Jun 2022 FORWARD COLLISION AVOIDANCE Purchased a Tesla Model Y approx. 3 weeks ago and within the first day experienced sudden, aggressive braking on the freeway with nothing around and no apparent reason for braking at all. This has happened at least ten times in the three weeks we’ve owned the vehicle. It’s very scary when it happens and we’re very concerned it will cause an accident with my family and small child in the back. It happens when the car is in auto pilot and adaptive cruise control. The car gives no alert and no indication that it’s going to brake hard all of a sudden in the middle of the interstate at 70 mph it will instantly drop speed by as much as 15mph or more. Thankfully nobody has been behind us yet. For us this is an extreme safety concern and we have expressed this to Tesla and I have sent out a maintenance technician today who took it for a simple test drive and said there’s absolutely nothing wrong with the car and that’s the safest car in the world and that what we were experiencing is normal behavior for a Tesla and to be expected. He said when it happens to stereo on the accelerator to get it to stop which is ridiculous. This is a known problem generally referred to as phantom braking and I was assured by the sales rep before accepting the car that this doesn’t happen in auto pilot and nothing to be concerned about. Now here we are stuck with a dangerous vehicle that they can’t fix and won’t take it back. Please help get these cars off the road until they fix this huge safety issue. I am shocked there has not been a recall. I will also file a complaint with the state of Washington.

Jun 2022 FORWARD COLLISION AVOIDANCE I was traveling on a highway when my Tesla Model Y abruptly slammed the brakes for no reason while Autopilot was engaged. The vehicle slowed down very aggressively, this could have caused an accident if there was another vehicle following close behind me. After the first incident, I blamed a dirty windshield/camera lens blocking the systems ability to see what was ahead. But it happened two other times on different days. At this point I was convinced that there is an issue with Tesla's Forward Collision Warning system. On a different occasion, I was driving under an overpass and the shadow on the pavement (overpass shadow) triggered the collision warning and once again aggressively applied the brakes. It is very unfortunate this is happening on this vehicle. I love my Tesla Model Y, but I am very hesitant to use Autopilot due to these issues. I personally like Tesla because they are using advanced technology to make driving a lot safer for everyone on the road. However, it is our responsibility to report these issues so the manufacturer can investigate the root cause. I hope this comes to a solution soon because it is a very dangerous situation that can cause harm to me and my family as well as others on the road. I will point out that I have been fortunate there were no close vehicles in behind me during the three separate times this occurred.

The car became very dangerous, instantly and the car was almost in an accident. The manufacturer has been completely unresponsive.

Jun 2022 VEHICLE SPEED CONTROL,FORWARD COLLISION AVOIDANCE Reporting “phantom braking” issues during 1600 mile cross-country from Kansas to California. During this trip there were approximately 45 incidences of the rapid uncommanded braking while on 4 lane divided highways. All driving was daytime, sunny weather at speeds between 70-80 mph. Braking occurred with both cruise control/auto steer enabled (No FSD), as well as only cruise control enabled.

Jun 2022 FORWARD COLLISION AVOIDANCE With Autopilot engaged (either adaptive cruise or self-steering), the car break unexpectedly on 2 way highways. This usually happened when a semi truck was coming in opposite direction, in its lane or the car in front will move into a left lane and slow down at a safe distance. This had the potential to be rear-ended. Not reported to Tesla or inspected. No prior warnings. There were 6-7 incidents in the span of 3 days of travel from WA state to CA, over 1100 miles, starting on May 18, 2022.

Jun 2022 AIR BAGS,SEATS Tesla Model Y passenger airbags are armed automatically based on the weight in the seat. The manual states the airbag should be on with a person weighing above 100 lbs. The airbag does not turn on consistently for my wife who weighs about 120 lbs. About 80% of the time the airbag remains off. Even when the airbag is on, sometimes it will turn off from a deceleration approaching a stop sign. Took the car in for a service at the Tesla service center and they say airbag staying off is "normal" for a 120 lbs person. They advised we tilt the seat cushion down in the back to help activate the weight sensor. The passenger weight sensor or the program logic of the car is not reliable to turn on the airbag for my wife consistantly.

Jun 2022 VEHICLE SPEED CONTROL,SERVICE BRAKES,FORWARD COLLISION AVOIDANCE I have encountered several "phantom" braking events while driving my Tesla Y in auto-pilot mode, all on the freeway or highway. This most recent event occurred while returning from Las Vegas. In each case, something dark across the road triggered the event. In two cases it was dark asphalt strips that were perpendicular to the highway. in the other case, it was a dark shadow of a bridge across the highway. I assume the car's auto-pilot system thought these things were solid objects and therefore braked. I captured each phantom braking event on the car's built-in camera and am enclosing the screen captures.

