| Oct 2025 | VISIBILITY/WIPER | Outer rear-view mirror on the driver side exhibits a vibration/shaking of the image. It is obvious and continuous vibration is observed typically on highway speeds of approximately 60mph or higher and lower speeds it is evident driving over slight bumps, concrete/asphalt road surfaces or bridges. Looking at the shaking image cause dizziness and it is even worse in night time with headlights from vehicles coming behind. It is annoyance to the point that makes you feel not to use the rear-view which can potentially put the vehicle and others in danger. Passenger side doesn't vibrate and makes it easy to notice the issue. Dealer inspected the mirror and said manufacturing issue and replaced it under warranty with no success is fixing underlying vibration issue. On the second visit, dealer denies the problem and says the mirror is replaced and operates according to toyota specification (it feels to us they do not care about the functionality). According to dealer they will do anything further only if Toyota release a TSB on this particular issue. | Crash: No Fire: No | Unknown |
| Oct 2025 | VISIBILITY/WIPER | The drivers side exterior mirror shakes. This occurs during all driving, most notably over bumpy roads and at highway speeds. The passenger side mirror does not have this problem. Because of the shaking, it is distracting when driving as it appears there is movement in the mirror. Additionally, it causes a distortion from the constant movement. The vehicle was purchased in June 2025 and has occurred since day 1. The vehicle was in the dealer on 9/30/25 and noted the issue, stating there is nothing physically wrong with the mirror but did agree with the assessment. They said this is common on vehicles (they see the complaint a lot) but Toyota has not acknowledged it so there is no fix and appears to be a manufacturing defect. | Crash: No Fire: No | Unknown |
| Oct 2025 | VISIBILITY | The contact owns a 2025 Toyota Sienna. The contact stated while driving 65 MPH, the sunroof glass shattered. There was no warning light illuminated. The local dealer was contacted; however, the vehicle was not diagnosed or repaired. The contact was informed that the VIN was not under recall. The manufacturer was not notified of the failure. The approximate failure mileage was 275. | Crash: No Fire: No | 275 |
| Oct 2025 | SERVICE BRAKES | When i am driving from high speed and trying to use my brakes, the whole rear car will start shaking around 48/45 miles per hour , I can hear our car seat from our baby how it makes vibrating noises and i can see my both door mirrors they're shaking too, and when it drops to 40mil per hour the shaking goes away. Went to the local Toyota dealer 3 times and they're saying that's normal. Very disappointed with a brand new car. We have another new car and doesn't do that for sure... Called Toyota national, open a case and they're saying, mechanic can't find anything wrong. They should drive this car for a longer distance and they will know for sure what I am talking about. | Crash: No Fire: No | Unknown |
| Sep 2025 | STRUCTURE | The contact owns a 2025 Toyota Sienna Hybrid. The contact stated that the rear liftgate appeared to show a design flaw, and the left and right sides of the liftgate appeared to be misaligned. The contact stated that the liftgate door was difficult to open manually or electronically. Several dealers were made aware of the failure on several occasions, but were not offered any assistance. The vehicle was taken to an independent mechanic who acknowledged the failure. The independent mechanic stated that the entire back panel was warped and that the door hinges were misaligned. The manufacturer was notified of the failure on three separate occasions but offered no assistance. The vehicle was not repaired. The failure mileage was approximately 10,000. The VIN was not available. | Crash: No Fire: No | 10,000 |
| Sep 2025 | SEAT BELTS,SEATS | Driver side passenger 2 nd row seat belt stopped working. Won't clip as needed leaving us unable to use seat. This is a safety issue as we are not able to use the seat as needed as one is not able to strap in. This triggers the sensor and this a driving violation. Problem has been reported. Vehicle has been reported and still pending to be replaced. Unkown when issue can be resolved. Passenger seat on the second row was stuck upon purchase. Dealer made aware. they claimed a sock was stuck on railing which was not possible as issue was reported before they could schedule for vehicle to be inspected. Such sock did not exist when incident was reported. Sock was not purchased until day before taking vehicle to schedule appointment. Tension is still there but agency said it is working properly. | Crash: No Fire: No | Unknown |
