| Jan 2023 | VEHICLE SPEED CONTROL,FORWARD COLLISION AVOIDANCE | I had engaged Tesla’s “Full Self Driving” feature in a 25 MPH zone. As the speed limit changed to 35 MPH the vehicle correctly adjusted to 35 MPH. However, right after that I entered a school zone marked by a flashing yellow light. I used the brakes to slow down to 18 MPH, which disengaged the “Full Self Driving” feature because the vehicle did not adjust on its own. Once I was at 18 MPH I re-engaged “Full Self Driving” and the vehicle immediately started accelerating to 35 MPH again, in spite of the fact that I was in a school zone still. It seems the ”Full Self Driving” feature cannot detect school zones, even when marked by flashing yellow lights. The onboard visualizations appears to have detected them as stop lights. | Crash: No Fire: No | Unknown |
| Jan 2023 | SERVICE BRAKES,VISIBILITY/WIPER,FORWARD COLLISION AVOIDANCE | While driving on the freeway into a tunnel during bright conditions while on Autopilot, the car unexpectedly braked heavily on a straight road, even though there were no debris or vehicles in front. Had to take immediate action since cars were driving around me. Similar event occurred when driving at night, luckily no cars were around me. During rain, automatic wipers did not activate. Required 2 actions to deploy wipers: pressing button on the turn-signal stock, then finding the menu on the screen to set wiper speed while driving in dangerous conditions. Another option is to use the voice command, however at times it takes 2-4 attempts to activate. | Crash: No Fire: No | Unknown |
| Jan 2023 | FORWARD COLLISION AVOIDANCE | On multiple occastions my car has suddenly applied the brakes when using cruise control. The braking ranges from mild to severe. This hasn't resulted in an accident but there has been a close call when a car was following closely. I have brought this to Tesla's attention and there response was along the lines of there is nothing that can be done to fix it and it should get better over time. It has not got better. | Crash: No Fire: No | Unknown |
| Jan 2023 | FORWARD COLLISION AVOIDANCE | We have experienced phantom braking at least once almost every time we drive on the interstate or on a two lane highway. For example, wen approaching a underpass with a concrete retaining wall or on a curve with a hill. In other instances, we do not understand why phantom braking occurred. | Crash: No Fire: No | Unknown |
| Jan 2023 | STRUCTURE | 2nd row floor is flexing. Same issue found on Model 3 which already has a service bulletin for it. Tried to tell Tesla Service but they won't address it and will not fix it per the service bulletin and every shop that I went will do their own repair that doesn't fix the issue. Here's the service bulletin on the Model 3 which is the same issue. SB-18-45-002 https://static.nhtsa.gov/odi/tsbs/2018/MC-10143016-9999.pdf | Crash: No Fire: No | Unknown |
| Jan 2023 | VEHICLE SPEED CONTROL,FORWARD COLLISION AVOIDANCE | Wanted to report a problem with Tesla's map navigation and adaptive cruise control (what they refer to as "Autopilot). I haven't seen this reported in the media so wanted to report it here. The map navigation system is problematic, when navigating using the map on the freeway, it will reroute a different path because it somehow thinks I am driving on surface streets. It does this consistently. Because of that, the car when on "Autopilot" will stop in the middle of the freeway, which is incredibly dangerous. This system aims to be safe (by stopping) but also causes greater danger to the following cars by having the car brake. This could be the source behind the "phantom braking problem" that is reported in the media. | Crash: No Fire: No | Unknown |
| Jan 2023 | VEHICLE SPEED CONTROL,SERVICE BRAKES,FORWARD COLLISION AVOIDANCE | “Phantom braking” episode: While driving on a two lane freeway using Tesla branded Autopilot (not full self drive) the car automatically and suddenly braked, reducing speed approximately from 70 mph to below 50 mph before manual intervention. This happened with no car in front or behind and while approaching a car in the lane to the right while also approaching an overpass. I also noticed there was a patch of uneven colored pavement beneath the overpass. One passenger in the front seat was thrust into her seatbelt in the event. | Crash: No Fire: No | Unknown |
| Jan 2023 | FORWARD COLLISION AVOIDANCE | On 12/26 evening, I drove Model Y from Asheville NC to NJ. Around midnight time, the car was on I-81 in the middle lane at the speed of 70 miles/hour. And it was on Full Self Drive (FSD). There were virtually no traffic on I-81. On my right, there was a truck were merging to the lane on my right (not merging to the middle lane where Model Y was). Without any warning, Model Y suddenly reduced the speed to 25-30 miles / hour. I had to disengage FSD. If this happened during traffic hour, it could cause severe accident. | Crash: No Fire: No | Unknown |
| Jan 2023 | FORWARD COLLISION AVOIDANCE | On 1/7/2022 I was driving the Tesla vehicle in FSD Beta mode (I don't see this option while reporting the issue). This was on highway101 in Sunnyvale/ Mountain View area. The vehicle was in the right most lane. This lane got converted into an Exit only lane. Tesla FSD failed to detect it (didnt change the lane) and It continue at the same speed in the exiting lane (Approx. 65 Miles/hour).The car further failed to detect the curve of the exit lane and was about to keep driving straight at that speed. I had to immediately intervene and take control of the car or my car would have ended up in the ditch or no man's land around the curve of the exit road. This was near miss scenario and a driver's worst nightmare and I hope such accidents are investigated/tested. | Crash: No Fire: No | Unknown |
