| Jan 2025 | POWER TRAIN | The morning my transmission failed... I started the truck and put it in R, at which point the entire truck shuddered for several seconds. At this point many, many warning messages appeared on the instrument cluster screen as well as the infotainment screen - and the truck would not move in Reverse. After moving the shifter into P and back to R, the truck backed out of the driveway. When the truck was shifted to D - nothing happened. The truck was shifted back to R and then back to D at which point the truck moved forward but would not shift out of 1st gear. Placed truck back in P and then back to D two more times and truck would move forward but not shift out of 1st gear. Turned truck off and on approximately 3 times and it began to shift gears when driving. I drove the truck to the closest Toyota dealership where the warning codes were checked, and I was told that my transmission was bad and needed to be replaced. I was told that it would need a new Torque Converter assy, new Automatic Transmission assy, and a new Hybrid Module assy. Total cost for parts would be $27,504.29.... Covered by warranty. My truck was purchased brand new in Virginia, MN on [XXX], and the failure occurred on [XXX] with less than 800 miles on the vehicle. I have noticed that the mat consistently becomes caught on top of the clutch pedal when I press the pedal all the way down. This causes the mat to obstruct the pedal’s full range of motion, making it difficult to fully disengage the clutch. As a result, I have to be extremely cautious and pull over to manually adjust the mat whenever this happens, as it poses a potential safety hazard while driving. I have recorded a video showing this issue occurring, but the file size is too large to submit through the NHTSA portal. As an alternative, I have taken a screenshot from the video that clearly shows the mat interfering with the clutch pedal. I am concerned that this issue could lead to an unsafe driving situation, especially in emergency or high-stress driving conditions. | Dec 2024 | POWER TRAIN | I bought the truck at 6 pm on [XXX] in Pennsylvania and drove straight home in Brooklyn, when trying to find a parking spot the truck started to act weird(10pm after 190 miles). The rpm was going really high with no power to the wheels and abruptly it seems to find gear and jumped off really fast a couple of times. I was driving in a residential area at low speed and was able to pull over to the curb. I could have hit a pedestrian, another car or lost control pretty badly if I was in a highway. After the initial incident all the lights came off (engine malfunction, brakes malfunction, collision system malfunction and others). The truck was towed and after a long wait, today the 5th of December, it was diagnosed as a transmission problem, Toyota is going order a new transmission and the broken one will be sent to Toyota for further inspection. They want to replace my truck and it seems they are not willing to cancel the deal. I lost most propulsion of the vehicle with no advanced notice. I barely was able to make it to a portion of the road with a shoulder. There was heavy traffic due to the thanksgiving holiday traffic. The error on the Vehicle indicated Hybrid System Failure. In the vehicle information I was able to access additional errors, “A malfunctions in the intelligent Clearance Sonar System has been detected”, and “A malfunction in the Drive Start Control System has been detected”. The vehicle was towed to a local Toyota dealer and the have diagnose that the transmission and a computer need to be replaced. This is not an issue with the Apple device, as it works as intended in other vehicles such as a 2020 Hyundai Kona and a 2024 GMC Sierra where it has been tested. This is a safety concern as it is distracting to the driver to have to intervene in order to use the Carplay functionality as it does not work like it is supposed to. I would have to give it 2-3 cranks to raise the seat. Overnight, the next morning, without anyone in the seat, it needs 2-3 cranks to raise the seat back to its maximum height. The smaller is approximately 30.6 inches high and the larger is 32.61 inches high. Toyota manufacturing sets the odometer and speedometer for the larger tire and says that is the only option. That means that the speedometer is off by 4 miles an hour at 6 4 mph and for every 100 miles shown on the odometer, the truck has only traveled 98 miles. It’s frustrating to recalibrate in your mind. | Oct 2024 | POWER TRAIN | What component or system failed or malfunctioned, and is it available for inspection upon request? Transmission, and yes its available for inspection. When I put the truck into drive from park, and even reverse, the truck would jerk. (I have videos) When accelerating around 13-14mph, I could feel the transmission "gear searching" with rough shifting. When decelerating around 15-17mph, the transmission would down shift too early, producing the feeling of engine breaking, and again rough shifting. When I needed to really go, I experienced "transmission lag" where the engine would be working, reving up, the feeling of engaging the clutch (coasting feeling), then after a moment the gear would catch and the truck would finally take off. I took the truck to a dealership to have it checked out, but the technicians said they couldn't reproduce this issue. I called corporate and they sent me to another dealership. This dealership said the same thing, BUT went the next step and pulled some transmission fluid and found metal shavings. They had the truck for almost 3 weeks and replaced the transmission and torque converter. When I went to pick up the truck I put the truck into drive and felt the jerk I experienced before. I put it into reverse then drive, and again the whole truck jerks. I asked to speak to a manager and he said he can have the foreman look at it the next day. I made him get in the truck with me and showed him the truck jerking when placed into drive. Next day, foreman said there was nothing wrong and performed the same as a new truck on the lot. I spoke to a higher manager and he recommended I bring the truck in for the regional Field Technician. I was stopped at a traffic light when the problem occurred. Multiple sensor alarms were triggered including Hybrid System Malfunction. I attempted to accelerate when the light changed, but it was very sluggish and felt like it was trying to take off from a high gear. I was less than 1 mile from a Toyota dealership when the malfunction occurred and was able to get the truck to the dealership. After a week of troubleshooting, the dealership informed me that the transmission was stuck in 6th gear due to a solenoid failure and they would need to replace the transmission. The transmission is currently on backorder with an estimated delivery between 10/14 and 10/26. The vehicle was taken to a local dealership for service, and Toyota refused to replace the transmission and torque converter as per their own repair procedure, claiming it must be replicated in order to authorize the repair. This is an already known fault by Toyota, yet their response was that a dangerous condition has to be duplicated even though vehicle diagnostic codes show that two transmission solenoids were stuck, and even though I provided them photos of the multiple warnings on the driver’s dash from when it occurred.
