Apr 2022 ELECTRICAL SYSTEM,VEHICLE SPEED CONTROL,ENGINE We were on the highway the power went out. The car did not have emergency brake . We could have gotten hit. We finally was able to stop on the shoulder with the driver not letting go off his foot pedal to keep the car from moving. We called Tesla and he wasn’t able to restart the car and yet he said the car has lots of power. The back door doesn’t open in an emergency as well

Apr 2022 VEHICLE SPEED CONTROL,SERVICE BRAKES,FORWARD COLLISION AVOIDANCE Car set at 75 MPH cruise control, brakes unnecessary with no obstructions or danger, and reduces speed to 45 MPH while having adaptive cruise control active. Tesla response = this is a Beta feature. Read your manual. This is incredibly dangerous and a rear end collision waiting to happen. I feel like this is reckless and to just blame it on being Beta is borderline criminal.

Apr 2022 VEHICLE SPEED CONTROL,FORWARD COLLISION AVOIDANCE While on cruise control, the car will, out of nowhere, apply the brakes very aggressively ("phantom breaking"). This despite there being no vehicles nearby or any potential threats to safety. This phantom braking is obviously very dangerous and is a reason why I don't really use it anymore. It has happened 3-4 times since I bought my car, and that's despite not really using cruise control very often (for this reason). Clearly there's a software fault of some kind

Apr 2022 FORWARD COLLISION AVOIDANCE The contact owns a 2022 Tesla Model Y. The contact stated while driving 70 MPH, the vehicle experienced the phantom braking system failure. The contact stated no warning light was illuminated. The contact was able to continue driving to his destination. The contact stated he experienced the failure twice. The vehicle was not diagnosed or repaired. A dealer was not contacted. The manufacturer had been informed of the failure via the App. The failure mileage was 1,790.

Apr 2022 FORWARD COLLISION AVOIDANCE When the car is in cruise control or auto pilot, it is performing a lot of phantom breaking. I.e breaking when there are no cars or other obstructions in front of the vehicle. This is a safety hazard for those vehicles that may be directly behind my vehicle increasing the risk of a rear end collision.

Apr 2022 VEHICLE SPEED CONTROL,SERVICE BRAKES,FORWARD COLLISION AVOIDANCE On April 15, 2022, I experienced two incidents of sudden braking while in cruise control on a round-trip four-hour drive in my Tesla model Y. I was driving at between 60 and 65 miles an hour on a four-lane road. Between the incidents, I reached my destination and the car was parked for several hours. In each case, there was no car in front of me in my lane, and in one case, as I remember, there were no cars in front of me anywhere near. The car slowed suddenly by 10 to 15 miles an hour, as if I had taken my foot off the accelerator. I applied pressure to the accelerator to return to cruising speed. In each case, the road was dry and the sky was largely cloudless. There was no warning. I bought the car new in January 2022. I have not had the car inspected since the incident. I did investigate on the Internet and learned that there have been many other such incidents with the Tesla Y and 3 models. I understand that the NTSB has an open investigation on the subject. The problem is very serious. Fortunately, there was no one traveling close behind me, or the sudden slowdown might have caused the following car to crash into me.

Apr 2022 POWER TRAIN There is no sound warning to switch gears. For example from forward to reverse. I was trying to go forward and I was actually in reverse. The gear change is on the steering stalk and doesn’t make any warning sound when switching. As the drivers eyes are looking forward, and not looking at the screen feedback, it makes it difficult to remember which gear you are in. Forward or reverse. I assumed I was going to move forward, but the car was actually in reverse. I hit the car that was parked behind me and did damage to my car. I’m feel that most cars makes a physical click sound with the gears shifting as well as a small jump in movement to remind the driver they have switched directions.

Apr 2022 FORWARD COLLISION AVOIDANCE Car randomly applies hard braking while traveling on FSD (full self driving) mode on freeway with no car or obstruction in front or back in current or adjacent lane. The brake is applied suddenly without any warning, and can result in accident if any vehicle is traveling behind.

Apr 2022 SERVICE BRAKES,FORWARD COLLISION AVOIDANCE When operating my vehicle in adaptive cruise control or autopilot, my model Y has continuously had incidents where it randomly slams on the brakes as if it’s attempting to avoid an obstacle. To my frustration, there is nothing in the road and I am slammed back in my seat. What I fear the most is that a vehicle following behind me could potentially rear end me due to this braking issue. I just took a 450 mile road trip, and this incident happened a dozen times along the way. I read online and this appears to be a trend with Tesla and I want to make sure this “phantom braking” issue is reported.

