Jun 2026 FORWARD COLLISION AVOIDANCE Tesla recently pushed a software update that introduces a major, unavoidable driver distraction right at the most critical moment of driving: the exact second you take manual control back from Full Self-Driving (FSD).   Whenever you disengage FSD, a mandatory, multiple-choice survey pops up on the center touchscreen asking why you took over. Unlike previous versions where this would disappear after a few seconds, it now stays on the screen permanently. There is no close button, and it does not time out.  This causes two clear safety hazards while driving: 1 It blocks the screen: The popup actively covers up part of the navigation map and basic screen controls. If you are trying to navigate a tricky intersection or see your route right after taking over, you can't see the map clearly because this giant survey is blocking it. 2 It forces you to look away from the road: Because the prompt never goes away on its own, it forces the driver to physically look down, read the small text options, and tap the screen to clear it. Forcing a driver to do a multiple-choice survey while actively managing a manual takeover in traffic is incredibly dangerous. Tesla should not be allowed to use a mandatory, permanent UI popup to collect data while a vehicle is actively moving. It is a severe visual and cognitive distraction at the exact moment a driver needs to be 100% focused on the road. Please investigate this software design and require Tesla to add an automatic timeout or a quick-dismiss button that doesn't require driver interaction.

Jun 2026 ELECTRICAL SYSTEM I purchased a new 2026 Tesla Model Y Standard and took delivery on approximately 30 May 2026. Within the first week of ownership, the vehicle displayed multiple safety-related warnings, including: * Steering Assist Reduced * Power Braking Assist Reduced * Automatic Emergency Braking Unavailable * Lane Departure Avoidance Features Unavailable * Automatic Vehicle Hold Disabled The warnings have occurred more than once. They initially disappeared and then returned on a later drive. Both incidents occurred while the vehicle was operating with Full Self-Driving (FSD) engaged. During the most recent occurrence, while parking at less than 3 mph, the vehicle’s braking assistance appeared reduced and the brake pedal required significantly more effort than normal. As a result, I nearly struck a curb because the vehicle did not slow as expected. The warnings indicated that increased steering and braking effort may be required. Because the vehicle is brand new and these warnings involve steering, braking, and collision-avoidance systems, I believe this creates a significant safety risk. I no longer feel the vehicle is safe to drive until it is inspected and repaired. Tesla has been notified and a service request has been initiated.

Jun 2026 LANE DEPARTURE: ASSIST While operating a 2026 Tesla Model Y on Full Self-Driving (FSD) Supervised, the vehicle's vision-based lane-keeping system failed on an unmarked residential road. At approximately 26 MPH, the vehicle approached a blind crest of a hill. Due to the lack of painted center lines, the FSD software miscalculated the road geometry and drifted significantly into the center/oncoming lane exactly as it reached the top of the hill. This lane departure created an immediate, severe safety hazard, as it placed the vehicle directly in the path of potential oncoming traffic with zero forward visibility over the crest. Due to the critical lane positioning error, immediate driver intervention was required. The driver took manual control of the steering wheel to force the vehicle back into the correct lane, causing the FSD system to instantly disengage. Simultaneously, the driver applied manual braking to reduce speed drastically down to 10 MPH to safely clear a resident working on the right shoulder and stabilize the vehicle. Prior to the failure, there were no warning lamps, error messages, or symptoms indicating the system would fail to maintain proper lane positioning. The system appeared to be functioning normally up until the crest of the hill. The vehicle and its Autopilot/FSD components have not yet been inspected by Tesla, police, or an independent service center following this specific event. Jun 2026 ELECTRICAL SYSTEM The contact owns a 2026 Tesla Model Y. The contact stated that while driving at 65 MPH, with the autopilot feature engaged, the vehicle approached a lane closure as big, metal arrow road signs where on the road indicating the road closure. The contact, who was distracted at the time, expected the vehicle to acknowledge the road closure and switch lanes. The vehicle failed to recognize the Chevron barricade lights on the highway and the vehicle ran through the road signs without warning. The contact was forced to take over and stop the vehicle due to the failure. Due to the failure, the front, driver side mirror detached from the vehicle. The contact exited the highway and inspected the damage to the vehicle. Despite the failure, the contact managed to drive the vehicle home. The service center was notified of the failure and an appointment was made to have the damages repaired. The cause of the failure had yet to be determined. The vehicle was not repaired. The failure mileage was approximately 21,000.