| Jan 2023 | FORWARD COLLISION AVOIDANCE,LANE DEPARTURE | I've reported to Tesla multiple times that on State Road 46 Tesla Auto Pilot and FSD BOTH come to complete stops on the highway while the speed limit is clearly posted 70mph the car is showing 45mph, AP is driving at 50mph then comes to a complete stop 0mph on the highway while traffic is doing 70+ passing me because the car thinks there is a stop sign that doesn't exist. When I spoke to Tesla about it the male tech (British accent) from Tampa service center said that's highly concerning and he would take the car home overnight to make sure they didn't overlook anything but they did nothing other than take away my FSD. Now I'm out $12,000 and Tesla keeps calling me a cheater/liar and I have many examples of the car malfunctioning and we are not talking a little glitch this is fully stopped on a highway the car is trying to drive around a roundabout that isn't there because the car thinks its 30ft below the highway on the street below. I've told Tesla multiple times while I was under consideration as AI engineer that they need to include the GPS altitude otherwise this exact example will happen; but what do I know, I'm just a guy with software development experience and a professional driver who just happens to be heavily invested in gps crypto. The highway has been opened for months, I've reported it to Tesla for months. Here's yet another example from today about 3 hours ago. This is like the 7th or 8th time I've reported fatal issues with the AP/FSD program Tesla can no longer gaslight me with excuses. This car has had more attempts on my life than all the countries of the world tried on Hitler combined and I'm guilty of nothing so I don't understand why Tesla has taken my money and treated me as a delusional idiot since day one. Here's a link to my Dropbox so you can watch the unedited authentic video I have dashcam footage and teslacam to match: https://www.dropbox.com/s/r33s5he9rvwtrwl/Video%20Jan%2009%202023%2C%2011%2014%2023%20AM.mov?dl=0 | Crash: No Fire: No | Unknown |
| Jan 2023 | FORWARD COLLISION AVOIDANCE,LANE DEPARTURE | New update came out and they took some of my advice; however, the problem got worse. I had an appointment at a new dealership 1 hour away and they ask me to see if I can replicate the issue and send them a video of it (yikes this is getting very dangerous) so on my way to the service appointment I do as I'm asked but the car keeps malfunctioning saying "Cheater detected" and I'm driving with my hands on the wheel with my dashcam on and iPhone with camera open ready to just grab it for the picture. I send it to them and try to use FSD again. It malfunctions again, says I'm a cheater twice in five minutes. At this time I'm unaware of what a "cheater" even is other than the traditional term. I get to the dealership and the car pops up with a window says "Full Self Driving has been removed due to constant ignoring of the blah blah blah". I tell the Tesla employee what happened he immediately says there' nothing he can do he cant fix it and I can't get my money back. I got all of this on dashcam and audio fyi. I come back the next day, the second I get out of the car and my foot hits tesla pavement a Tesla employee is harassing me to move my car except its the Uber Tesla paid for me to come pick up my car, side note the guy next to him has a huge handgun on his hip, what a safe feeling welcome... The guy inside tells me what cheaters are, I look it up on YouTube. Basically its a wrist weight people attach to the steering wheel to trick the car. I don' own anything like that the car was confusing it with my apple watch which I told Tesla and was promptly updated to the car in 24 hours. So now I'm driving a car I paid $12,000 for FSD that has driven me into coming traffic many times and calls me a cheater until it steals my $12,000 like some sort of horror arcade game. Now the car only has Auto Pilot which also calls me a cheater so its all un-useable. I have heard every excuse in the book from Tesla. Now they want to start charging me $110 to have the AutoPilot looked at. | Crash: No Fire: No | Unknown |
| Jan 2023 | STRUCTURE | As I was going up the ramp from Peachtree-Dunwoody Road onto 285E on Thursday, Jan 5th around 7pm, there is a tree on the left hand side that has long branches that extend into the road. The branches of the tree hit the side of my car. I heard it as I was driving but didn’t think anything of it. When I got home I had the chance to look at my car. There is a scratch on the side of the car that the tree limbs hit. The scratch actually looks like someone keyed my car. My car is brand new and I want to report this incident to prevent others from getting their cars scratched up. It would be helpful if the tree limbs could be cut down to prevent car damage. I also would like to be compensated for the damage to my car. I am happy to provide pics of the tree and my car. | Crash: No Fire: No | Unknown |