Apr 2022 FORWARD COLLISION AVOIDANCE My Tesla Model Y has an ongoing problem with unexpected and unnecessary automatic braking when there is no apparent reason for the system to act--there is never anything in the lane ahead to brake for. The car just brakes and rapidly slows down at highway speeds. This has not resulted in a collision yet, but I'm fairly certain that it could result in a rear end collision if the driver following my car is "tailgating." At the very least, this is likely annoying to drivers following my car when this occurs. This is a very unnerving experience and should be fixed by the manufacturer. Tesla owner forums refer to this phenomenon as "phantom braking." This only seems to occur when I'm using the adaptive cruise control and/or the autopilot features on the vehicle.I have noticed this happening often on two lane and four lane highways, but not on freeways to date. I recall this happening every time I drive for any distance using the cruise control and/or autopilot. When I turn off the cruise control and/or auto pilot, it stops.

Apr 2022 VEHICLE SPEED CONTROL,FORWARD COLLISION AVOIDANCE while vehicle in autopilot mode and driving on 2 lane highway, vehicle would suddenly brake hard for no reason to do so. The same would occur on the freeway at the beginning of a bend in the road. This phenomenon commonly occurred and I have stopped using the autopilot feature for fear of being hit from behind if the vehicle suddenly braked for no apparent reason. No previous warning lamps or messages when the problem first occurred. I did see messages on 4/1 during my drive that some of the cameras on the car were disabled or blocked and the messages disappeared and reappeared several times for about an hour and then stopped appearing. I have not had these messages reappear since. I have a service appointment scheduled at the end of April.

Apr 2022 VEHICLE SPEED CONTROL,FORWARD COLLISION AVOIDANCE Highway driving using cruise control and car suddenly brakes hard. Has happened 3 times. All the incidents seem to be on slight hills with no traffic/nearby cars or road hazards. Took place in remote parts of New Mexico on Interstate 25. The braking is hard enough to cause an accident or to be rear ended due to the fast deceleration.

Mar 2022 FORWARD COLLISION AVOIDANCE The Autopilot feature malfunctioned twice on one drive today, while passing slower moving large vehicles on a divided highway. As I was passing these vehicles, the adaptive cruise control rapidly decelerated, which required intervention in my part to avoid the cars behind me from hitting me. This has not been reported to dealer, has not been inspected, and no warnings occurred before, during or after the events.

Mar 2022 FORWARD COLLISION AVOIDANCE The traffic-aware cruise control triggers constant forward collision warnings on my commute, and the system highlights as threats vehicles that are parked on the side of the road safely out of the path of traffic. The warning activates automatic emergency braking, which results in a collision hazard to vehicles behind me due to unexpected and unpredictable heavy braking. I stopped using the feature after the fifth such incident within the same approximate 7 mile trip. I have read about this safety issue previously and am aware that it is under investigation by the NHTSA, but did not realize it was this bad.

Mar 2022 VISIBILITY The contact owns a 2022 Tesla Model Y. The contact stated while driving on the highway at an undisclosed speed, the rear driver's side window detached from the vehicle. There were no warning lights illuminated. The local dealer was not contacted. The vehicle was not diagnosed or repaired. The manufacturer was contacted but no additional assistance was provided. The failure mileage was approximately 5,950.

Mar 2022 SERVICE BRAKES,FORWARD COLLISION AVOIDANCE When driving on a two lane highway today, I had 3 episodes of the car applying the brakes to suddenly decrease speed when a vehicle was in the oncoming lane. I also had another event of emergency braking when a car was crossing the highway on a perpendicular road far ahead of my car. When I reached that road, the car had already safely crossed. Cruise control is unusable on the Tesla because of these events.

Mar 2022 SERVICE BRAKES,FORWARD COLLISION AVOIDANCE This car indiscriminately applies the brakes in a very aggressive manner on all manner of roads including multiple lane divided highways. It will regularly slow from 75 mph to under 50 mph while driving on interstate highways for no reason whatsoever. I have come close to being rear ended at least 5 times and on a recent trip to Arizona and back. One incident nearly caused a multiple car accident. This is an unacceptable fault and is very very dangerous. Tesla needs to be forced to address this problem immediately. My car does this all the time. If you want to come drive it just say the word.

Mar 2022 SERVICE BRAKES,FORWARD COLLISION AVOIDANCE While using adaptive cruise control on the highway, the car applied the brakes on its own. There were no cars in front of us, nor on the side of us. We were very lucky there was no one behind us or we would have been rear-ended. We went from 70 mph to 65 mph in a matter of an instant. I was able to apply the gas to stop the braking. There were no warnings or messages ahead of the incident.

Mar 2022 VEHICLE SPEED CONTROL,FORWARD COLLISION AVOIDANCE Just got the Model Y and drove from Kansas City to Denver and back to Kansas City. When using the cruise control the car would suddenly break for no apparent reason. The breaking rapidly decreases the speed of the car. This would occur multiple time without any warning or apparent consistency. Talking with other Tesla drivers at charging stations, this seems to be a well-known and common problem. Because of the unpredictable nature of this, I feel it is unsafe to use the cruise control when there are other cars present, especially cars that are behind the vehicle. I have no idea if the break lights come on with the phantom breaking occurs. If they do not, this would make the situation even worse. Cruise control has been around for years--it should work properly and **safely** on such a high-end automobile.
