Nov 2022 VEHICLE SPEED CONTROL I floored the accelerator, and heard a pop. The pedal then remained depressed about 50% of the pedal travel. I was able to drive the car normally, but only using about 50% of the pedal range to do so. I took it to Tesla for service and after diagnosis, had to replace the accelerator and brake pedal assembly. The mounting plate that held both pedals broke. They said it was impossible to break by stepping on either the brake or accelerator. An “outside force” must have caused the failure and they refused to cover the part and labor under warranty.

Nov 2022 LANE DEPARTURE On 11/17/2022 about 6pm when I drive on highway I -5 from San Diego to irvine, I am driving in the middle of the lane, the car keeps on going to the left and almost hitting the wall when I was on the left lane. I moved to the third lane and the car tried to do the same thing. At one point I lost the control of the car and it suddenly changed to the lane on my left. Fortunately there is no car on my left at that time. I used basic cruise control at that time, not Auto pilot .I have lane departure avoidance on at that time, not sure what's going on. The girl I carpool with said the lanes displayed in the screen is very clear. After I got off the highway, we checked the front camera and it is clean. My husband turned off lane departure avoidance function and restarted the PC of Tesla. We drove in the highway again and Tesla behavior is normal.

Nov 2022 SUSPENSION Went to get my state vehicle inspection and had heard a noise actually from the time I picked the car up new at dealer. The noise sounded like something rattling in the "frunk" down by front driver wheelwell. I thought nothing if it until the state inspector at the Tesla focused shop in Londonderry NH called me from waiting room and showed me on the lift what the noise was. He showed me what I now know is called a "jam nut" on the tie rod end of the front driver wheel. It was not even touching the actual tie rod end to hold it in place. I noticed after he tightened it the steering wheel became tighter slightly in a good way. The noise I heard was gone since picking the car up as well. This concerns me greatly. I am not sure if other nuts or bolts are correctly set on the suspension or even throughout the car as the robot or human device attaching bolts may have had a muscalibrated device and this likely is not limited to one nut on one vehicle unless it was simply gross human incompetence.

Nov 2022 ELECTRICAL SYSTEM,LANE DEPARTURE,FORWARD COLLISION AVOIDANCE I have Full Self Driving I paid $12,000 for and immediately wanted a return upon receiving the car it doesn't work as described. Tesla declined to refund me for the FSD software months ago. I have brought the car into service 3 times now and Tesla each time says there is nothing wrong with the car despite 5,000 gigabits of my own dashcam footage showing the car driving me into oncoming traffic, swerving towards concrete barriers, not seeing 5 people in a cross walk when turning at 25 mph, randomly SLAMMING on the breaks and much more. The touchscreen will freeze leaving the driver with no turn signals, no speedometer etc. I feel like the car is possessed trying to kill me, I am a full time Uber driver. Tesla service manager tells me with a straight face "just don't use it." "just don't make any left turns". The front facing camera keeps rattling so I pulled the Teslacam footage and the camera is so blurry you can barely see anything so I bring it in for service and they say nothing is wrong. This is beyond dangerous this is a dereliction of duty that will result in lost lives, NHTSA you MUST intervene I have already alerted my Governor and State Attorney, they have verified they have received my testimony. I make a choice every day to get in a car I know is trying to kill me or not be able to feed my family. Tesla service manager Prince HARASSED me by calling 15+ times while I was at dinner with my 9 month pregnant wife and threatening me by telling me he's tracking my car to Publix and to my home. Wouldn't take the car out of service mode until I drove 45 minutes back to return his phone; I should have thrown it off a bridge! because he left his phone in my car with a 1 hour timer counting down. This is a cover-up please launch a REAL investigation Tesla is stealing billions Elon makes Enron look like a hill next to a mountain this just ain't right! Last time I checked 3 service trips without a solution qualifies the car as a Lemon and needs to have recalls.

Nov 2022 SERVICE BRAKES,FORWARD COLLISION AVOIDANCE I was driving on I-70 in Colorado, heading westbound. I had the vehicle in basic "cruise control" speed mode. No autosteer or anything like that. Our of the blue, the car jammed on the brakes on its own, as if it was about to hit something. However, there were no vehicles in the area. It was a significant reduction in speed. Probably from 65-35. Luckily, there wasn't anybody behind me. It was scary, to say the least. There were no warnings prior to the episode. I believe that this is referred to as "phantom brakes". Nov 2022 FORWARD COLLISION AVOIDANCE I have been experiencing repeated episodes of phantom braking in which the vehicle will suddenly reduce speed, typically without any other vehicles in the vicinity. I routinely drive from Twin Falls to Boise, which is a distance of about 130 miles, and it regularly happens at least twice. In some instances the speed has dropped suddenly by 15 - 20 mph, which is very frightening, but in other cases the speed only drops by 3 - 4 mph. In all instances there is no reason for the braking. However, the other day it happened with a semi-truck behind me, creating a potentially dangerous situation. This has happened with both cruise control and autopilot. This has been happening since I first purchased the vehicle, which was two months ago. I have 5,000 miles on the vehicle.

Nov 2022 FORWARD COLLISION AVOIDANCE The contact owns a 2022 Tesla Model Y. The contact stated while driving 75 MPH with the Adaptive Cruise Control engaged, the contact experienced unintended braking from the ADAS feature. The contact merged into another lane to regain control of the vehicle. There was no warning light illuminated. The vehicle was not diagnosed or repaired. The dealer nor the manufacturer was notified of the failure. The failure mileage was approximately 1,200.