Jun 2026 LANE DEPARTURE: WARNING Incident Report – 2026 Tesla Model Y Date of Incident:XXX Time of Incident: 12:30 am Location: [XXX] I was operating my 2026 Tesla Model Y and was driving manually. Full Self-Driving (FSD), Autosteer, and other self-driving functions were not engaged at the time of the incident. While traveling on Highway 16 at approximately 60 mph, the vehicle approached the right-side white lane line. At that point, I experienced a sudden steering intervention that turned the vehicle sharply to the left. The steering input was unexpected and occurred while I was manually controlling the vehicle. Following the steering intervention, the vehicle crossed the centerline into the opposing lane of travel and continued off the roadway into the ditch on the opposite side of the road. The incident resulted in damage to the vehicle. Road conditions were dry. Weather conditions were safe. Lane markings were clear. I am requesting that Tesla preserve and review all vehicle telemetry, event logs, steering-assistance records, and any other relevant data associated with this incident. Specifically, I request an investigation into whether Lane Departure Avoidance, Emergency Lane Departure Avoidance, or any other driver-assistance feature activated and contributed to the steering maneuver. I am also requesting a determination regarding responsibility for the resulting vehicle damage and whether Tesla will reimburse repair costs if a system malfunction or defect is identified. May 2026 UNKNOWN OR OTHER My bulldog died yesterday in the new Tesla y we bought because the side storage in the back doesn’t have a cover. I regret ever buying that car. The side storage should come with a cover especially for the size of the space. My explorer came with covers. She went in head first and suffocated to death. I pulled over as soon as I realized something was wrong and had do compressions on her in front of my kids. But it was too late. This is a safety issue. Fix it before it happens to other families. My family is devastated by this. People put their animals back there. It should be safe. We should be able to trust the product especially at the cost we paid. The car has dog mode which means it is supposed to be safe for you to leave your dogs in the car. It is not. I have records of her cremation

May 2026 VISIBILITY/WIPER Second time in the five months I have owned the car that I need to replace the windshield in its entirety. Not only does it chip incredibly easily, but any chip immediately becomes a large crack, usually overnight and definitely before Safelite or a similar company can be called to try to fix it. In addition to being dangerously fragile and very expensive to fix - although very lucrative for Tesla since I can’t drive without an intact windshield - it’s very difficult to get an appointment. This time around I must drive around with an ever-expanding crack for 16 days before driving 100+ miles to the nearest Tesla service center to get it replaced. How can it possibly keep me and my family safe if it breaks that easily and that often and why is it normal to replace the windshield that often in any car much less a brand new car?

May 2026 SUSPENSION The car is unstable on the highway when there is a crosswind - either from wind gusts or from large vehicles. The car is easily pushed by wind until lane assist activates to stop it from going outside of the lane. It feels very unsafe to drive on the highway. This is how the car was when it was bought. An alignment was done during the first Tesla service visit and that helped slightly. The second Tesla service visit they said the vehicle was operating as expected and then checked wheel balance after I insisted. They said they fixed wheel balance but it still did not change how the car handles in crosswind. The third Tesla service visit (different service center) they said the vehicle was working as expected and said they could not drive for a significant distance to test. If lane assist ever fails when I'm not ready to instantly react to a gust of wind, it feels like it would easily move into another lane or off the road. When passing large vehicles where wind changes direction, the car sometimes gets pushed left and right which sometimes triggers an alarm and shows an on screen message saying to take control of steering. I test drove another of the same model car for an hour and had the same unstable feeling in the wind but was unable to test long enough to find an area with high crosswind.

May 2026 ELECTRICAL SYSTEM This is the opposite of a complaint. Somebody may have already submitted a blurb about their Tesla Y traveling on a highway with the speed limit of 60 giving an extra foot of room to a motorcyclist that was passing their vehicle. This happened to me today for the first time while I was in full self driving mode . My car was aware of the motorcyclist wanting to pass me and the car in the next lane.  Sensing this it moved over about a foot in my lane to allow the motorcycle passage  more room. A few minutes later, it did it again for a different motorcyclist. It did this all while in full self driving mode. Kudos to this safety technology.

