Mar 2022 FORWARD COLLISION AVOIDANCE The traffic-aware cruise control triggers constant forward collision warnings on my commute, and the system highlights as threats vehicles that are parked on the side of the road safely out of the path of traffic. The warning activates automatic emergency braking, which results in a collision hazard to vehicles behind me due to unexpected and unpredictable heavy braking. I stopped using the feature after the fifth such incident within the same approximate 7 mile trip. I have read about this safety issue previously and am aware that it is under investigation by the NHTSA, but did not realize it was this bad.

Mar 2022 VISIBILITY The contact owns a 2022 Tesla Model Y. The contact stated while driving on the highway at an undisclosed speed, the rear driver's side window detached from the vehicle. There were no warning lights illuminated. The local dealer was not contacted. The vehicle was not diagnosed or repaired. The manufacturer was contacted but no additional assistance was provided. The failure mileage was approximately 5,950.

Mar 2022 FORWARD COLLISION AVOIDANCE The Autopilot feature malfunctioned twice on one drive today, while passing slower moving large vehicles on a divided highway. As I was passing these vehicles, the adaptive cruise control rapidly decelerated, which required intervention in my part to avoid the cars behind me from hitting me. This has not been reported to dealer, has not been inspected, and no warnings occurred before, during or after the events.

Mar 2022 SERVICE BRAKES,FORWARD COLLISION AVOIDANCE This car indiscriminately applies the brakes in a very aggressive manner on all manner of roads including multiple lane divided highways. It will regularly slow from 75 mph to under 50 mph while driving on interstate highways for no reason whatsoever. I have come close to being rear ended at least 5 times and on a recent trip to Arizona and back. One incident nearly caused a multiple car accident. This is an unacceptable fault and is very very dangerous. Tesla needs to be forced to address this problem immediately. My car does this all the time. If you want to come drive it just say the word.

Mar 2022 SERVICE BRAKES,FORWARD COLLISION AVOIDANCE When driving on a two lane highway today, I had 3 episodes of the car applying the brakes to suddenly decrease speed when a vehicle was in the oncoming lane. I also had another event of emergency braking when a car was crossing the highway on a perpendicular road far ahead of my car. When I reached that road, the car had already safely crossed. Cruise control is unusable on the Tesla because of these events.

Mar 2022 SERVICE BRAKES,FORWARD COLLISION AVOIDANCE While using adaptive cruise control on the highway, the car applied the brakes on its own. There were no cars in front of us, nor on the side of us. We were very lucky there was no one behind us or we would have been rear-ended. We went from 70 mph to 65 mph in a matter of an instant. I was able to apply the gas to stop the braking. There were no warnings or messages ahead of the incident.

Mar 2022 VEHICLE SPEED CONTROL,FORWARD COLLISION AVOIDANCE Just got the Model Y and drove from Kansas City to Denver and back to Kansas City. When using the cruise control the car would suddenly break for no apparent reason. The breaking rapidly decreases the speed of the car. This would occur multiple time without any warning or apparent consistency. Talking with other Tesla drivers at charging stations, this seems to be a well-known and common problem. Because of the unpredictable nature of this, I feel it is unsafe to use the cruise control when there are other cars present, especially cars that are behind the vehicle. I have no idea if the break lights come on with the phantom breaking occurs. If they do not, this would make the situation even worse. Cruise control has been around for years--it should work properly and **safely** on such a high-end automobile.

Mar 2022 FORWARD COLLISION AVOIDANCE While using Autosteer, the automatic emergency braking engages almost every time we pass by a semi-trailer truck and while there is oncoming traffic on a two-lane road. This sudden and unprovoked engagement of the automatic emergency brake system has almost caused us to be rear ended three times over the last four days and is officially causing concern. This issue will be reported to Tesla. The problem has not been reproduced by the manufacturer or service center yet. The vehicle has not been inspected by the dealership since we drove it off the lot 3 weeks ago. Warning indications do not go off all the time

Now I don’t feel safe using it and I am only able to use the feature when I am sure there is no one behind me. One incentant almost resulted in rear end collision. Car thought something was there and applied break abruptly and person behind was following closely and almost crashed into me. Since then I am not able to trust this feature. A major software flaw for such an expensive car. And a very serious safety concern.

