When we went to retrieve those items, the lid only opened partially. We were finally able to remove the wallet but the phone had slid down into the compartment where the door to that console opening slides into when you open and close it. The lid would not open all of the way and blocked our access to that space when you opened it. The only way to remove the phone was to take it to our local dealership where they spent hours removing seats and pieces of the console to remove the phone from the inaccessible area and then put everything back together. This cost us roughly four hours of work time and $311.99 out of our own pocket. Apparently this is a known issue with this vehicle and is a design flaw where items of the right size (including iPhones) can slide into that compartment while the lid is closed and then are inaccessible. | Crash: No Fire: No | Unknown |

| Jun 2026 | AIR BAGS,FORWARD COLLISION AVOIDANCE | The Supplemental Restraint System (SRS), specifically the airbag system, is the component of concern. During a two-impact collision involving my 2025 Toyota Sienna XLE, none of the airbags deployed despite significant front-end damage that resulted in the vehicle being declared a total loss. The vehicle is currently in the possession of the insurance company or its storage facility. I do not know whether it has been released for salvage, but it may still be available for inspection. I, my wife, and our children were all wearing seatbelts at the time of the collision. After an oncoming vehicle struck the front-left portion of my vehicle while making a left turn, my vehicle lost control and collided with a concrete pillar. None of the airbags deployed during either impact. All occupants sustained injuries and were transported to the emergency department. I suffered injuries to my right leg, wrist, chest, neck and back. My wife sustained injuries to her neck, chest, back, and knee, and our children sustained injuries including head impacts. My wife and I continue to receive physical therapy. The vehicle was declared a total loss following the collision, and no dealer or independent repair facility has evaluated or reproduced the concern. The vehicle was inspected by the responding law enforcement officers at the crash scene, and a police crash report was completed. The insurance company (GEICO and Allstate Fire and Casualty) inspected the vehicle and declared it a total loss. To my knowledge, Toyota has not yet inspected the vehicle, and I do not know whether the Event Data Recorder (EDR) or Supplemental Restraint System has been analyzed. Prior to the collision, I did not observe any airbag warning lights, SRS warning messages, or other indications that there was a problem with the Supplemental Restraint System. | Jun 2026 | VISIBILITY/WIPER | June 3, 2026, approximately 5:30 PM EDT, I entered my 2025 Toyota Sienna to move it from the street beside my home, to my driveway. Upon closing the driver's door, the rear liftgate window exploded into the vehicle. This occurred without any warning or adverse action by myself or anyone else. The vehicle has been in no collisions, and has only 6429 miles on the odometer. I phoned Toyota's "Customer Experience Center" to report the incident and to seek warranty service for this problem. I was told to drive the vehicle to my Toyota Dealer, which is located 115 miles away, for inspection. I explained that it is unsafe to drive a vehicle filled with broken glass and with no rear window 115 miles. If I vacuum up the glass, there is nothing for the dealership technician to inspect. If I tape plastic over the opening, it could come loose at highway speed, and cause an accident, and the tape adhesive could damage the vehicle finish. The Toyota agent advised me that the vehicle must be inspected at a Toyota Dealership, and if the vehicle is unsafe to drive to the dealer, we could have it towed. But in a subsequent call to the "Customer Experience Center", my wife was informed that Toyota would not cover a tow of that distance in their warranty, and it was our fault for buying a car and taking it to an area that far from a dealer. The phone number we called is located inside the vehicle owner's manual cover under a heading, "If there is not a Toyota dealer near you, please call the following number:" In the morning, once they are open, I will call the nearest dealership, 115 miles distant, to seek further guidance. Based on my background in both mechanical engineering and automotive failure analysis, it is my opinion that the window and its support system was not designed to withstand the prolonged sun and heat exposure experienced. The selling dealer marketed the vehicle as "Certified Used" on Cars.com for at least 64 days after the recall was announced. The dealer's sales representative also represented the vehicle in writing via SMS as "Gold Certified" with the full Toyota Certified Used Vehicle warranty package on [XXX] — one day before sale. The dealer disclosed the recall only at the closing table on [XXX], after I had relied on the prior representations to negotiate the price, submit a credit application, drive to the dealership, and tender a down payment. Toyota's interim recall guidance prohibits occupation of the second-row seats until remedy is performed. The remedy is currently not available. My [XXX] daughter has [XXX] and uses a wheelchair; second-row sliding-door access is essential for her safe transport. The vehicle has been in dealer storage since 01/08/2026 because it cannot safely transport my family. As of 05/24/2026 — 229 days after the recall was announced — no remedy date is available per the dealer's Service Manager. The vehicle is available for inspection upon request. The Sienna was bought as a seven‑seater, but due to the recall it functions as a four‑seater, making it impractical and unsafe for my family.
