| Feb 2023 | FORWARD COLLISION AVOIDANCE,LANE DEPARTURE | I have experience multiple instances of "phantom braking" where the car suddenly brakes when there is nothing in front of the car. The most concerning incidents are when traveling on interstate highways while Autopilot is engaged. The car will brake suddenly, rapidly, and unexpectedly. I experienced this problem that first day I picked up the vehicle and continue to experience the problem on extended interstate drives of several hours. The car is available for inspection upon request. The biggest safety concern is that a closely following car could rear end the rapidly braking Model Y. Another concern is that the car could loose control on ice or snow while rapidly braking. Despite three requests, Tesla has declined to inspect the vehicle. | Feb 2023 | SEATS | After having strapped our kids to their car seats, my daughter had her feet resting on the rails of the driver seat in front of her and her feet were trapped and squished really hard when the driver seat started to automatically adjust between my driver profile and my husband's profile. I am 5'3" and he is 6'3" so his profile has the seat further back than mine. My daughter started to scream and cry immediately as we tried to quickly reset the driver profile to mine so she could free both of her feet. She had slight bruising and soreness after the incident. This is reproducible and the only way to prevent was to delete my husband's driver profile to avoid having his phone trigger the self adjusting mechanism when he opens the driver door. The vehicle has not been inspected for this issue by Tesla or any one else. We believe this is a feature that is by design but it poses a risk to anyone sitting in the back and unable to rest their feet on the floor such as a kids in their car seats. There are no warnings, messages about not resting your feet on the seat rails and the seat doesn't stop and doesn't detect if something is on the rails. I couldn't find a way for the driver profile to be locked to a single driver and be manually triggered for any other driver. I tried to search how to report this to Tesla and I couldn't find an obvious way to do it. The number listed in the manuals doesn't have an option for this. | Feb 2023 | ELECTRICAL SYSTEM | A backup camera is required on all new cars. Tesla has a backup camera system comprising 3 views. When reverse gear is engaged, camera views should appear every time on the instrument display. On the first drive of a given day, the camera views are not properly visible until the car has been driving for a few minutes. Specifically, “night mode” on the infotainment display (effectively the instrument cluster) becomes stuck on dark colors and unreadably low brightness. When reversing during daylight hours, the screen is not bright enough to view the backup cameras. Additionally, audible cues for turn signals are inaudible. After about 1/4 mile of driving, the display “catches up” to day mode and responds with appropriate brightness. I believe the dysfunction of the system is a safety hazard to vehicle occupants, other drivers, and pedestrians. I have previously contacted Tesla service about this issue. I was told that the issue is related to software; nothing can be done; it will be fixed at a later, indeterminate date via software updates. | Feb 2023 | FORWARD COLLISION AVOIDANCE | Tesla was in "full self-driving” (FSD) mode in a quiet residential area. Vehicle chose to make a left turn on a less than perfectly symmetric T junction. The car was on the thru street at the top of the T. Street had a middle yellow turning zone with yellow lines on the outer side and dotted ones inside. Camera was not filming at the time the crash began. but did start recording midway. Tesla may have other footage. In retrospect, while indicating it was intending to turn, I believe the car never entered the middle yellow zone, but instead proceeded directly to the middle of this unusually wide neighborhood intersection where no other vehicles were close by in any direction, although a few were approaching from a distance. It appeared safe to complete the turn, but instead the car feinted forward several times and hesitated to commit to the turn as cars gradually approached from both directions, but were still a safe distance away. As I waited for it to turn, the vehicle suddenly accelerated at an alarming rate into the turn, so fast that it slid off the road and over the curb into a wall causing extensive damage to the vehicle. There were no other cars involved and I was not injured, although the car suffered extensive damage and is still being repaired more than two months after the accident. My foot was definitely not on the accelerator throughout the accident and everything happened so very quickly that I had no time to brake or take other action. The oncoming car was quite far away and there was plenty of time to turn safely. If the car issued a warning (although I did not detect one) it would have been almost simultaneous to the amazingly rapid acceleration, leaving no time to react. My speculation is that in its confusion, the vehicle may have transferred nearly instantly from FSD mode to “Accident-Avoidance” Mode, but only Tesla would have that information. | Crash: Yes Fire: No Towed | Unknown |

