All of a sudden the car did an automatic hard brake and the car started to veer toward the center divider. The car only had 64 miles and was brand new. This was the first time I had used autopilot and had been on the freeway. There was a truck on my right and it was a 2 lane freeway. Nobody has inspected the car.

Jun 2022 FORWARD COLLISION AVOIDANCE While on autopilot my car will randomly brake hard. This issue seems to occur when passing a semi truck on a curve or narrow roadway; when shadows or pavement color creates a clear contrast, and other instances for seemly no good reason.

Jun 2022 FORWARD COLLISION AVOIDANCE While having the autopilot engaged (speed and lane control) with no vehicle near by, the autopilot reduced the speed, aggressively, by applying the brakes. I immediately stepped on the gas pedal to override the braking. This has occurred a few times (3-4 times), since it was first experience. Additionally, the autopilot will engage the brakes when there is an oversized vehicles (mainly RV's) in the next lane, even if the oversized vehicle is properly in their own lane.

Jun 2022 SERVICE BRAKES,FORWARD COLLISION AVOIDANCE We rented this vehicle for 2 days, in which we experienced phantom braking thrice. It was seemingly the forward collision detection. The first time was after a car had fully crossed the highway at a reasonable distance ahead of us. The second time was when it detected cyclists along the shoulder, with no danger of collision. The third time was as we approached a sharp turn in the highway, where a semi-truck was parked/stationary on the shoulder mid-turn. In each of these instances there was no need to slow the vehicle, but the car suddenly braked almost to a complete stop. Cars around and behind us were surprised and we were forced to swerve. It is unclear if the brake lights engage during these sudden phantom braking incidents.

Jun 2022 FORWARD COLLISION AVOIDANCE It is the phantom braking issue. When I was using autopilot driving at high way at around 70 miles per hour and a car behind was approaching my car, I passed a car at my right lane and changed to the right lane after I turned on the turn signal and the autopilot mode was changed to cruise control mode. As soon as I completely changed the lane, the car decrease the speed suddenly. It is dangerous since the car behind my car was close, so I had to increase the car speed by push the speed paddle. No injure happened but it is an issue needs to be fixed. I have not reported this to Tesla and have not sent the cat to the service center for inspection yet.

Jun 2022 FORWARD COLLISION AVOIDANCE Phantom Breaking. I was driving just after the sunset on the highway 101 with autopilot/FSD engaged today 06/05/2022 at about 8.15 pm. The car was driving over 70 mph but suddenly it applied break for no obvious reason. There was a truck behind me and I was really afraid I would be rear ended, but luckily I took the control and was able to avert the situation. It is a significant safety issue and I am afraid riding Tesla with autopilot engaged.

Jun 2022 VEHICLE SPEED CONTROL,FORWARD COLLISION AVOIDANCE On June 3rd I was driving on I-80 heading West towards Chicago. The car was on Auto Pilot (Cruise Control) and traveling at 70 mph. Without any warning or warning lamps/messages the braking system suddenly applied the brakes. I had to quickly apply the accelerator. If a truck or car would have been following me closely they would not have had time to react. I am afraid to use the auto pilot in heavy traffic for fear that I could be rear ended as semis always tailgate in Chicago. I have a service appointment set with Tesla on Monday the 6th of June to have this looked at.

Jun 2022 SERVICE BRAKES,FORWARD COLLISION AVOIDANCE "Phantom braking" has been an issue in which it randomly brakes while using cruise control. We have been on flat, wide open roads with no cars or visible obstacles in front of us. We are concern as we have an infant and it jerks everyone forward when it happens. We are on high alert especially when someone is behind us because it might cause an accident. This must be immediately corrected by Tesla.

The contact stated that while driving 72 MPH with the self-driving feature activated, the vehicle experienced phantom braking causing the speed of the vehicle to abruptly decrease. The contact indicated that the failure had occurred on eight separate occasions while driving on the highway the same day with no other vehicles nearby to cause the system to activate. The cause of the failure was not determined. The manufacturer and local dealer were not notified of the failure. The VIN was not available. The failure mileage was 1,000.

Jun 2022 FORWARD COLLISION AVOIDANCE Ever since Tesla removed the radar units in the Model 3 and Y phantom breaking has gotten worse and worse. I have a 2022 Model Y Performance and every time I use the Basic Autopilot, which is glorified adaptive cruise control, it will slam on its breaks for no apparent reason. It happens at night, in the day, and without any warning. I have almost been rear ended multiple times now. Tesla said they were working on a fix but that was over 6 months ago now and it's only getting worse. I am no longer comfortable using that feature with how unsafe it is.

