Engineers from Tesla inspected the vehicle and said everything was working normally, but it continues to malfunction and almost causes accidents with cars behind in traffic having to slam on their brakes as well.

Sep 2024 SERVICE BRAKES,FORWARD COLLISION AVOIDANCE I was driving on a street at the speed limit there were no cars in front of me and behind me thankfully the car just braked very hard suddenly for no reason and I lost complete control over the vehicle after the car came to a complete stop I was able to continue driving. Thankfully there were no cars behind me because if there were I don’t even want to imagine the accident I could’ve caused aside from this the breaking was so hard that my whole body hurts and I am afraid to drive the vehicle as I do not know when to expect this breaking since it can happen so suddenly at any time.

Sep 2024 TIRES The contact owns a 2024 Tesla Model Y equipped with Continental Tires, Tire Line: Pro Contact, Tire Size: 255/45/R20, DOT Number: (N/A). The contact stated that several weeks after purchasing the vehicle, a neighbor informed the contact that a tire was flat. The contact attempted to pump air into the tire; however, the air immediately seeped out. Due to the failure, the vehicle was towed to an independent mechanic where the tire was replaced with the same brand tire. The contact stated that almost a month later, the tire pressure indicator light appeared on the infotainment screen while the contact's wife was driving the vehicle. The contact was called to the scene and attempted to inflate the rear passenger tire; however, the air seeped out of the tire. Upon inspection, the contact discovered multiple cracks along the inner sidewall which caused the tire to peel. The vehicle was towed back to the independent mechanic where the vehicle remained. The vehicle manufacturer was notified of the failure and referred the contact to the tire manufacturer. The tire manufacturer was notified and informed the contact that a tire representative would need to inspect the tire. The vehicle was not repaired. The tire and vehicle failure mileage was approximately 550.

Aug 2024 UNKNOWN OR OTHER,FORWARD COLLISION AVOIDANCE I was driving and going about 50mph and there was no other car near me. Suddenly the car slammed on its brakes. Dropped to probably 10mph. My mom and I were both wearing seatbelts but our heads were thrown forward. My neck and shoulder are sore and stiff today now. There were dark skid marks in the road ahead of us and I believe the car saw those and broke because of them. If there had been a car behind us, we would have be rear ended. There were no warnings beforehand. When the car broke, lights flashed and there was also an alarm. The vehicle has not been inspected by police or manufacturer.

Aug 2024 STEERING My 2024 model Y experienced a steering rack failure. It took three steering racks for Tesla to fix the issue. I was given the vehicle back and the same issue occurred two weeks later. This time it was worse, including failure of the entire power steering issue.

Aug 2024 UNKNOWN OR OTHER,LANE DEPARTURE,BACK OVER PREVENTION This is a complaint regarding a malfunction of the Tesla Full Self-Driving (FSD) feature, which resulted in damage to my vehicle and may potentially pose a significant safety risk to public. On 07/02/2024, my newly purchased Tesla [Model-Y 2024] was parked next to a pillar in a parking garage. After ensuring that the Full Self-Driving sign was on, I activated the FSD feature. However, as soon as the function was engaged, the vehicle instantly took off and made a right turn. This sudden and unexpected action caused the right side of the back passenger door to scrape against the iron guardrail on the pillar and caused damages to the right back passenger door and a portion of right back fender. This action happened so sudden that did not have the chance to react and take manual control. I contacted Tesla to report the issue and seek assistance, but after +4 weeks of contacting and restless following up with Tesla service teams, Tesla stated that the software was functioning properly and denied any responsibility for the malfunction, and they closed my case without offering any resolution or further investigation. Luckily, this incident happened in a confined parking garage. It could have resulted in more severe damage or injury had the circumstances been different. Additionally, Tesla's dismissal of the issue and refusal to accept responsibility is deeply concerning, especially about the reliability and safety of their Full Self-Driving technology. I sincerely request your office to investigate into this incident, and I hope you could advise any appropriate action to examine and address this issue. I attached a few photographs of where my vehicle was parked before the FSD was turned on, and the damages on both my vehicle and the guardrail at the time of the incident. Aug 2024 LANE DEPARTURE The contact owns a 2024 Tesla Model Y. The contact stated that while driving at an undisclosed speed with auto-polite activated, the vehicle suddenly pulled in the left direction across three lanes of traffic. As a result, the vehicle collided with the passenger-side front headlight and came to a complete stop in the roadway. The air bags did not deploy. No warning lights were illuminated. The vehicle was towed to an independent mechanic, who was unable to diagnose the failure. The vehicle was not repaired. A police report was filed, and injuries were reported. The contact transported himself to his primary care doctor, who treated him for nondescript injuries to his neck, shoulders, right hand, and hips. The manufacturer was notified of the failure, and a case was opened. The failure mileage was 9,000.