Nov 2022 VEHICLE SPEED CONTROL,SERVICE BRAKES,FORWARD COLLISION AVOIDANCE Alert was coming randomly while driving the car about several safety systems being disabled automatically without any audible notifications. Alert clears up after the car is locked for several minutes. Below is the alert message.. Automatic emergency braking disabled Automatic Vehicle Hold disabled Traction control disabled Stability control disabled Cruise control disabled This makes the car suddenly unsafe to drive if you are not paying attention to the screen. I have been on the road driving car at 60-70MPH several times when this happened. This can turn in to an accident. I provided the information and created a service appointment at Tesla which took a month before they can get me a scheduled appointment. Tesla fixed the issue with below note after further review. "A loose connection in the area of the steering gear was causing severe communication problems between the stability controller and the autopilot computer." Oct 2022 VEHICLE SPEED CONTROL,FORWARD COLLISION AVOIDANCE Phantom braking occurs in the Tesla Model Y when the vehicle’s TACC system is engaged. This is not auto drive, this is the systems Traffic Aware Cruise Control. Unexpected Braking occurs decreasing “cruise speed” approximately 8-11 miles per hour in a matter of seconds. On a trip from the Dallas TX area to the Colorado Springs area, this unexpected phantom braking occurred approximately 3 dozen times on open highway and in clear, sunny weather. This is a major safety concern as this brisk deceleration can cause the vehicle to be rear-ended by vehicles following the Tesla.

Oct 2022 FORWARD COLLISION AVOIDANCE The AutoPilot or Full Self Driving malfunctions by suddenly braking for no apparent reason. Going from 70mph to 50mph on a busy highway can cause cars in the back to crash into my car. Along a two lane road the car suddenly started braking and veered right when a large 18 wheeler passed in the opposite lane. Causing cars behind to suddenly brake to avoid hitting my car. Another time car suddenly braked because there was a change in color where the road had been patched and there was a slight dip in the road. The problem has been confirmed by many other drivers and NHTSA is looking to the issue. I do not know what the Tesla Company is doing about this problem. There was no warning before the sudden braking happened.

Middle of the highway, no vehicles around, no bridges or any shadows from anything. Tesla model y braked from 65mph to 20mph suddenly without warning. Serious safety issue, had a vehicle been following me i would have been rear ended.

Oct 2022 STEERING,FORWARD COLLISION AVOIDANCE I started my car and drove out of the parking lot of my apartment complex. I was trying to drive to the grocery store. At that time, the car's auto-pilot and navigation features were not on. After turning onto the main road near my apartment complex, I turned on the car's auto-pilot feature. I did not turn on the navigation feature. But the car suddenly started navigating to my work address anyway. I wanted to keep driving straight on the main road, but the car immediately turned left to go into a side street that I was already passing. The car sped up when it turned, and I did not have enough time to change direction or to break. The car steered into a tree off of the main road. I attached photos to the claim form.

Oct 2022 FORWARD COLLISION AVOIDANCE Driving from L.A. to S.F. on I-5 yesterday, we twice experienced phantom braking while using Autopilot. In both cases, there was no car close in front of us. The system just malfunctioned and suddenly braked from maybe 70 to 55 or even 50, which feels very dangerous in case a car behind us might not notice and rear-end us. It was late afternoon and we were driving somewhat into the sun. It feels like it might have something to do with sun angle and the fact that there was no car in front of us. There were also some mirages on the road that it might have mistaken for a car.

This was the first time we ever used the cruise control on the highway. The braking was aggressive, and enough to strain the passenger neck. It was scary and without warning. We feel it is too dangerous to use cruise control on our vehicle ever again for feet of causing an accident.

Oct 2022 FORWARD COLLISION AVOIDANCE I have a new 2022 Tesla Model Y and in less than 500 miles it has already "phantom emergency braked" on 5 separate occasions. The conditions was freeway driving, no traffic within 200-300 yards, daytime, and the geography was gentle rolling hills with little to no vegetation. On all occasions my speed was set at 75-80 mph. The events ranged from momentary autopilot brake application slowing the vehicle about 10 mph to very aggressive autopilot braking slowing the vehicle 30-40 mph. There are no other warning lamps, messages, or pre or post symptoms. Of course it goes without saying how dangerous this is especially if there were a vehicle following closely behind. Tesla is very aware of this problem. This particular vehicle, being brand new, has not had a service appointment to date but I am in the process of setting one up.

Oct 2022 EXTERIOR LIGHTING What component or system failed or malfunctioned, and is it available for inspection upon request: Auto high beams; car is available for inspection How was your safety or the safety of others put at risk: In Arlington Heights, IL the car started automatically switching the high beams on and off. At first the auto high beams would engage for 7-10 seconds, then disengage for 7-10 seconds. Then the auto high beams engaged for 2-4 seconds and disengage for 2-4 seconds, as if flashing them. This is unsafe as it is distracting to the driver (me), it can obstruct the view of oncoming traffic, and it makes it appear as if I am driving erratically which can result in a traffic stop / unnecessary police intervention. Has the problem been reproduced or confirmed by a dealer or independent service center - no, they will not take the vehicle in to their service center and only provided mobile service appointments during the work day. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others - no Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear - the car was recently serviced (October 5, 2022) for a nonfunctioning A/C compressor and heating pump. I am not aware of what other service / software changes+resets were made to the car while out of my possession.