| Jan 2023 | VEHICLE SPEED CONTROL | The contact owns a 2022 Tesla Model Y. The contact stated while driving 75 MPH with the cruise control activated, the vehicle decelerated independently and started shaking. There was no warning light illuminated. The contact deactivated the cruise control and the vehicle started operating as designed. The vehicle was not diagnosed nor repaired. Neither the dealer nor the manufacturer was contacted or notified of the failure. The failure mileage was 3,413. | Crash: No Fire: No | 3,413 |
| Jan 2023 | FORWARD COLLISION AVOIDANCE | While driving on a controlled access highway with the full self driving beta running we had a number of instances where the vehicle would hard break with no risk of collision. These unnecessary hard breaking events could have easily resulted in us being rear-ended on the highway. | Crash: No Fire: No | Unknown |
| Jan 2023 | FORWARD COLLISION AVOIDANCE | Rented the vehicle from Hertz for a holiday road trip of 3100 miles. Experienced multiple instances of "phantom braking" where the cruise control would slam on the brakes with nothing in front of the car within the range of vision. The car would rapidly slow from the posted speed limit to under 45 MPH and then about 5-10 seconds later resume as if nothing had happened. This problem happened to me several times per driving day, and seemed to be more prevalent when cresting a rise in the highway. A couple of time times, this happened with a car behind me, and the sudden braking could have caused the vehicle behind to rear-end the Tesla. In heavy traffic it is easy to see how this phantom braking could cause a chain reaction accident. At this point, I think that Tesla needs to permanently disable their cruise control in all Model Y vehicles. | Crash: No Fire: No | Unknown |
| Jan 2023 | UNKNOWN OR OTHER | There is an outstanding recall 22V702 - automatic window reversal systems and the next course of action is that Tesla will deploy an over-the-air (“OTA”) firmware update to affected vehicles. However, my tesla account doesn't have that software available yet. Registrar of Imported Vehicles has searched the records for the above referenced vehicle and has either detected an outstanding recall, or has been unable to verify the current recall status of this vehicle. So unfortunately, they are unable to release an inspection form until such time as they need confirmation that the recall, if any, has been corrected, or that there are no outstanding recalls for this vehicle. Steps taken by me to eradicate this: 1) I tried contacting the tesla dealer at various locations in Canada and USA. 2) I did reach out to the Tesla manufacturer and they told me that they will try to deploy firmware updated for my vehicle and they told me to wait for 1 business day( this conversation happened like 5 days ago) and some associate from this location will reach out to me and i can visit that service center for the resolution of recall. Tesla dealers along with their service centres are totally unaware of what’s going on and don’t have visibility of when this software will be available. This is very big blocker for me as my car will not be registered in Canada. Can we please resolve this recall so that my import process gets completed. This is surely emergency case where I am unable to drive my vehicle and I will be charged too. I have already exhausted those days. Tesla is unable to solve this. Can we please escalate this issue and solve this recall on my car and also once done Tesla should issue recall clearance certificate that I need to submit to RIV for my inspection process. | Crash: No Fire: No | Unknown |
| Jan 2023 | STEERING,SEATS,VISIBILITY/WIPER | 1. Whoever designed the seats never sat more than 2 hours straight in them and is most likely below 150 lbs with the body shape of a petite person. Damaged coccyx and lower backs since inception of idea. Insanity will ensue after long periods of travel (+2hrs) in rear seating and extended third row seating for adults and children. You get more seating comfort from a Nissan than a Tesla. 2. Windshield wiper sensitivity lowers when using manually then activating Auto. Sometimes sensitivity goes down with autopilot. Auto wiper sensitivity not constant. 3. If you have more than 4 people in your family, do not buy a model S3Y 4. Bad panels which also affect steering column, with squeaking, why is that not a lemon? 5. Long range below 32F falls to half its mileage per full charge. Proper trip charging should be at 10% to 15% more than the minimum 0% for trip charge. 6. Autopilot forces high beams whenever there is fog. Autopilot will never engage on curves, only straight aways 7. Supercharger long trip navigation will not allow filtering locations with bathrooms or trashcans nearby. Walking far distances to find restrooms and trashcans are a normal thing. Teslas not meant for taking long trips. 8. Signage for charging misleading, "10 to 30 minute parking" will only charge 10 to 50 KW max while "vehicle charging only" will charge the advertised supercharger max output. 9. No climate control in third row for MY, safety hazard and human rights issue. | Crash: No Fire: No | Unknown |
