Date Component Summary Severity Mileage
Jun 2023 ELECTRICAL SYSTEM,FORWARD COLLISION AVOIDANCE The 2019 Mazda CX-5 has a design and manufacturing issue with its infotainment system. The screen activates functions without pressing the screen or activating the joystick. It changes the station, calls by phone and changes important security parameters such as braking assistance. Reflecting a notice on the main board. There are thousands of cases of affected vehicles. This problem is a serious situation that Mazda must correct immediately, because it distracts the driver and changes extremely important safety parameters. The infotainment system is not just for listening to music or making calls. This system manages many aspects of vehicle safety. Mazda replaced the part with a new part number for obvious reasons. All vehicles that have the affected part must be replaced with the new part number to avoid possible accidents. The defective part is K123-61-J0A. The replacement is K123-61-1J0C. Thank and best regards.

May 2023 ENGINE The cylinder head is “seeping” out oil Not a crack and not gasket failure Dealer service advises they could help us get it fixed IF ONLY we had receipts of oil changes. We do not because we change the oil ourself.

Apr 2023 ENGINE My 2019 Mazda CX-5 has 61,500 miles on it and has a cracked cylinder head and has a massive oil leak. I took it to the dealer and they are aware of this issue but told me Mazda hasn't so far made a recall. No warning lights went off and i lost 1 quart of oil over a 30 mile drive. Upon Acceleration the engine could seize and could cause a crash. A fire is also not out of the question as the parts in the engine area are hot and oil can catch fire. Mazda declined to pay for the total cost of the repair but did partially pay for some of it. They should be held accountable for the lousy design and it's only a matter of time someone will have an accident because of it.

Apr 2023 ELECTRICAL SYSTEM,UNKNOWN OR OTHER UNKNOWN While stopping the radio system (which can not be powered off) will change and go to other apps. This causes the driver to take their eyes from the road and hand from the steering wheel to press buttons to get the radio system under control. When the radio does this it often goes to the settings and changes the navigation voice volume to the loudest it can go essentially frightening the driver and causing them to look away from the road and divert their attention to the radio system. This occurs every time the vehicle is driven and often occurs when hitting the brakes.

Apr 2023 VISIBILITY/WIPER I purchased this vehicle used and with 35,000 miles on it, took it to Mazda for an oil change. The windshield wipers never seemed to work that well, and Mazda told me that the actual wiper arms (not blades) were installed on the wrong sides of the windshield (ie: Driver arm was on the passenger side, and Passenger arm was on the driver side). I removed them arms and reinstalled on the correct side and now the wipers work fine. I am reporting this as I assume they were installed incorrectly when the vehicle was manufactured, and I'm concerned that other vehicles may be affected.

Apr 2023 ELECTRICAL SYSTEM,UNKNOWN OR OTHER The screen in the middle of Mazda vehicle starts selecting menu, change the radio station or selects/plays whatever it likes. It suddenly activates some functions on its own. Most likely similar issue as in this document - https://static.nhtsa.gov/odi/tsbs/2019/MC-10164268-0001.pdf This is dangerous for the driver as the automatic change in the screen and menu items distracts the driver which may cause accident and put life in danger. This is generic problem and searching on the internet, lot of people are suffering with this issue. When the issue started last year, we thought this is something random occurrence and will go away by itself but with the time, this has gone worse. Now looking in the internet we came to know this is actually an ongoing issue.

Mar 2023 ENGINE While driving the vehicle at highway speed, the check engine light came on at just under 28,000 miles. I took the vehicle to AutoZone, who did not come up with a code; however, I took it to the dealer with engine code P0126, which stands for insufficient coolant temperature. Apparently, there is a flaw in the coolant control valve, but the part is on backorder until June or July. In the meantime, my gas mileage is awful, my car takes forever to warm up, and the engine performance is awful and the vehicle is not safe to drive at highway speeds. This really merits a recall and a whole lot more attention from a company that was Consumer Reports most reliable brand for 2022. I am really disappointed with Mazda and how they have handled this situation and they have permanently lost my business over this.

