| Aug 2023 | POWER TRAIN,ENGINE,FUEL/PROPULSION SYSTEM | Vehicle sometimes fails to accelerate when the accelerator is depressed. When this occurs, the vehicle moves forward slightly but then stalls while the engine is still running with no warning lamps or messages displaying. On several occasions this has caused me to enter into traffic (usually from an intersection or parking lot) and then discover the vehicle has stalled and won't accelerate. Removing my foot from the accelerator and then reapplying accelerator pressure usually corrects the issue but creates a dangerous situation and increases the chance for collision. The issue was first noticed on my vehicle about 2 months into ownership and has been reported several times to Vandergriff Toyota, Arlington, Texas. No resolution has been offered as the dealership can't identify or replicate the problem. T-SB-0111-22 was issued in December of 2022 and supposedly addressed this issue, but the problem continued after this work was performed on my truck. This issue is also well documented on the 2022 -2023 Tundra Forums with many owners still experiencing this problem. My vehicle is available for inspection. | Crash: No Fire: No | Unknown |
| Aug 2023 | SERVICE BRAKES,FORWARD COLLISION AVOIDANCE | Twice it has felt like I was being rear ended. Something is making a slamming motion on the truck when it is stopped. First time was at a stop light, second time was backing up but stopped to put the truck in "drive". I have tried looking up to see if this truck was affected in a 2022 recall but the information is unavailable. | Crash: No Fire: No | Unknown |

| Aug 2023 | SEATS | The driver side front seat side shield panel cracked two months after vehicle was purchased new. It was replaced under warranty at the Toyota dealership. It cracked again three months later and was replaced again by the Toyota dealership. About 3-4 months later, it cracked again and was replaced by the Toyota dealership. This was on 04-21-23. On June 29, 2023, it cracked again for the fourth time. Dealership so far has not agreed to replace it under warranty and we are pursuing warranty claim with Toyota headquarters. I have owned 5 Toyota trucks and never have had this problem. When the front seat side shield panel cracks, it is a plastic part inside that is breaking. It is probably occurring when entering and exiting the vehicle possibly sliding across the side edge of the seat. There are several of the same problems with the seat posted on the internet for this truck. This is a design flaw and needs to be corrected by Toyota. It creates a safety hazard as you cannot operate the seat properly when it is broken. | Crash: No Fire: No | Unknown |
| Jul 2023 | UNKNOWN OR OTHER | Driving down interstate at approximately 75 mph when I heard a loud explosion similar to a gun shot. The sun shade began shaking in my head liner. Once I regained my composure I was able to stop on the side of the interstate and inspect the roof from the outside. The whole middle section of the sunroof has exploded upwards and out of my vehicle. | Crash: No Fire: No | Unknown |
| Jul 2023 | SERVICE BRAKES | My vehicle since purchase has had squeaking brakes and the vehicle only has 4500 miles. At the dealership( Wesley Chapel Toyota) today I was told this was a known issue and that its a material issue that is still being addressed by Toyota , yet when I call Toyota directly they have no understanding that any recall is in place and opened a case ( [XXX] ) but also suggested let the NHTSA know. Breaks are critical and don't understand why this is not being addressed in a more expedient manner ; why wait until someone is seriously hurt. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6) | Crash: No Fire: No | Unknown |
| Jul 2023 | UNKNOWN OR OTHER,LANE DEPARTURE | My vehicle was parked next to a fire hydrant and when i returned to the vehicle none of the warning cameras or alerts was able to pick up the object. | Jul 2023 | UNKNOWN OR OTHER | Had been driving for 35 miles on 70mph freeway when dash lights illuminated stating the parking break would not disengage. This truck has an electronic parking break that was not engaged. Truck then said it was going into reduced power mode and shut off acceleration over approx. 5 MPH. I later read the alerts in the Toyota app and it stated “ a malfunction in the electronic control of engine” and a “malfunction in the intelligent control system”. I nearly got rear ended by multiple semis since I had no power. Took truck to dealership but because the “they couldn’t recreate the issue”, they gave the truck back. This is a huge safety issue with these trucks and Toyota needs to do something about it. It appears to be a common issue with the turbos in this new engine. | Crash: No Fire: No | Unknown |
| Jul 2023 | FUEL/PROPULSION SYSTEM | My gas gauge does not work properly . When I fill it up it showing full. I will put on any where from 40-70 miles and my gauge will show I have 3/4 of a take before empty. Then I turn it off and when I turn it back the gauge goes back to full. Went to a dealership twice . The first time they replace a gas pump and some other things. I picked it up drove it around and the same thing happened. Took it in a second time and they told me Toyota has no solution to fix my gauge . | Crash: No Fire: No | Unknown |