| Sep 2025 | VISIBILITY/WIPER | The driver side mirror vibrates at highway speed enough as to make it useless to observe cars behind you. Moving the mirror around does not help. | Crash: No Fire: No | Unknown |
| Aug 2025 | VISIBILITY | Wobbling vibration on exterior mirror, driver side. It looks like incoming vehicles shake sideways. This happens on any bumpy road and it is easier to observe at highway speeds. Also, every time I close the door, mirror shakes as it its loosely attached. Driverside mirror has too much play and shakes. There is no issue with play and shakes with passenger side mirror. Purchased on [XXX] Vehicle is available for inspection. The distorted image resulting from mirror wobbling affects driver's awareness of surroundings, visibility gets worse at nights/rain due to blurred lights. This can lead to an accident. My intuition when I first noticed was going to be a cold break, but I realized that it was the mirror wobbling causing it. The car coming behind was far enough and I was able to react. Summary of actions and requests to Toyota: I went to my dealership Crestmont Toyota, Pompton Plains, NJ and they have confirmed the play and wobble issue, however, they said it's the same thing with other 3 Sienna's they had on LOT. Dealership informed that this is how its manufactured and they can't do anything until TOYOTA issues a recall for this problem. First occurrence: Day one, driving back from the dealership. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6) | Crash: No Fire: No | Unknown |
| Aug 2025 | UNKNOWN OR OTHER | Trunk hatch knocked out of alignment. The dealership said it is because the door was closed when something was in the way. If there was really something in the way it should've been picked up by the sensors. | Crash: No Fire: No | Unknown |
| Aug 2025 | VISIBILITY | Driver side wing mirror vibrates at low speeds and worsens at highway speeds greatly affecting rear visibility. Passenger side has no vibration but drivers side is so severe it's difficult to make out anything in the mirror image. | Crash: No Fire: No | Unknown |
| Jul 2025 | SEATS | The seats in the car squeak and shake when passengers are not in them. | Crash: No Fire: No | Unknown |
| Jul 2025 | UNKNOWN OR OTHER | Moonroof on my new 2025 toyota sienna has water leaking in. I've brought it to the dealership to try to resolve this. They said they fixed it. But I've just got a carwash and I still water leaking in. I have video of this. It shouldn't leak or be design to leak like the dealer said. | Crash: No Fire: No | Unknown |
| Jul 2025 | VISIBILITY | Driver side mirror is loose and is vibrating even at normal/slow speeds | Crash: No Fire: No | Unknown |
| Jun 2025 | SEATS | I have a 2025 toyota sienna and the 3rd row seats is not locking when in the fold down position. While driving the 3rd row seats go from the stowed position into the seating position making loud noise. | Crash: No Fire: No | Unknown |
| Jun 2025 | BACK OVER PREVENTION | I bought this van brand new. I quickly realized that the reverse camera view has some issues. I believe images are not clean and sharp, as it is supposed to, and has unreasonable amount of distortions. Cleaning the camera from outside didn't do anything. I also noticed that the bottom part of center blue alignment line is little bit to the right and not align with the top. I checked online and it appears that other owners are also suffering from this same issue. Toyota should recall all affected vehicle and fixed this issue as soon as possible as this is a major safety issue when backing up! | Crash: No Fire: No | Unknown |
| Jun 2025 | UNKNOWN OR OTHER | I bought this vehicle brand new. I quickly realized that both rollers on the sliding doors were causing excessive noise, which I found out that many other owners have complained as well, when closing & opening. This is because of a poor assembly from Toyota as the rollers are not machined prefect painted & are either oversprayed or less sprayed causing gaps on the rollers. These gaps or uneven paints causes excessive noise when rollers are moved on their track. There are several new Sienna owners who are waiting to have this issue fixed as the parts are backordered. I had to make total of 4 trips to my local dealership to have the issue fixed. I can still hear a little bit of over excessive noise from one of the sliding doors. Toyota should immediately recall all the affected vehicle and have this issue fixed. | Crash: No Fire: No | Unknown |