Mar 2023 VISIBILITY The contact owns a 2019 Mazda CX-5. The contact stated while driving at approximately 75 MPH with the sunroof opened in the vent position, and the rear windows and driver's side and passenger windows opened, the contact stated he heard an abnormal sound like a gunshot and the sunroof glass shattered, and pieces of glass entered the cabin. The contact pulled over and observed that a hole measuring approximately 12 inches by 6 inches had opened in the sunroof. The contact noticed that the glass surrounding the hole had an uplifted edge of approximately 2 inches around the hole. The contact had not taken the vehicle to a local dealer or independent mechanic. The vehicle had not been repaired. The manufacturer had not been informed of the failure. The failure mileage was approximately 48,000.

Mar 2023 ELECTRICAL SYSTEM The Infotainment System has consistent "ghost touching". Not only is this a distraction while trying to drive it is changing the safety settings and display settings available in within the system such as ADAS settings and MPH\KMH speedometer options. Other less-severe options are consistently changed as well such as navigation volume etc. The problem is easy to reproduce, just need to start the car. I just paid $300 for a full replacement of the screen from Mazda, along with an update and the problem not only persists but is worse. The problem got progressively worse but there were no light indicators etc.

Feb 2023 ENGINE AND ENGINE COOLING The contact owns a 2019 Mazda CX-5. The contact stated became aware of engine oil on his driveway. The contact stated that while driving and coming to a stop, there was an abnormal oil burning odor inside the vehicle. The contact stated the check engine warning was illuminated. The contact had taken the vehicle to an independent mechanic who diagnosed that there was an oil leak coming from the cylinder head and determined that the cylinder head needed to be replaced. The independent mechanic researched online and related the oil leak failure to Mazda Service Alert Number: SA-031/21 (Engine and Engine Cooling). The vehicle had not been repaired. The manufacturer had been informed of the failure. The failure mileage was approximately 89,000.

Feb 2023 POWER TRAIN,ENGINE I drove my wife to work and noticed that the car was stuttering and had weak acceleration. I parked in my garage and a couple of hours later returned to see a massive oil leak on my driveway, I had to tow the car to the nearest dealership where a cracked cylinder was diagnosed. There was also a terrible burning smell. If the massive leak happened on an acceleration ramp or on the highway there could have been an accident.

Jan 2023 UNKNOWN OR OTHER My vehicle appears to have what is known as the "ghost touch". The radio ends up clicking random things, skips songs, goes to the maps and zooms all the way in. This puts the driver at risk if they are distracted by what is happening to their radio. I informed the dealer, but they thought it just needed a software update. The issue continued even after the update that I had to pay for. Warning lamps to not appear, and it happens at random times, it's never consistent. It's been happening for a while now, probably over a year, and months after I had purchased it as a used car.

Jan 2023 FORWARD COLLISION AVOIDANCE My 2019 Mazda CX-5 automatically applied the brakes while the car was on cruise control traveling at 65 mph on a highway, and there wasn't a car or any object in front of me or to either side of my car. The car applied the brakes firmly and brought the speed down quickly and dangerously. I quickly depressed the accelerator to get the car back to highway speed to avoid a rear-end collision. This issue has happened 3 times in the past 2 weeks, all while traveling at highway speeds on cruise control for an extended period of time (hours). I believe this is the same issue with the Mazda Smart City Brake Support system that led to a recall of 2019-2020 Mazda-3 vehicles. My Mazda CX-5 is out of warranty and the dealer stated it is not covered by the recall.

Nov 2022 POWER TRAIN,VEHICLE SPEED CONTROL After purchasing this vehicle I started noticing that as I accelerated from a stop the transmission would shift hard into 1st gear and 3rd gear. This is also occurring during deceleration as if the car brakes I'm stages due to the downshifting hard. The dealership has refused to look into the issue. They have refused to add the notes to my vehicle records in effort to avoid accountability.

Sep 2022 FUEL/PROPULSION SYSTEM Unknown. The engine started to stall as I was driving and the gas pedal wasn't working. The car shutdown after reaching 0 moles per hour and it would jerk when I tried starting it. The car had enough fuel, battery and was service a month prior. The car would not start and had to wait a few hours to try again. The issue is similar to the recall this model has but when I checked my VIN is all cleared . Please Help.

