| Oct 2025 | ENGINE | [XXX] , a long-time Chevrolet customer and GM Family First Program participant, reports a recurring, life-threatening defect in his 2024 Chevrolet ZR2 3.0 Diesel truck. The issue centers on repeated fuel injector failures that have caused the vehicle to become uncontrollable while driving, posing serious safety risks to his family and others on the road. Despite multiple incidents and documented repairs, Chevrolet has failed to provide a permanent solution or meaningful customer support. Mr. [XXX] is requesting NHTSA’s intervention to investigate the defect and hold GM accountable. | Oct 2025 | POWER TRAIN | 8 Speed Transmission without any warning, started shaking and clunking and slipping while doing 55mph in traffic causing sudden loss of momentum. 8 speed transmission failure. Dealership says bad transmission control module and won’t inspect internal components. Chevy is still playing games with people with their defective 8 speed transmissions. They have not fixed all of their previous problems | Crash: No Fire: No | Unknown |
| Oct 2025 | POWER TRAIN | Truck did not have reverse. Service engine came on after going forward and possible loud shudder like going over a pot hole. Not sure if it was a pot hole or transmission. Dealer fixed the valve body in Transmission after 1 week. | Crash: No Fire: No | Unknown |
| Oct 2025 | ELECTRICAL SYSTEM,SERVICE BRAKES,ENGINE | We have had four previously documented issues with the electrical, brake and engine in our truck. Today, we were advised by the dealer that the truck had been fixed, and a new ECM had been installed. They advised they drove the truck for 100 miles and there were no issues. We picked the truck up and on the way home, while on the highway, the truck went into low power mode, the brakes went out, the check engine light and slip light came on. The dash began flashing detection warning lights (see attached photos): Brake system failure 62 MPH top speed, Service steering column lock, Reduced Acceleration drive with care, Service ESC, Service traction control. We immediately turned around and went back to the dealership and showed them what was going on. They took photographs and had the technician come out to look at the vehicle. He looked at the lights and codes and admitted he does not know what is going on. The service manager said that the company representative was coming tomorrow (10/21/25) and that he was going to speak with him and recommend a buyback. We left the truck with the dealer and were given a loaner car. This is the 5th incident with the same issues. | Crash: No Fire: No | Unknown |
| Oct 2025 | SEAT BELTS,SEATS | The car seat anchor system is very poorly designed and extremely difficult to use. I just spent an hour trying to ensure the seats for our grandchildren are installed correctly. The upper anchors are poorly placed, difficult to access and do not inspire confidence the seats will be secure in the event of an accident. This is after I had to take the truck in for a warranty claim because the lower car seat anchor on the back seat passenger side was covered by factory upholstery and could not be used at all. It’s very frustrating when one attempts to comply with safety regulations but poor design inhibits the proper installation of car seats. | Crash: No Fire: No | Unknown |
| Oct 2025 | ENGINE,FUEL/PROPULSION SYSTEM | I have a 2024 Silverado 1500 Z71. I purchased new, within the first month I started experiencing issues related to the fuel. Stalling, dying, missing at idle and failing to start. I’ve reported it to the dealerships, with the same response unable to duplicate. Sept 22, 2025 I was driving alone rounding a curve in the road when my vehicle lost complete power causing me to loose control crashing into trees. I was unharmed physically, but mentally this hurt. To know this all could have been avoided had Chevy customer service or the dealerships listen and looked at the dtc that were clearly shown. P3187 fuel pump pressure performance low pressure. Stated failed since last cleared. Dealership did not acknowledge or discuss with me this dtc. Instead they gave me a multi point inspection stating unable to duplicate. There negligence caused me to wreck | Crash: Yes Fire: No | Unknown |

| Oct 2025 | ENGINE | There is a safety recall on my truck and dealer wont take it in to address the issue. I have been trying for months and they say that they are waiting for guidance from GM. We don't feel safe driving the truck. Asked for GM to buy it back and they denied, told me to find another GM service center. I tried another and they said the same thing. I'm running in circles and shouldn't be forced to drive an unsafe truck that is ~1 year old. | Crash: No Fire: No | Unknown |
| Sep 2025 | ENGINE,FUEL/PROPULSION SYSTEM | GM is not handle this recall in a timely manner. They are refusing to inspect vehicle to see of there are any defects. This is concerning, as based on the recall notice, the engine may fail at anytime. This could cause a crash or leave me or my wife stranded on some road. My dealer would do the inspection but to do the inspection but they said they are not allowed until I have gotten "permission" from GM. An immediate inspection should be done ensuring if the vehicle is safe to drive. | Crash: No Fire: No | Unknown |

| Sep 2025 | ENGINE,FUEL/PROPULSION SYSTEM | Engine was making noise and ultimately stopped responding when I would push the gas, leaving me stranded on the side of the road. Luckily, I was able to get the vehicle off the road in time to not cause an accident. The vehicle could have stalled and seized up in the middle of the highway. This problem has been confirmed by the dealer in the attached work order invoices. It has not been inspected by anyone other than the Mountain Chevrolet dealership, to my knowledge. The dealership has the vehicle running again, but is not able to confirm if the 'new' engine in my vehicle is the one address the recall issue, reference number N252494000. Additionally, this vehicle broke down again two months later for a related issue per the additional service invoice attached. For all of these, GM has not been willing to pay for the full value of the rentals for each service, totaling over 60 days without my vehicle. I've requested this through the GM Customer Care to no avail. I would also like to receive confirmation that the 'repaired' engine in my vehicle resolves this open recall concern. I've been stranded on the side of the road twice now with this newer vehicle and would love some assurance from them. At a minimum I want GM to pay for the full value of the rental cars. For the record, I also requested GM buy-back the vehicle which they have rejected my request. Any help you can provide is appreciated. Thank you. | Crash: No Fire: No | Unknown |