Mar 2022 VEHICLE SPEED CONTROL,SERVICE BRAKES,FORWARD COLLISION AVOIDANCE My car has problems with uncommanded slowdowns while cruise control and/or Autopilot are active. The vehicle will slow down suddenly in any number of predictable and unpredictable situations. The extent of the slowdown is variable as well. It can range from a slight “hiccup” in the speed, which is simply unpleasant, to a significant, rapid decrease. (Like, more than 10mph, very quickly.) This can occur at various times of day, though I don’t drive much at night (it has happened then as well). Driving on any two lane road with a vehicle following at any time is risky and in my opinion hazardous. Particularly if the vehicle following is even remotely close. If the following vehicle isn’t paying attention and the car decides to brake for whatever reason (and apparently there are LOTS) that’s a collision waiting to happen. I have refused to use my car’s cruise control system since shortly after I got the car due to this issue. I try it every once in a while to see if it’s any better, but it hasn’t made it more than 5 miles before another event. Not only is it a terrible experience for passengers, it’s unpleasant to have to keep my foot above the accelerator pedal to react to the stupid braking the car will do. Some of the predictable things that cause braking can include, not limited to - oncoming vehicles on a 2-lane (especially trucks, and almost everything bigger than an F150), many larger mailboxes, and in some parts of my commute - simply nothing at all. I don’t know what the car sees, but it brakes. Less predictable are the times it will brake for shadows. Can’t use cruise control when there’s trees shadowing the road. It is absurd. No vehicle this day and age should risk causing an accident because of random shadows!! To be clear, I’ve never had a problem in a divided highway. Everywhere else, though, forget it.

No other vehicles were nearby, no nearby structures, no paper or debris drifting on the road. This happened about 6 to 8 times over two days. I am glad that no one was following me.

Mar 2022 UNKNOWN OR OTHER,FORWARD COLLISION AVOIDANCE Tesla Y has a special setting for carwashes, that driver has to enable, which puts the car in neutral, closses all the windows and plus some more. Unfortunately system failed; car accelerated on its own and I hit a car in front of me, so it failed the second time and car didn't stop on its own to avoid collision. I called Tesla and this incident is under their investigation.

Mar 2022 VEHICLE SPEED CONTROL,FORWARD COLLISION AVOIDANCE While cruise control is engaged, the vehicle will slam on the brakes when there are no obstacles or vehicles nearby. It happens out of nowhere and without any warning from the vehicle. We live in a more rural area and has happened on the interstate there each time. It is not a highly congested road and each time it has happened, there is nothing around. It has happened on numerous occasions and will take the car from 80 mph and attempt to stop completely. This could easily cause a rear end collision at high speeds if there had been a vehicle in close proximity. This has also happened even with the Automated Emergency Braking disabled.

There is NO object or other vehicle in the way when this happens. It has happened many times starting in December 2021 in a brand new vehicle and happened most recently 3 days ago. Every time it happens it is scary. Very disturbing and renders cruise control unusable. No word from Tesla that this is fixable or even a priority. Please make Telsa address this serious safety hazard before someone is hurt or killed. Thank you.

The contact stated while driving 70 MPH with the Self Driving feature activated, the vehicle experienced phantom braking. The dealer was made aware of the failure and informed the contact that the vehicle was in learning mode and needed a software update. The vehicle was not repaired. The manufacturer was not made aware of the failure. The failure mileage was approximately 50.

Mar 2022 FORWARD COLLISION AVOIDANCE I have owned my car for only 2 weeks. Every single night that I am driving home from work (50 miles away), I have significant phantom braking. Most of the time I lose around 5-10mph, which is startling and could cause an accident if someone behind me wasn't paying attention, but once last week, while going 70mph I dropped almost 20mph within a second. Luckily, there were not any cars directly behind me. If there had been, I would have been rear ended. It is extremely frightening and only happens when I am using the cruise control. Every time it has happened, there is nothing in front of me. It's on a stretch of interstate that is 2 to 3 lanes wide, on both sides, with a large grassy median. No possible on coming traffic. I have not received any warnings or messages before, during or after.

Mar 2022 FORWARD COLLISION AVOIDANCE I just bought a brand new Tesla Y and the next day went on a road trip with my wife and two little kids in the back seat. I put on AutoPilot. While driving it suddenly decelerated, which nearly caused an accident. A tractor trailer almost slammed in the back of our car. This created great anxiety and fear. The next day, with me driving alone, I tried it again and this "phantom braking" occurred. This is very dangerous. PLEASE pressure Tesla to look into this. Why do we need a tragedy for NHTSA to investigate?? This needs to occur now.
