| Dec 2022 | AIR BAGS | The front passenger seat airbag does not properly arm when occupied by an adult female weighing between 120-125 pounds. According to the vehicle's Owners Manual, it should arm at any weight in excess of 100 pounds. There is an indicator on the main display that clearly displays when the passenger airbag is armed and when it is unarmed. This is an intermittent issue, occurring more than 50% of the time the seat is occupied. The best we can do to characterize the behavior is as follows: 1) The passenger airbag is more likely to arm correctly if the passenger gets into the car at the same time as the driver. 2) It is less likely to arm correctly if the passenger gets in after the driver has started the car and/or driven it a bit. Two examples: Driver gets in to set the temp or navigation before a trip and passenger gets in a few minutes later, or driver retrieves car from a parking place and then drives a short distance to pick up the passenger. In both cases, the airbag is unlikely to arm correctly. 3) Even after a successful drive (in which the airbag armed), say to the grocery store, it is unlikely to arm when the driver and passenger get back in together a few minutes later. In all cases above, the longer it is driven in a single trip, the more likely it is to randomly arm/disarm itself multiple times during that trip. By failing to correctly and consistently arm when occupied by a 120-pound adult passenger, it leaves said passenger unprotected in case of an accident. Tesla Service performed static, calibrated-weight tests, and confirmed that it takes greater than 125 pounds to consistently arm the front passenger airbag. They have replaced the passenger seat twice so far, and claimed that the most recent seat installed passed the 100-pound test, but the issue persists. | Dec 2022 | ELECTRICAL SYSTEM,STRUCTURE,ENGINE | First my car was having issues charginging at the charge stations. Then as soon as that issue occurred, I had to replace the car battery. Then I noticed that the windows would randomly retract while I was driving my vehicle. My child had pointed out that his window was down, when he did not roll it down. Then I noticed some leakage from underneath my vehicle. My main concern of fluids leaking from my vehicle was my main concern. The Tesla dispatcher confirmed that concern by stating that fluids should not be leaking from my vehicle. When he started accusing me of having a car accident that caused me to be even more concerned. He told me that the Tesla service center on 1100 Colorado Ave, Santa Monica, Ca. would find out if I did have an accident by running a diagnostics. When I had my car towed to the Tesla service center, their main concern was a hole in my tire and their professional opinion was to just patch up the tire and not replace the damaged product. Crystal, a representative from the Service center said that she would address the main concern of the leakage which she never did. She told me that my car was ready for pickup. I was again confused because she never addressed my main concern. Then I asked what I was paying for and she simply said that it was on my estimate and that it’s a patch for my tire. She was very adamant about me picking up my car before the Service Center closed at 5:30 but told me at the last minute to do so. She did not explain the details of the leakage until I inquired about it. I did not receive the diagnostics that were promised to me by your Tesla dispatcher. Her explanation to my main concern was, “ No leak was detected under the car so just recommend monitoring and definitely taking a pic when it happens. If the leak is by my left front tire, it’s more likely to be my AC and can be normal.” That did not make me feel like Tesla’s Supplier Code of Conduct was being followed. My car was in middle lane with no vehicle in front in any of lanes. Suddenly without any warning, auto pilot was disengaged and car moved from 70 mph to 30 mph within few seconds shockingly. | Dec 2022 | ELECTRICAL SYSTEM | I am writing to file a safety concern with vehicles manufactured by Tesla Motor Company Inc (“TESLA”). The complaint relates to safety failures with the automatic retracting window system. This complaint is different from the NHTSA previous issued recall for pinch points (issued on or around September 19th, 2022). During the vehicle software update process, owners can enter the vehicle and close the door (there is no engineered prevention of opening the doors). However, upon attempting to exit the vehicle the window does not automatically retract. When the owner closes the door after exit the windows are prone to break or shatter (see attached photo) which creates a safety hazard. The specific hazard is with broken glass spread in the cabin of the vehicle. Tesla charges customers to replace the window in spite of the known defect (see attached paid invoice, which documents how the damage occurred). Tesla states "it is a known documented issue and is therefore the customers responsibility". In lieu of correcting the acknowledged known issue or providing a proper engineered control to avoid the safety hazard created by the defect; Tesla has apparently accepted the defect and passed the risk of the defect to customers who are not formally made aware of the problem and are expected to understand the problem based on documentation in third party forums. The street was pretty empty and it had 2 lanes in each direction. I was driving the left lane with the speed at about 40 mph and had the autopilot (the one that keeps lane, not FSD) turned on. When the car was approaching the