However, the auto wipers have a high false positive detection rate, which leads to the wipers unnecessarily turning on while I'm driving. Even after exiting adaptive cruise control, the wipers stay on auto. This is extremely problematic because it makes me have my eyes + one one hand off of the wheel to turn the wipers off. | May 2022 | FORWARD COLLISION AVOIDANCE | I was driving West bound an the 91 Freeway, just about 1 miles East of the 241 Fasttrack at about 5:30 PM on 30 April 2022. The 241 Fasttrack was shut down for road work and the traffic was averaging about 4 miles and hour, but very stop and go traffic, not a smooth flow. I put the can into dynamic cruise control -- this was done by press down twice on the drive shifter on the right side of the steering wheel. I then took the car out of dynamic cruise control by pressing the drive shifter up once. But I much have forgot I did so as I press the drive shifetr up once again, which puts the can into reverse. I pressed the gas pedal and the car started going backward and I crashed into a vehicle -- I believed the car was in Drive, not Reverse. No one was hurt. My car was undamaged; his 2004 Toyota Corolla had some cracked paint and the left driver side bumper was push back about 3/8th on an inch. I paid the man $450 and he will contact me if he needs more. My complaint is this: the drive selector is being used for drive selection AND engaging and disengaging dynamic cruise control -- it may me to easy to make a mistake where the drive thinks he took it out of dynamic cruise control, but in fact put it into reverse. I do not think the same motion for turning off dynamic cruise control and putting the car in reverse should be on the some stick with the same motion. The only indication is a audible 'tone' when disengaging the dynamic cruise control, but road noise, cabin noise, radio music can be that tone less noticeable. I had the car on 'joe' mode which decreases the tone sound. I don't think 'joe' mode is a safe function as it makes critical sounds less noticeable. The dynamic cruise control sounds should not have to compete with the radio -- the radio should be silenced when the dynamic cruise control tones occur. | Crash: Yes Fire: No | Unknown |

I was driving at between 60 and 65 miles an hour on a four-lane road. Between the incidents, I reached my destination and the car was parked for several hours. In each case, there was no car in front of me in my lane, and in one case, as I remember, there were no cars in front of me anywhere near. The car slowed suddenly by 10 to 15 miles an hour, as if I had taken my foot off the accelerator. I applied pressure to the accelerator to return to cruising speed. In each case, the road was dry and the sky was largely cloudless. There was no warning. I bought the car new in January 2022. I have not had the car inspected since the incident. I did investigate on the Internet and learned that there have been many other such incidents with the Tesla Y and 3 models. I understand that the NTSB has an open investigation on the subject. The gear change is on the steering stalk and doesn’t make any warning sound when switching. As the drivers eyes are looking forward, and not looking at the screen feedback, it makes it difficult to remember which gear you are in. Forward or reverse. I assumed I was going to move forward, but the car was actually in reverse. I hit the car that was parked behind me and did damage to my car. The car just brakes and rapidly slows down at highway speeds. This has not resulted in a collision yet, but I'm fairly certain that it could result in a rear end collision if the driver following my car is "tailgating." At the very least, this is likely annoying to drivers following my car when this occurs. This is a very unnerving experience and should be fixed by the manufacturer. Tesla owner forums refer to this phenomenon as "phantom braking." This only seems to occur when I'm using the adaptive cruise control and/or the autopilot features on the vehicle.I have noticed this happening often on two lane and four lane highways, but not on freeways to date. The same would occur on the freeway at the beginning of a bend in the road. This phenomenon commonly occurred and I have stopped using the autopilot feature for fear of being hit from behind if the vehicle suddenly braked for no apparent reason. No previous warning lamps or messages when the problem first occurred. I did see messages on 4/1 during my drive that some of the cameras on the car were disabled or blocked and the messages disappeared and reappeared several times for about an hour and then stopped appearing.