The contact stated that while pulling into a driveway, the vehicle unintendedly accelerated, colliding with a vehicle in front. The contact stated that the vehicle then went backward, crossing the road, colliding with the neighbor's garage door, and pushing it to the side as the vehicle stopped once it collided with the parked vehicle in the garage. The contact stated that no warning lights were illuminated. The contact stated that she suffered from a headache for a couple of days but no medical attention was sought. No one else was injured. A police report was filed. The vehicle was towed to the Tesla collision center in Austin, TX. The vehicle was towed to the collision center where it was still awaiting diagnoses and repairs. The manufacturer was not made aware of the failure. The approximate failure mileage was 400. Consumer's son is filing the complaint on behalf of his mother.*JB

May 2022 VEHICLE SPEED CONTROL,SERVICE BRAKES,FORWARD COLLISION AVOIDANCE We were driving on interstate. We had cruise control engaged but had accelerated to pass a vehicle. The car rapidly braked with no warning, to at least 10 miles lower than our cruise control speed. There were no warnings associated with this rapid braking and there were no risk factors around us at all. This is fortunate, as if someone had been close behind us an accident could have very easily happened. -- This has happened once before, while driving on interstate. Very startling and inexplicable. -- Both times have hit the accelerator to get back to a speed appropriate to flow of traffic.

May 2022 UNKNOWN OR OTHER Child opened door to exit vehicle. Finger was resting near window. Window retracted as door opened and took child's finger with it. Child's finger was trapped and smashed between glass and door. Finger was able to be removed with injury to finger. Window did not pop back up when door was closed in an effort go release the finger.

May 2022 UNKNOWN OR OTHER Dear Sir/Madam, We bought the car from Tesla two weeks ago. We got a flat tire and have been stranded for a while. We been given very conflicting responses. First, Tesla told us that within 24 hours they will send us a tire tech. Second, Tire tech was scheduled, but in the last minute they cancelled. Then, they said Saturday. It was cancelled again. They said Monday. Then, they said we do not have your wheel so need to tow. It has been very frustating.

May 2022 FORWARD COLLISION AVOIDANCE This happened on our first and so far only highway drive since purchasing our 2022 Tesla Model Y LR AWD car new last week. I was driving on I-25 southbound in Wyoming north of Cheyenne. Speed limit was 80 mph (and my speed). I was using adaptive cruise control and Autosteer. Partly cloudy day, no traffic, long stretches of straight highway with gently rolling hills. Collision avoidance braking was turned on but at lowest sensitivity setting. There were several instances of phantom braking where speed dropped slightly (5-10 mph) but resumed instantly. These would not have been a problem unless someone had been tailgating me. One time, same conditions, the collision alert (red flashing lights and alarms) was set off and car did a hard braking on its own from 80 to maybe 50 before I took over control. This was very dangerous and could have caused me to be rear ended if a following driver had not been attentive. In all cases I was NOT near an overpass and there was no surrounding traffic in either southbound lane. There is a considerable distance separating the northbound lanes which had little traffic anyway. I was NOT near an overpass and could see no shadows or dips that could explain the problem. It does not appear to necessarily be a problem with the Autosteer system or ACC but rather the collision avoidance system (which uses the same sensors and algorithms, I know).

May 2022 FORWARD COLLISION AVOIDANCE My vehicle hits the brakes when cruise control or autopilot is on, and it’s pretty frequent. It happens randomly when other vehicles are around but just as often when there are no other vehicles around. We passed a semi truck on the interstate doing 80, moved into the lane in front of it, suddenly my Tesla hits the brakes and goes from 80 to 60 in about a second. The semi was right on our bumper. Now when there’s a vehicle behind us we try not to have the cruise control on. I could go on and on. Bottom line is, it does it all the time, it’s not safe, and Tesla keeps telling me it’s a firmware problem. I added a screenshot of just our last drive to Denver from South Dakota. But it does it all the time.

May 2022 FORWARD COLLISION AVOIDANCE when using cruise control or autopilot the car will brake for no reason. Most often this will occur at highway speeds causing the car to decelerate at a dangerous rate. Yesterday I was driving at 68 mph and the car braked to under 30 mph in less than 2 seconds causing me to almost get rear ended by a semi truck. This will occur when there is no one else on the road in front of my car and no obstacles on the road. It seems to occur any time I use these features regardless of location or speed.This is very dangerous and has made both the cruise control and autopilot unusable on the vehicle.

On a 485 mile trip on a 2 lane highway going one direction no oncoming traffic lanes and no vehicles close by vehicle would suddenly just slow down rapidly 20vto 30 moh. It did this 9 times until I just turned off cruise control.

There were no cars within half a mile, and we had turned on adaptive cruise control. Suddenly, there was a strong brake applied for no apparent reason, which was very disturbing for the entire family. We have never turned on adaptive cruise control since. This appears to be an instance of "phantom breaking" according to our online research.
