Dec 2022 STEERING,FORWARD COLLISION AVOIDANCE,LANE DEPARTURE I was driving with my wife and two kids at around 8:30pm (it was dark outside already) on November 29, 2022. The street was pretty empty and it had 2 lanes in each direction. I was driving the left lane with the speed at about 40 mph and had the autopilot (the one that keeps lane, not FSD) turned on. When the car was approaching the intersection with the green light on, it suddenly decided to turn left (even though there was a dedicated left turning pocket on the left hand side). It looks like it was confused and after trying to change the direction it pointed the car right to the road sign. Hopefully I had my hands on a wheel and was watching the situation, so I immediately turned the wheel right to get back to our lane. If I wasted a fraction of second we could end up smashing into the road sign at about 40mph.

Dec 2022 VISIBILITY/WIPER,FORWARD COLLISION AVOIDANCE The wipers "swipe" when passing interstate streetlight when in the "Auto" mode. Additionally, the "Auto" mode is automatically enabled when the adaptive cruise control is set. It is not possible to turn off the wipers while using the cruise control, making it basically useless at night. I have been advised on two occasions by Tesla Service that this is a firmware issue and there is no fix. The complaint was last verified by Tesla Service on Dec. 1, 2022.

Nov 2022 UNKNOWN OR OTHER,LANE DEPARTURE Hi, My main concerned about having a Tesla is not having warning beeping sound when reversing. I almost ran over someone because Tesla rear-view camera doesn't provide virtual alert/audio when getting too close to an object or if a pedestrian decides to walk behind your vehicle while you are about to reverse or pull in reverse into a parking space. I feel this should be a mandatory monitoring system for all Tesla when reversing, just like BMW or ACURA, you do get a beeping warning sound when there's something there or when vehicle is getting too close to any objects. Nov 2022 SERVICE BRAKES Driving down road going about 46-50 mph at about 6:45pm tonight and the car suddenly slammed on brakes. Brakes locked, but thank goodness car behind me wasnt following close. My head/neck was jerked hard enough to cause headache and terrible soreness. I am very shaken. Came home and told my husband, took ibuprofen for my headache. He googled the problem, found this website. We are very concerned, since we own 2 Teslas and it seems people have the same complaints on both models we own.

Nov 2022 SERVICE BRAKES It was snowing and I was driving down a hill going at least 5 miles an hour because there where at 10 cars in front of me. When I went to break the car, the breaks didn’t work and I slid the hill hitting to vehicles. The breaks would skip and wouldn’t lock.

Nov 2022 UNKNOWN OR OTHER,FORWARD COLLISION AVOIDANCE Narrative November 18, 2022 At approximately 8:30 in the morning I was driving my new Tesla Model Y through the rolling hills of South Central Pennsylvania on I-70 heading to Breezewood in order to catch the Pennsylvania Turnpike West. Suddenly and without warning, the car began to break as hard as a car can brake without skidding. Almost like a full panic stop. The car must have stopped in four or five seconds from 60 miles an hour. I was in the left lane at the time and I tried to get over onto the shoulder but I was unable. The car came to a complete stop in the middle of the right lane. The car was in the center of the right lane and was completely locked up. I was unable to get it into neutral. I was unable to get it into a gear so I turned on the emergency flashers. Right behind me were three semis. The first one had to swerve to avoid me. The trailer almost flipped over into me. The semi behind him had to swerve to avoid me and thank God there were no cars in the left lane. The third semi had to take evasive action to get around me. I am grateful those three truck drivers were on the ball at that moment. After they went by, I got out of the car and ran up the hill to flag other trucks and cars to move into the left lane. The car had come to a stop on the other side of the top of a crest so cars and trucks didn't have much notice coming up on my car. Meanwhile, I called 911. I stood at the top of the crest for the better part of an hour and a half, maybe two hours waiting for the police to show, but they never did. I did receive some help from passersby who stopped to help and called 911 as well. After almost two hours, a road crew showed up to lay down flares. And finally a tow truck came to tow me to Crystal Lake PA. Almost 8 hours later I finally got my car into a Tesla dealer in Mechanicsburg PA. When I got there [XXX] the service manager told me they did a preliminary diagnostic and determined that the computer chip in the car was defective. Nov 2022 VEHICLE SPEED CONTROL,FORWARD COLLISION AVOIDANCE,LANE DEPARTURE Vehicle is approximately 9 months old with 13,488 miles. Starting today, the cruise control system which includes the adaptive cruise control and lane keeping assist features has failed. It will not activate, and attempts to activate it result in an error stating "cruise control unavailable." As a result, the car will not brake as it approaches other vehicles; will not maintain its speed; and will not maintain its lane. I made an appointment with the manufacturer's service center to have it inspected and repaired, but the manufacturer cancelled the appointment the same day it was made stating, in writing, that it is a "known issue," "replacing any component will not rectify the issue," and instead it "should be rectified in a future firmware update" without any time frame as to when. I asked is there is a service bulletin or recall notice and was advised that "specifics" cannot be provided "unless otherwise authorized."