| Apr 2025 | STEERING,WHEELS | Purchased a **2025 Sienna XSE** with severe alignment issues since new. After **two failed repair attempts** at Fort Bend Toyota, the steering is now worse, and they damaged my wheel without offering solutions. This is a **safety hazard**—the car drifts dangerously at slow and highway speeds. The dealership dismissed my concerns, forcing me to escalate to **NHTSA** and **Toyota Corporate**. Avoid for serious mechanical issues—safety clearly isn’t their priority. | Crash: No Fire: No | Unknown |
| Apr 2025 | SEATS,VISIBILITY | The drivers mirror shakes every bump you hit so everything looks blury going above 30mph. This is a huge safety concern as you can't see cars clearly in the mirror. One of the second row seats also shakes considerably. I have brought it to the dealer multiple times and they will not fix it. | Crash: No Fire: No | Unknown |
| Mar 2025 | VISIBILITY/WIPER | With the TV pulled down, there is absolutely no visibility though the rear view mirror even when adjusting the seat and mirror. | Crash: No Fire: No | Unknown |
| Feb 2025 | UNKNOWN OR OTHER | Driving roughly 60 mph on empty road with no cars in front. Sunroof was closed, and exploded with great force and 80% of the glass flew off. No objects impacted the sunroof prior to the sunroof exploding. | Crash: No Fire: No | Unknown |
| Feb 2025 | VISIBILITY | Driver side mirror visibly rattles when driving, but even worse when on highway. Safety concern is that visibility is blurred when driving. Vehicle is available for further inspection. Safety is put at risk since visibility becomes blurred when trying to change lanes. Yes concern has been reproduced, however I was told Toyota won't declare a warranty issue because other Sienna's and models of Toyotas have similar issue. Component has not been inspected by Toyota. However research I have done shows that Toyota is not acknowledging as a safety concern. No warnings. I purchased the 2025 Sienna Van February 8, 2025. I noticed almost immediately when I left the dealership, and turned back around for the dealership to inspect. | Crash: No Fire: No | Unknown |
| Jan 2025 | LATCHES/LOCKS/LINKAGES | Drivers and passengers front doors make the sound as if they unlock and lock. The manual lock lever will flip as well however the doors will not unlock. This happens randomly both while trying to exit the vehicle and enter. When locked inside you have to crawl over the console to get to a door that functions . This is a major concern if you are in an accident and cannot exit the vehicle. Dealer look at my vehicle but could not duplicate it. I have a video of this when it happens | Crash: No Fire: No | Unknown |

My intuition when I first noticed was going to be a cold break but I realized that it was the mirror wobbling causing it. The car coming behind was far enough and I was able to react. Summary of actions and requests to Toyota: Two dealerships in central Texas have confirmed the issue but case was dismissed. Details below. 1. Dealership 1: Issue acknowledged by shop foreman , mirror changed but this did not fix the issue. Opened case with Toyota Brans Engagement and requested to work with a different dealership. This is the dealership where the car was purchased. Report wrongly stating rattled noise they weren't willing to correct it and pushed me to sign. 2. Dealership 2: Issue acknowledged again. Since mirror change did not fix issue shop foreman opened Dealer Product Request. 3. Current status: On 1/3/25 Toyota Brand Engagement advocate called to say that even though the shop foreman at the 1st and 2nd dealerships acknowledged the problem, the resolution was that it was a "normal" condition for the car. Case Reference #[XXX]. No warnings related to the problem. First occurrence: Day one, driving back from the dealership. Conditions were probably not met during test drive (you need to pay attention, more on that in the body of the post). Confirmation on day 2 on another drive, all with less 2 digit mileage, perhaps low 100s. Posting my case online has gotten 4 people reporting the same issue with cars since 2021 in multiple trims. Similar issue on RAV4 reported on [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6) | Crash: No Fire: No | Unknown |
| Nov 2024 | SEATS | Headed seat barely warm | Crash: No Fire: No | Unknown |
| Nov 2024 | UNKNOWN OR OTHER | The sliding doors will shut off after 60+ minutes of being left open. In order to close them afterwards, you must fight with the power closing mechanism which makes it incredibly slow to close. This is dangerous if someone is threatening you and you need to get in your car and close the door. Or if a swarm of bees is coming at you. It takes about 30 seconds to close the door when this happens. Toyota claims this is to save power but this makes no sense as an excuse as the rear. Liftgate can stay open indefinitely without problem. | Crash: No Fire: No | Unknown |