May 2026 FORWARD COLLISION AVOIDANCE: WARNINGS At about 2:16 PM on Monday, 5/18/2026, I was driving southeast in the right lane of Amaya Drive from Fletcher Parkway in La Mesa, CA, planning to turn right across the trolly tracks onto Severin Drive. There is an intermittent LED sign indicating NO RIGHT TURN in the southeast corner of the intersection, just below and to the right of the ground-level stoplight, that illuminates when the trolley is passing so traffic going straight on Amaya can still proceed. In the first Street View PDF I uploaded, it is not illuminated. I had FSD engaged, and as I approached the intersection the NO RIGHT TURN LED was illuminated. If I recall correctly, I assumed FSD would stop for the NO RIGHT TURN LED; but instead, it continued to make the right turn. Luckily my son was in the right seat and yelled several times for me to stop, which I did—just a few feet from the crossing arm, which can been seen elevated in the middle of the second Street View PDF I uploaded, and which was just coming down for the approaching trolley. By the time I got stopped, The crossing arm was fully down, and as noted above I ended up just a few feet from it, and would clearly and unequivocally have driven under and through it if I hadn’t braked suddenly. It’s clear that this was completely the fault of FSD either not reading or not being aware of the intermittent NO RIGHT TURN LED sign. Since FSD clearly doesn’t read NO RIGHT TURN signs, particularly intermittent LED signs, the solution seems to make this intersection NO RIGHT TURN whenever the light is green to pass through the intersection on Amaya, although this will hold up the cars who would like to turn right when there’s no trolley approaching.

May 2026 STRUCTURE:BODY I am reporting a recurring and unresolved electrical/safety issue with my new Tesla Model Y. The issue has occurred 5 times within the past 4 weeks and affects multiple systems simultaneously. When the issue occurs: * The air conditioning stops working completely. * The driver-side front and rear windows cannot be rolled down or up. * The driver seat adjustment may not function properly. The problem appears intermittently. It may disappear temporarily and then return hours or days later. Tesla service has inspected the vehicle multiple times, but the issue continues to recur. This creates a significant safety concern because: * The driver window may not function during rain or emergencies. * The AC may fail during extremely hot weather. * The driver seat may not adjust properly, potentially affecting safe vehicle operation and driver visibility. * The intermittent nature of the failure makes the vehicle unpredictable and unreliable to operate safely. The issue has not been permanently resolved despite repeated service visits. I am concerned there may be a broader electrical or control system defect affecting vehicle safety.

May 2026 UNKNOWN OR OTHER,FORWARD COLLISION AVOIDANCE,LANE DEPARTURE The latest Tesla autopilot software forces users to engage in an autopilot product survey after disengaging autopilot while driving in order to gain access to audio/radio controls. The survey is not dismissible without reading a complicated prompt and choosing a response, and it completely obstructs audio controls until you participate in the survey. The driver therefore cannot turn off music to listen for emergency sirens, pedestrians, or other potential safety cues while driving at any speed, unless the driver also takes eyes off the road to participate in the forced product survey. This is by design and affects all Tesla drivers who use the autopilot feature.

May 2026 SUSPENSION 2025/2026 Tesla Model Y Juniper experiences noticeable vibration starting at approximately 75 mph on smooth highways. The vibration is felt throughout the entire vehicle: steering wheel, doors, seats, and pedals/floor. It is not limited to the steering wheel. Tire balancing, alignment, and foam checks have all been performed with no resolution. May 2026 BACK OVER PREVENTION: WARNINGS While my vehicle was operating in Full Self-Driving (FSD) mode, it encountered an accident upon attempting to park in my garage. The collision resulted in substantial damage to the driver’s side rear end and the garage wall. Notably, there was no manual intervention during the entire journey from my son’s school and the parking attempt. The FSD system failed to detect the proximity of the garage and did not initiate braking or provide any warning before impacting the wall. In this instance, the FSD system exhibited multiple failures. I have subsequently reported the incident to Tesla.

May 2026 FORWARD COLLISION AVOIDANCE: WARNINGS While driving, the vehicle repeatedly triggered emergency braking without any obstacle present. The car continuously displayed warnings indicating pedestrians in front and behind the vehicle when no people were present. Due to these false detections, FSD became disabled during operation. The repeated sudden braking caused severe lower back pain, and I will be seeking medical treatment. This vehicle is unsafe to operate and presents a serious safety risk to myself and surrounding traffic. I have full video evidence of the incidents.