| Dec 2022 | VISIBILITY/WIPER,FORWARD COLLISION AVOIDANCE | When I am driving and go into Cruse Control my wipers continue to come on when it is not raining or there is no moisture. Sometimes they are going at full speed as if it is pouring rain. When this happens, I am forced to hit my brakes to disengage Cruse Control, then take my eyes off the road and find the wiper icon and turn them to the off position. This happens often and has almost caused me to drive off the road a few times. I took the car back to Tesla and was told when I engage Cruse Control the wipers are by default set to "Auto". Also they had seen this before and there was nothing they could do to fix the issue. They also said Tesla was working on the software to improve the wipers. Why am I forced to have my wipers set to "Auto" when I use Cruse? I should be able to set them to "Off" and they only come on when I activate them. If this continues on my car and others there will be accidents because of the distraction trying to turn the wipers off. | Crash: No Fire: No | Unknown |
| Crash: No Fire: No | Unknown |
| Dec 2022 | SEAT BELTS,SERVICE BRAKES | Seat belts are hitting the door and seat. Model y car seat belts are not installed properly. As per Tesla regenerative braking supposed stop the car but car is rolling. As per Tesla tech regenerative doesn’t work until the battery is heated up. They never provided this to customers and this could result in accidents for people who rely on regenerative braking | Crash: No Fire: No | Unknown |
| Dec 2022 | FORWARD COLLISION AVOIDANCE | Car did a hard braking while going 75mph on autopilot, no car or obstacle in front when it happened. The car following me had to do an emergency lane switch maneuver to avoid collision. | Crash: No Fire: No | Unknown |
| Dec 2022 | FORWARD COLLISION AVOIDANCE | The contact owns a Tesla 2022 Tesla Model Y. The contact stated while driving at 65 MPH in the rain, the vehicle experienced phantom braking and stopped suddenly. The vehicle then veered to the right independently. The brake pedal was depressed but failed to respond. The contact lost control of the vehicle. The vehicle spun out and crashed into a wall on the side of the road. The air bags deployed. The contact sustained bruises and significant pain to the left leg. The contact also sustained soreness of her fingers, arms and shoulders. The contact did not seek medical assistance. The contact’s sister transported her to her residence. The contact was unsure whether a police report was filed. The vehicle was towed to a tow lot. The vehicle was then towed to a collision center. The contact was not using autopilot mode at the time the failure occurred. The manufacturer was notified of the failure and informed the contact that she would need to repair the vehicle at her own expense. The vehicle was not diagnosed or repaired. The failure mileage was approximately 5,000. | Crash: Yes Fire: No Injuries: 1 Towed | 5,000 |
| Dec 2022 | VEHICLE SPEED CONTROL,FORWARD COLLISION AVOIDANCE | While driving with Autopilot engaged at approximately 65 mph the car suddenly decelerated to less than 40 mph and then accelerated back to 65. This happened again within a minute of the first occurance and I turned off Autopilot but left cruise control on. It then happened again and I turned cruise control off. Fortunately there were no cars following close behind me nor was I following close behind another car. In the first two cases there were intermittant shadows across the road cast by trees along the road side. In the third there was a large black programmable traffic information sign 20 ft or so off the side of the road that had no information displayed. There was no damage to the car or injuries to the occupants. The vehicle has not been inspected by the manufacturer. No warnings were displayed prior to the event. | Crash: No Fire: No | Unknown |
| Dec 2022 | UNKNOWN OR OTHER | Interior Cabin Cameras prior to a certain build date around late May, for all models and trims, do not have the necessary Infrared (IR) Leds to see driver at night/and or dark environments, leading to system to disengage due to “being blinded or covered” if a user has their car in the FSD beta testing software. This issue was reproduced by the Mobile Service Technician and then replaced accordingly, with the new Interior Cabin Camera Fixture with the IR Leds fully solving and correcting the error from before. Their part was not inspected by any agencies and error is displayed before ADAS system disengagement. This issue is also known and reproduced across many different people, with this replacement correcting the issue. Necessary action would involve ordering a retrofit for all cars with the old interior cabin camera to the new interior cabin camera. | Crash: No Fire: No | Unknown |
| Dec 2022 | FORWARD COLLISION AVOIDANCE | During a recent road trip, my TESLA 2022 MY experienced multiple "phantom braking" incidents. The worst one involved while driving at highway speed (77 MPH). On a clear day, with a dry road, with no traffic nearby, no shadows in the area, the vehicle suddenly applied the brakes. It was like it detected something in the road and jammed on the brakes. I used the accelerator to compensate, but, it tried to slow down for 20 seconds before resolving. My wife (passenger) has medical conditions that were exacerbated by being frightened. I also was frightened, which is dangerous while driving. If there was a vehicle behind us, there cold have been an accident. There was no warnings before, during, or after the incident. Multiple people online have described their situation. It is exactly the experience that I have had with my vehicle. | Crash: No Fire: No | Unknown |
