Unknown |
| Feb 2022 | FORWARD COLLISION AVOIDANCE,LANE DEPARTURE | The car is automatically braking when it sees an approaching vehicle in the oncoming lane (2 lane highway). It puts me at risk for being hit from behind due to unexpected braking. The car is also occasionally going into the left turn lane instead of staying straight when on auto drive mode and then quickly veering out of the lane. | Crash: No Fire: No | Unknown |
| Feb 2022 | FORWARD COLLISION AVOIDANCE | I was driving on freeway 101 north in LA and was merging into freeway 405 north. My auto pilot was engage and my car suddenly Decided to apply the breaks and reduce the speed from 65 miles per hour to around 45. This was done while there were no visible vehicles in sight and in front of me. The auto pilot is quite unreliable. | Crash: No Fire: No | Unknown |
| Feb 2022 | SERVICE BRAKES,FORWARD COLLISION AVOIDANCE | While traveling on I81, at dusk, on 2/16/2022, the car automatically, with no apparent reason, momentarily applied the brakes, causing the car to veer sharply to the left - this occurred twice on a 20 mile trip - once while passing a semi, once while driving under an overpass - autopilot was with ‘distance speed control’ on - car is new with less than 300 miles (purchased on 2/11) - Because to this, I consider the car to be unsafe to drive with autopilot ‘on’ | Crash: No Fire: No | Unknown |