Jul 2022 VISIBILITY/WIPER While I was driving on the highway at 60 mph the glass sunroof exploded with a loud bang. Luckily the sunroof shade was closed, otherwise shards of glass would have fallen on me causing injury and possible loss of control of the car. I immediately reported the incident to my insurance company and the next day brought the car into the Mazda dealership. The service manager claims he never heard of sun roofs exploding and there were no outstanding recalls on sunroofs. I left the car at the dealer for further inspection by Mazda.

Jun 2022 TIRES P225/55R19 99V: On November 2, 2021, I was slowing down at a stop light. I was about 5 minutes away when my tire separated. I heard my car make a big sound and drop to the ground on the front passenger side. I lost control and hit the side of the curb, but I was able to pull my car over to a side road safely. It took over 3 hours for the tow truck driver to come, but when he did, he said he hasn’t seen one like this in a while, but it is a defective tire. The middle of the tire was a clean separation from the sides. If it was a tire blow-out the sides of the tire would be uneven, but this was as if the glue that held it together was loose, and the middle just fell out. The tow person tried to take the spare tire and he found that it was incorrect. My car a had 25,000 miles and I bought it new from the dealership and they made the mistake of giving me the wrong tire. Mazda corporation later had to replace it. Mazda dealership told me to contact customer service to discuss the tire. I emailed customer service several times and it took nearly 2 months for them to answer. I called several times, and I would wait on hold and they would hang up. When I finally got someone on the phone, she told me that Mazda warranty does not cover the tires. That is completely in the hands of the tire manufacturer. I contacted Toyo Tires and waiting a month for a response, I was contacted by a customer service. He mentioned that photos and videos were not enough, but that I had to send the tire back. I mentioned that Mazda did not tell me to keep the tire and when they replaced it, they disposed of it. I received a letter stating that there is nothing they can do. My car is a 2019 CX-5 and in the same year the CX9 was recalled for tire separation issues from the same company. I am extremely frustrated how Mazda and Toyo has handled this entire issue.

May 2022 EXTERIOR LIGHTING The contact owns a 2019 Mazda CX-5. The contact stated that the center display was malfunctioning. The contact stated that the rearview camera turned off inadvertently or failed to operate as intended while reversing the vehicle. The contact also stated that the screen display brightness would decrease independently. Additionally, the contact stated that the exterior lights would malfunction due to the failure. There was no warning light illuminated. The vehicle was taken to the dealer where the computer software was updated. The dealer also informed the contact to remove the Mazda navigation SD card. However, the failure persisted. The vehicle was taken to the dealer where it was diagnosed that the center display touch screen panel had failed. The vehicle was not repaired. The manufacturer was notified of the failure and informed the contact that they could not assist as the VIN was not included in any recalls. The failure mileage was approximately 40,000.

May 2022 AIR BAGS,VEHICLE SPEED CONTROL Tuesday, 5/3, I was parking my car in the New Leaf Grocery parking lot in Aptos, CA. I pulled into the lot and made slow right into an available space. I had my foot on the accelerator, tapped down on it gently to ease into the space. My car lurched forward at such high speed that I was unable to remove my foot from the accelerator. I swerved to the right as the car went over a partition and hit another parked car on the other side of the partition. My airbag did not activate. I've replayed the experience in my mind and tried convincing myself that this could have been a matter of human error on my part. I am beyond convinced that what I experienced yesterday was a case of sudden unintended acceleration. I am concerned and afraid to drive my car now. Had circumstances been different, someone could have been killed. Had I not swerved right, I was headed to crash into a building. I called 911 and since no one was hurt there was no Police report. To address questions set-out above: Speed control (sudden unintended acceleration) and air bag failed My safety and the safety of others was put at risk As of yet the problem has not been reproduced however there are a great number of reports related to unintended acceleration (however not acknowledged as a problem by Mazda) I plan to have my Mazda inspected by a local dealership service center There were no warning messages or symptoms in advance of the incident occurring I've owned my Mazda for 3 years and never experienced anything like this. I have never had a car accident to date.