Nov 2022 FORWARD COLLISION AVOIDANCE,LANE DEPARTURE After my 3rd trip to tesla service (required 1 month wait each time I've had this problem since the first week) I video and audio recorded the tesla service team dropping my car off in an overflow lot for 2 and a half days before unplugging my dashcam on the 3rd day. 8 hours later I'm at the dealership and they tell me and I quote "left turns isn't a feature of FSD, just try not to use it anymore. By the way, they unplugged your camera. We found nothing wrong with your car after reviewing one whole day of footage, your cars out front." Then I get to the car and plug my dashcam back in immediately and the master key for all the Teslas is in my car (last time the same manager left their phone in my car and harassed me for hours while I was driving). Then I return the key to the manager on video, leave the parking lot, set my home destination, turn on FSD (Full Self Driving) and less than one mile away at the first turn the FSD makes it drives into oncoming traffic. This is probably the tenth time it has happened and I drive my family and passengers in this car so having to constantly bring it in and be told it has no problems after 10 total days in the shop has opportunity cost me between $2,500-$5,000 in lost revenue plus the intentional emotional infliction by gaslighting me the customer for basically not just letting my car kill me and other innocent people who didn't opt into Beta. Elon wants a war with Apple about censorship whenever I post videos of my tesla driving into oncoming traffic on Twitter because Tesla wont fix it after months (or return my $12,000) it gets banned. Same company different boss, now they censor evidence of Teslas malfunctioning and say you're imagining it LOL. You want a war, fine. I have 5,000 gigabytes of dashcam footage with over 1,000 witnesses (passengers). Now they tell me they cant service my car without evidence being submitted but the app is blocking me from submitting photos and they deleted my job applications on Tesla OS.

Nov 2022 VISIBILITY The contact owns a 2022 Tesla Model Y. The contact received notification of NHTSA Campaign Number: 22V702000 (Visibility) however, the part to do the recall repair was not yet available. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The contact stated that after he exited the vehicle, the passenger’s side rear window inadvertently rolled up quickly, and the window failed to detect his son's hand on the window. No injuries were sustained. The vehicle was not diagnosed nor repaired. The manufacturer was not made aware of the failure. The failure mileage was approximately 7,000. Parts distribution disconnect.

Nov 2022 SERVICE BRAKES While driving at freeway speeds, a vehicle cut me off. In order to avoid a collision, I pressed the brake pedal causing all brakes to lock up and the vehicle skidded. I felt the vehicle starting to hydroplane sideways over the dry surface and I heard the screech of the tires. I released the brake pedal, I felt the tires regain traction and I corrected the direction of travel. It was apparent that the ABS system did not function as intended. I contacted Tesla Service and they sent me a message stating that my vehicle needs to be repaired. I am still waiting for the estimate so I do not have specifics as to what needs to be repaired, but since they are having me take the vehicle to their shop, it appears that it can not be repaired over the air. There were no warning lamps or messages prior to the failure.

Nov 2022 FORWARD COLLISION AVOIDANCE,LANE DEPARTURE I've brought the car into the dealership 3 times and they swear they have gone over the whole car and there are no issues with the car but at the first stop light next to the dealership the car starts driving into oncoming traffic which I recorded on dashcam. The car keeps phantom braking (braking for no reason) swerving towards concrete barriers and brick walls, driving into oncoming traffic on roads its navigated before in the same and different conditions. Just yesterday the indicator noises stopped working so I was forced to reset the car while driving my whole family, this is very dangerous and far too common. I have been harassed by Tesla employees who refuse to find any solution. I have been requesting a refund for FSD since the week I bought the car. This is a huge scam and cover up and when you post videos of your car trying to kill you on twitter or IG (post Elon) they shadow ban you. It is against their agenda. Don't believe the lies this is like ENRON x 1,000. FTX is NOTHING compared to the theft and deciet happening at Tesla.

Nov 2022 ELECTRICAL SYSTEM,EXTERIOR LIGHTING The lane departure warning goes off frequently and at random times when there is no issue with it at all. The windows are constantly malfunctioning and turning down when there is nothing obstructing the path of the window. The tail lights will go out at random times and will only be notified by other drivers particularly at night making the vehicle unsafe to operate and unreliable.

Nov 2022 VEHICLE SPEED CONTROL I floored the accelerator, and heard a pop. The pedal then remained depressed about 50% of the pedal travel. I was able to drive the car normally, but only using about 50% of the pedal range to do so. I took it to Tesla for service and after diagnosis, had to replace the accelerator and brake pedal assembly. The mounting plate that held both pedals broke. They said it was impossible to break by stepping on either the brake or accelerator. An “outside force” must have caused the failure and they refused to cover the part and labor under warranty.