intersection with the green light on, it suddenly decided to turn left (even though there was a dedicated left turning pocket on the left hand side). It looks like it was confused and after trying to change the direction it pointed the car right to the road sign. Hopefully I had my hands on a wheel and was watching the situation, so I immediately turned the wheel right to get back to our lane. Additionally, the "Auto" mode is automatically enabled when the adaptive cruise control is set. It is not possible to turn off the wipers while using the cruise control, making it basically useless at night. I have been advised on two occasions by Tesla Service that this is a firmware issue and there is no fix. I almost ran over someone because Tesla rear-view camera doesn't provide virtual alert/audio when getting too close to an object or if a pedestrian decides to walk behind your vehicle while you are about to reverse or pull in reverse into a parking space. I feel this should be a mandatory monitoring system for all Tesla when reversing, just like BMW or ACURA, you do get a beeping warning sound when there's something there or when vehicle is getting too close to any objects. | Nov 2022 | UNKNOWN OR OTHER,FORWARD COLLISION AVOIDANCE | Narrative November 18, 2022 At approximately 8:30 in the morning I was driving my new Tesla Model Y through the rolling hills of South Central Pennsylvania on I-70 heading to Breezewood in order to catch the Pennsylvania Turnpike West. Suddenly and without warning, the car began to break as hard as a car can brake without skidding. Almost like a full panic stop. The car must have stopped in four or five seconds from 60 miles an hour. I was in the left lane at the time and I tried to get over onto the shoulder but I was unable. The car came to a complete stop in the middle of the right lane. The car was in the center of the right lane and was completely locked up. I was unable to get it into neutral. I was unable to get it into a gear so I turned on the emergency flashers. Right behind me were three semis. The first one had to swerve to avoid me. The trailer almost flipped over into me. The semi behind him had to swerve to avoid me and thank God there were no cars in the left lane. The third semi had to take evasive action to get around me. I am grateful those three truck drivers were on the ball at that moment. After they went by, I got out of the car and ran up the hill to flag other trucks and cars to move into the left lane. The car had come to a stop on the other side of the top of a crest so cars and trucks didn't have much notice coming up on my car. Meanwhile, I called 911. I stood at the top of the crest for the better part of an hour and a half, maybe two hours waiting for the police to show, but they never did. I did receive some help from passersby who stopped to help and called 911 as well. After almost two hours, a road crew showed up to lay down flares. And finally a tow truck came to tow me to Crystal Lake PA. Almost 8 hours later I finally got my car into a Tesla dealer in Mechanicsburg PA. | Nov 2022 | FORWARD COLLISION AVOIDANCE,LANE DEPARTURE | After my 3rd trip to tesla service (required 1 month wait each time I've had this problem since the first week) I video and audio recorded the tesla service team dropping my car off in an overflow lot for 2 and a half days before unplugging my dashcam on the 3rd day. 8 hours later I'm at the dealership and they tell me and I quote "left turns isn't a feature of FSD, just try not to use it anymore. By the way, they unplugged your camera. We found nothing wrong with your car after reviewing one whole day of footage, your cars out front." Then I get to the car and plug my dashcam back in immediately and the master key for all the Teslas is in my car (last time the same manager left their phone in my car and harassed me for hours while I was driving). Then I return the key to the manager on video, leave the parking lot, set my home destination, turn on FSD (Full Self Driving) and less than one mile away at the first turn the FSD makes it drives into oncoming traffic. This is probably the tenth time it has happened and I drive my family and passengers in this car so having to constantly bring it in and be told it has no problems after 10 total days in the shop has opportunity cost me between $2,500-$5,000 in lost revenue plus the intentional emotional infliction by gaslighting me the customer for basically not just letting my car kill me and other innocent people who didn't opt into Beta. Elon wants a war with Apple about censorship whenever I post videos of my tesla driving into oncoming traffic on Twitter because Tesla wont fix it after months (or return my $12,000) it gets banned. Same company different boss, now they censor evidence of Teslas malfunctioning and say you're imagining it LOL. You want a war, fine. I have 5,000 gigabytes of dashcam footage with over 1,000 witnesses (passengers). The car keeps phantom braking (braking for no reason) swerving towards concrete barriers and brick walls, driving into oncoming traffic on roads its navigated before in the same and different conditions. Just yesterday the indicator noises stopped working so I was forced to reset the car while driving my whole family, this is very dangerous and far too common. I have been harassed by Tesla employees who refuse to find any solution. I have been requesting a refund for FSD since the week I bought the car. This is a huge scam and cover up and when you post videos of your car trying to kill you on twitter or IG (post Elon) they shadow ban you. It is against their agenda.
