| Jul 2022 | FORWARD COLLISION AVOIDANCE | While driving on highways /interstates and using the vehicle cruise control or autopilot, the vehicle will suddenly apply the braking system when there is nothing in front of it. I would list specifics, but it happens every time that we take the vehicle on the highway/interstate, usually within the first 15 minutes. This has happened multiple times on trips, at 80mph (South Dakota) or at 65mph. I have noticed that white vehicles tend to trigger it more than others, even if they are just sitting on the side of the road or parked in a driveway off of the highway. Hills also seem to throw it off, when you are at the top and just over the crest, it has applied the brakes multiple times. This has gotten progressively worse since I've owned the vehicle (Dec 2021) which is why I am only now reporting it. I will no longer use it as I firmly believe me or my family would be seriously injured or killed if there is a vehicle behind me, especially at 80 mph that was not expecting the car to brake hard and as a result rear ended me. This is a significant vehicle vision system issue and it really does need to be addressed before lives are lost, if they have not been already. There are no other issues with the vehicle, no warning lights on and absolutely no indication the vehicle will apply the brakes until you are suddenly lurched forward due to rapid deceleration. I requested get the loaner while they can’t solve the problem but they denied and cancel my service appointment . It dangerous to drive and very unpredictable when the system crash while driving. While driving expect the car will reduce Speed but it not and no speed will display will cause over speed limit. The the pre collision system will not work at all and the system crash every few minutes and screen will shut off it self. On the course of my road-trip it happened more times than we could count. One horrific time was when a white Cadillac Escalade was tailgating me the Tesla threw on the brakes heavily and almost caused the car behind me to hit me. This is very dangerous and the other driver started threatening me and my family harm. I need Tesla to fix this issue with a great sense of urgency. This is extremely dangerous and in my case we were lucky to not sustain any physical harm or death but the chance was and is still great if they do not fix this issue with autopilot braking randomly not for just myself and my family but countless of other Tesla owners that are affected by this scary unsafe issue. Random braking occurred again on 07/05/22 while on a highway with no traffic. This time consumer's model y was on cruise control. The manufacturer informed the contact to take the vehicle in for service. The contact stated while driving approximately 65 MPH, the computer inadvertently shutdown and the steering wheel was difficult to maneuver. The vehicle then drove off the road, and the contact drove over a grassy area and the curb. The TPMS warning light illuminated. The front axle was completely fractured. The police were not initially on the scene; however, the police arrived the following day. A police report was not filed. There were no injuries sustained. The contact attempted to have the vehicle towed several times, but was unsuccessful; however, the vehicle was finally towed to the dealer. The vehicle was not diagnosed or repaired. After the contact consistently discussed the failure; he was informed by the insurance company that the vehicle was deemed a total loss. The contact received a notification for NHTSA Campaign Number: 22V296000 (Back Over Prevention, Equipment) and related the failure to the recall. Phantom breaking occured on July 6 in Sacramento on Business 80, two incidents about 20 minutes apart. First incident, vehicle suddently slowed from about 65 to 30. Second incident, vehicle suddently slowed from about 65 to 10 (almost stopped) where we almost rearended. The driver was extremely angry as evidenced when he changed lanes to pass us (can't blame him), as Tesla began to reaccelerate. I have heard of phantom breaking incidents, but twice in one short trip where the second had the potential for a serious accident would indicate to me that Tesla needs to take action. | Jul 2022 | VEHICLE SPEED CONTROL,FORWARD COLLISION AVOIDANCE | On July 2nd, 2022 I was driving back from a family visit in Palo Pinto County, TX and had auto pilot engaged for part of the drive. In a span of 40 minutes of that drive on a continuous stretch of Interstate 20 headed east to Dallas the car softly automatically braked once, and twice more strongly braked. In one of those instances I was near another car (near is defined as within 1/8th to 1/4 of a mile) and in both of the other instances I had NO one else within close proximity that could've caused a "reaction". That stretch of i20 is 2 lanes each direction with a median in between, no oncoming vehicles are within at least 75 - 100 feet. In the instance with the car closer, had I not been more reactive and paying attention closely, this WOULD have lead to an accident with another vehicle. In the instances with no traffic IMMEDIATELY near me.. given that rate of speed, it could STILL lead to an accident by over-reaction from others not expecting to see someone slamming on their brakes in a 75MPH zone. First several times it is very uncomfortable to experience, causing concern to be easily hit from the rear in areas with more traffic. The issue is not reproduced as it seems to happen when the car "thinks" it has a reason to brake,different enviromental changes such as cars, in the other lane, or shadows seem to have an affect as well. My wife had just used it shopping. Drove 300 yards with No warnings. Then stopped at a light then when she was accelerating it opened and broke windshield, hood and hinges, scratched left fender inside. She was able to pull over without cause anymore damage to her or anyone else. I then meet her and I was able to drive the car with the hood open and damaged. I then removed the hood completely in order to drive it home all while no safety warnings going off. | Jun 2022 | UNKNOWN OR OTHER,FORWARD COLLISION AVOIDANCE,LANE DEPARTURE | Several times, after over the air software updates, several components of the electrical systems would stop working, including GPS & cameras, resulting in no cruise control, no back up camera, no navigation, no blind spot warning/heads up collision warning/lane departure warning. Essentially, all safety features, cruise control and navigation fail when GPS and cameras fail. In all cases, the reset options provided such as powering off the carf, powering off the screen etc did not help and instead the 12 V battery had to be disconnected, sometimes more than once, before car would reset to a safe state. this requires removal of a panel, removal of cabin filter, removal of cable to battery. This is a time consuming process and could theoretically void warranties. This does not appear to be associate with signage along the road. It appears to happen more frequently in the right most lane. There are certain locations where this problem always occurs, but we cannot determine what is different about that particular exit than others. The most dangerous situations have been when the breaking occurs with a car closely following while driving on an interstate in the mountains (I-70). A relative driving my car was almost rear ended at those speeds while driving on I-70. The breaking is random and unexpected, which makes it more dangerous because as driver you are not prepared for this to occur. At a speed limit of 65mph, when you are driving in a curve and suddenly the car brakes, especially when using regenerative breaking, it is the same as slamming on the brakes. This could cause you to loose control over your car. I started to experience the car braking for no apparent reason. As I continued on my trip and reached a top speed of 75 mph, the car suddenly and without warning slammed on the brakes! It took me a minute to react and regain control of the car. I absolutely would have been rear-ended if anyone had been nearby or possibly caused a chain reaction accident. I am extremely lucky and now very remiss about purchasing this car. I luckily had a passenger who looked up what was happening. We turned off the emergency braking for the remainder of the trip but it is not something you can turn off permanently. You have to turn it off every single time you start a new trip. | Jun 2022 | FORWARD COLLISION AVOIDANCE | On Saturday, 25 June, while driving our 2022 Tesla Model Y north on highway 395 between Lone Pine and Bishop, California, and while cruise control (but not lane assist) was active, I experienced three instances of so-called phantom braking. More precisely, on three occasions within a 10 to 20 minute window, the car automatically braked sharply despite there being no vehicle or other obstacle ahead of me. Each time, the braking was sudden, forceful, and without justification. Had a car been following closely behind me, I could well have been involved in a collision. I stress that my foot was not on the brake pedal when the phantom braking took place. After the 3rd episode, I turned cruise control off and drove another couple of hours (to Mammoth Lakes) without incident. The following day, my wife and I drove the Tesla to our home in the Bay Area. We did not activate cruise control at any point during this approximately 6 hour drive, and experienced no additional incidents. Understandably, we are reluctant to re-attempt the use of cruise control any time soon.