Nov 2022 LANE DEPARTURE On 11/17/2022 about 6pm when I drive on highway I -5 from San Diego to irvine, I am driving in the middle of the lane, the car keeps on going to the left and almost hitting the wall when I was on the left lane. I moved to the third lane and the car tried to do the same thing. At one point I lost the control of the car and it suddenly changed to the lane on my left. Fortunately there is no car on my left at that time. I used basic cruise control at that time, not Auto pilot .I have lane departure avoidance on at that time, not sure what's going on. The girl I carpool with said the lanes displayed in the screen is very clear. After I got off the highway, we checked the front camera and it is clean. My husband turned off lane departure avoidance function and restarted the PC of Tesla. We drove in the highway again and Tesla behavior is normal.

Nov 2022 SUSPENSION Went to get my state vehicle inspection and had heard a noise actually from the time I picked the car up new at dealer. The noise sounded like something rattling in the "frunk" down by front driver wheelwell. I thought nothing if it until the state inspector at the Tesla focused shop in Londonderry NH called me from waiting room and showed me on the lift what the noise was. He showed me what I now know is called a "jam nut" on the tie rod end of the front driver wheel. It was not even touching the actual tie rod end to hold it in place. I noticed after he tightened it the steering wheel became tighter slightly in a good way. The noise I heard was gone since picking the car up as well. This concerns me greatly. I am not sure if other nuts or bolts are correctly set on the suspension or even throughout the car as the robot or human device attaching bolts may have had a muscalibrated device and this likely is not limited to one nut on one vehicle unless it was simply gross human incompetence.

Nov 2022 ELECTRICAL SYSTEM,LANE DEPARTURE,FORWARD COLLISION AVOIDANCE I have Full Self Driving I paid $12,000 for and immediately wanted a return upon receiving the car it doesn't work as described. Tesla declined to refund me for the FSD software months ago. I have brought the car into service 3 times now and Tesla each time says there is nothing wrong with the car despite 5,000 gigabits of my own dashcam footage showing the car driving me into oncoming traffic, swerving towards concrete barriers, not seeing 5 people in a cross walk when turning at 25 mph, randomly SLAMMING on the breaks and much more. The touchscreen will freeze leaving the driver with no turn signals, no speedometer etc. I feel like the car is possessed trying to kill me, I am a full time Uber driver. Tesla service manager tells me with a straight face "just don't use it." "just don't make any left turns". The front facing camera keeps rattling so I pulled the Teslacam footage and the camera is so blurry you can barely see anything so I bring it in for service and they say nothing is wrong. This is beyond dangerous this is a dereliction of duty that will result in lost lives, NHTSA you MUST intervene I have already alerted my Governor and State Attorney, they have verified they have received my testimony. I make a choice every day to get in a car I know is trying to kill me or not be able to feed my family. Tesla service manager Prince HARASSED me by calling 15+ times while I was at dinner with my 9 month pregnant wife and threatening me by telling me he's tracking my car to Publix and to my home. Wouldn't take the car out of service mode until I drove 45 minutes back to return his phone; I should have thrown it off a bridge! because he left his phone in my car with a 1 hour timer counting down. This is a cover-up please launch a REAL investigation Tesla is stealing billions Elon makes Enron look like a hill next to a mountain this just ain't right! Last time I checked 3 service trips without a solution qualifies the car as a Lemon and needs to have recalls.

Nov 2022 SERVICE BRAKES,FORWARD COLLISION AVOIDANCE I was driving on I-70 in Colorado, heading westbound. I had the vehicle in basic "cruise control" speed mode. No autosteer or anything like that. Our of the blue, the car jammed on the brakes on its own, as if it was about to hit something. However, there were no vehicles in the area. It was a significant reduction in speed. Probably from 65-35. Luckily, there wasn't anybody behind me. It was scary, to say the least. There were no warnings prior to the episode. I believe that this is referred to as "phantom brakes". Nov 2022 FORWARD COLLISION AVOIDANCE I have been experiencing repeated episodes of phantom braking in which the vehicle will suddenly reduce speed, typically without any other vehicles in the vicinity. I routinely drive from Twin Falls to Boise, which is a distance of about 130 miles, and it regularly happens at least twice. In some instances the speed has dropped suddenly by 15 - 20 mph, which is very frightening, but in other cases the speed only drops by 3 - 4 mph. In all instances there is no reason for the braking. However, the other day it happened with a semi-truck behind me, creating a potentially dangerous situation. This has happened with both cruise control and autopilot. This has been happening since I first purchased the vehicle, which was two months ago. I have 5,000 miles on the vehicle.

Nov 2022 VEHICLE SPEED CONTROL,SERVICE BRAKES,FORWARD COLLISION AVOIDANCE Alert was coming randomly while driving the car about several safety systems being disabled automatically without any audible notifications. Alert clears up after the car is locked for several minutes. Below is the alert message.. Automatic emergency braking disabled Automatic Vehicle Hold disabled Traction control disabled Stability control disabled Cruise control disabled This makes the car suddenly unsafe to drive if you are not paying attention to the screen. I have been on the road driving car at 60-70MPH several times when this happened. This can turn in to an accident. I provided the information and created a service appointment at Tesla which took a month before they can get me a scheduled appointment. Tesla fixed the issue with below note after further review. "A loose connection in the area of the steering gear was causing severe communication problems between the stability controller and the autopilot computer."