| Jan 2023 | FORWARD COLLISION AVOIDANCE,LANE DEPARTURE | "Full Self Driving" without explanation stopped working while I was using it. I tried to park the car, walk away, lock it; I even rebooted the car by holding both scroll wheels on the steering wheel as I've been directed in the past. Nothing worked. I noticed during my shift that "Auto Pilot" still worked on the highway so I was using it for a couple miles successfully. Then when the time for the exit came the car took the exit normally but then when it switched to "FSD" the car stopped steering and drove me directly towards a drainage ditch next to the off ramp and all steering assist and lane keeping was turned off all at the same time (while traveling at 50mph on I4). I was really scared luckily I never took my hands off the wheel but anyone with lesser reaction time would have crashed and possibly died. I got this on dashcam, pulled over to calm down (McDonalds near MCO), then drove back to prove it happened with the iPhone as Tesla has directed me to do on many occasions to "prove it". So I got it all on video you can see on this link here since NHTSA only allows low quality short videos less than 10mb like its the 1990's. The more I report Tesla and tell my riders about the child slaves in the Congo mining the cobalt in the batteries the more frequently the car tries to kill me. | Jan 2023 | FORWARD COLLISION AVOIDANCE | At the end of my shift the "Full Self Driving" started working again, also without explanation. This time its driving me home, gets to the neighborhood I drive in and out of multiple times a day for the entirety of my "FSD Beta" and only address the car has "lived" at. This time it SLAMS on the brakes with someone behind me (scary) then quickly takes the wrong left turn, then another wrong left turn, then starts the ACCELERATE in a neighborhood/school zone directly towards a cul-de-sac with 2 story houses, speeds up more, then abandons all the "Full Self Driving" while I'm flying towards a power transformer box and curb when I SLAM the brakes (I gave her all she's got, she's heavy!) luckily I stopped with just a couple feet to spare. Then I drive back the direction it was supposed to go which also for the record it was showing a different path on the map, the entrance it always is. So the map was showing the blue line for GPS to turn into the second entrance, this time was the first time its ever taken the first entrance and I've literally never driven down the road it almost crashed me on but its been there for 6 months or longer. So just to clarify Tesla has FAILED at least 10 rounds at fixing the car and they cannot offer a solution. This car is a LEMON! Do something, you're the government I shouldn't be having a car try to kill me five times a day for several months without a recall. This clown car even plays the wrong songs and song titles! This is a coverup either the car is faulty and Tesla refuses to fix it or its hacked and being used for attempted murder. Either disgusts me and I was Tesla/Elon's biggest fan for over a decade, then I bought his car. The rest is history... | Jan 2023 | FORWARD COLLISION AVOIDANCE,LANE DEPARTURE | Driving both east and west on 429 near the Wekiva river results in the car coming to a complete 0mph stop in a 70mph two lane highway with barriers on both sides and no signs or lights. Then the car starts changing lanes on the highway going about 15mph while cars are whipping past me at 70+mph with the horn on. The last 8 or 10 updates have not solved this issue I've been very alarmed and done everything in my power to avoid using the software and have demanded a refund many times, this is simply a deadly scam. The FSD contract is a license to kill and silence your attempts to alert the public like a good Samaritan; it must be shut down until proven safe. Each time they say they fixed it, each time I go out and risk my life and they lie, and lie and lie and now the stories of people dying from this exact scenario are starting to pile up (pun intended). The car is STILL confusing its sea level or altitude positioning on the GPS for the round about with stop signs below the sunpass highway. Fact of the matter is I have over 15,000 GB's of footage and there are hundreds if not thousands of scenarios where the car needs to be corrected. I am a professional driver and have software development experience but we cannot allow a private corporation to use human American citizens as lab rats and throw them aside when their software that is not safe yet kills the very people trying to improve the system. Not to mention innocent by standards who have never seen the inside of a Tesla nevermind an arbitration contract. This is all completely unethical.