Jun 2022 SUSPENSION,VEHICLE SPEED CONTROL,SERVICE BRAKES I was pulling out of the garage and I heard disturbing sound coming from the bottom it sounds like something is breaking. I thought I drove over something like water bottle but a lot louder. So I stopped halfway through the garage and looked under, nothing visible looked around each tire nothing, checked the monitor screen no warning, nothing, so I thought maybe the sound is not coming from the car and I decided to continue, pulling out and made it to the drive way and still a lot of noise sounds like big crack sound. I parked and checked the tires nothing. Looked under the front bumper, it looks like something is a little out of shape but it wasn’t clear. Then I realized I can’t drive like this so I started to drive forward the noise was very loud, barely made it to the original parking spot then I got out of the vehicle, checked again and a lot of stuffs hanging near front wheels, looks like something is wrong with the suspension, the car has only 2000 miles on it. The worst part is that there is not a single warning sign on that big screen which supposed to show If something is not in order. If it wasn’t for the unpleasant noise, my 7 month pregnant wife would have been driving in the middle of a freeway in less than 5 minutes. I am still shaking from how close we were to be a real danger to ourselves and to other drivers on the roads. We got lucky that it happened right before we get to the roads. I want a full safety and defects inspections on the 2022 model Y

All of a sudden the car did an automatic hard brake and the car started to veer toward the center divider. The car only had 64 miles and was brand new. This was the first time I had used autopilot and had been on the freeway. There was a truck on my right and it was a 2 lane freeway. Nobody has inspected the car.

Jun 2022 FORWARD COLLISION AVOIDANCE While having the autopilot engaged (speed and lane control) with no vehicle near by, the autopilot reduced the speed, aggressively, by applying the brakes. I immediately stepped on the gas pedal to override the braking. This has occurred a few times (3-4 times), since it was first experience. Additionally, the autopilot will engage the brakes when there is an oversized vehicles (mainly RV's) in the next lane, even if the oversized vehicle is properly in their own lane.

Jun 2022 SERVICE BRAKES,FORWARD COLLISION AVOIDANCE We rented this vehicle for 2 days, in which we experienced phantom braking thrice. It was seemingly the forward collision detection. The first time was after a car had fully crossed the highway at a reasonable distance ahead of us. The second time was when it detected cyclists along the shoulder, with no danger of collision. The third time was as we approached a sharp turn in the highway, where a semi-truck was parked/stationary on the shoulder mid-turn. In each of these instances there was no need to slow the vehicle, but the car suddenly braked almost to a complete stop. Cars around and behind us were surprised and we were forced to swerve. It is unclear if the brake lights engage during these sudden phantom braking incidents.

Jun 2022 FORWARD COLLISION AVOIDANCE Phantom Breaking. I was driving just after the sunset on the highway 101 with autopilot/FSD engaged today 06/05/2022 at about 8.15 pm. The car was driving over 70 mph but suddenly it applied break for no obvious reason. There was a truck behind me and I was really afraid I would be rear ended, but luckily I took the control and was able to avert the situation. It is a significant safety issue and I am afraid riding Tesla with autopilot engaged.

Jun 2022 FORWARD COLLISION AVOIDANCE It is the phantom braking issue. When I was using autopilot driving at high way at around 70 miles per hour and a car behind was approaching my car, I passed a car at my right lane and changed to the right lane after I turned on the turn signal and the autopilot mode was changed to cruise control mode. As soon as I completely changed the lane, the car decrease the speed suddenly. It is dangerous since the car behind my car was close, so I had to increase the car speed by push the speed paddle. No injure happened but it is an issue needs to be fixed. I have not reported this to Tesla and have not sent the cat to the service center for inspection yet.

Jun 2022 VEHICLE SPEED CONTROL,FORWARD COLLISION AVOIDANCE On June 3rd I was driving on I-80 heading West towards Chicago. The car was on Auto Pilot (Cruise Control) and traveling at 70 mph. Without any warning or warning lamps/messages the braking system suddenly applied the brakes. I had to quickly apply the accelerator. If a truck or car would have been following me closely they would not have had time to react. I am afraid to use the auto pilot in heavy traffic for fear that I could be rear ended as semis always tailgate in Chicago. I have a service appointment set with Tesla on Monday the 6th of June to have this looked at.

Jun 2022 SERVICE BRAKES,FORWARD COLLISION AVOIDANCE "Phantom braking" has been an issue in which it randomly brakes while using cruise control. We have been on flat, wide open roads with no cars or visible obstacles in front of us. We are concern as we have an infant and it jerks everyone forward when it happens. We are on high alert especially when someone is behind us because it might cause an accident. This must be immediately corrected by Tesla.