Aug 2024 SEATS Whenever turning a clicking sound is occurring underneath the driver seat. The seat does not seem to be properly secured from the factory. Tesla said it does not seem to be a safety concern over the phone and will not work to get my vehicle into the service department in a reasonable timeframe. Many weeks to schedule with no alternative.

Aug 2024 STRUCTURE My tesla frunk has opened twice without warning or command on, [XXX] and [XXX]. This is after the 2024.20.3 update that was supposed to fix it. Jason from roadside assistance had me do a soft reboot and hopes it won't open in the future. Thankfully both incidents occurred in the parking lot. Jul 2024 VEHICLE SPEED CONTROL The vehicle was under full self-driving mode and approaching a shopping center. There was a crossroad with signs on the corner. The vehicle was supposed to turn left but kept slowly approaching the signs on the corner. I was ready to step on the brake to stop the full self-driving mode while expecting the vehicle to turn left or step on the brake. Instead, the vehicle accelerated toward the signs, so I had to steer the wheel to the left and apply the brake immediately. The vehicle jumped a bit as it ran over the corner of the crossroad and stopped after about 10 feet. I was fortunate because I took action immediately, avoiding hitting other people or property. However, human reactions seem not to be quick enough to prevent some accidents as in this case. During this incident, the right corner of the front bumper of the vehicle hit the signs, and the top right corner of the vehicle (right above the front window) was also slightly damaged.

Jul 2024 UNKNOWN OR OTHER *Subject: Reporting Tesla Motors' Exposure of Sensitive VIN Information** To Whom It May Concern, I am a security researcher and ethical hacker who has identified and reported a significant data leak involving Tesla Motors. Specifically, Tesla is leaking sensitive Vehicle Identification Numbers (VINs) of its customers through an API URL. The URL in question is: [XXX] ` This exposure allows anyone to pull detailed recall reports of these vehicles from the National Highway Traffic Safety Administration's website: [XXX] ` Furthermore, these VINs can be used to search for private documents shared on social media, posing a serious privacy and security risk. Many vehicle owners intentionally redact or block their VINs in public documents to prevent misuse. Despite the clear implications, Bugcrowd analysts have closed my report as "not applicable" and claimed that it has "no security impact." This decision disregards the privacy concerns of Tesla EV owners, whose VINs are readily available to malicious actors. My attempts to bring this issue to light were further suppressed when Tesla removed my post from the Cybertruck Facebook group and blocked me from the platform. Given Tesla's notorious history of ignoring such violations, I am seeking intervention from relevant authorities to take appropriate action against both Tesla Motors and the Bugcrowd platform for their negligence in handling this critical security issue. Jul 2024 FORWARD COLLISION AVOIDANCE The incident occurred on [XXX] at [XXX] PST in Sammamish WA, where my car (2024 Tesla Model Y) incorrectly enabled Automatic Emergency Braking , and unexpectedly applied brakes when I was traveling at 45 mph on an empty road. The sudden braking reduced the speed of my car to less than 15 mph in a span of 2 seconds and eventually brought the car to a halt . I strongly believe this was done in error due to a system malfunction, as there was no potential cause of a collision given there were no incoming vehicles or any vehicles ahead. There were also no pedestrians or any objects such as animals or wildlife on the road. One this incident occurred, an alert with code “APP_W050” appeared on the car screen. (photo attached). Due to this abrupt unexpected braking, It caused a significant treat to my safety, as I could have potentially lost control of the car and slid off the road into a ditch or another vehicle traveling behind me could have potentially collided with me in the rear end. Jul 2024 ELECTRICAL SYSTEM,VEHICLE SPEED CONTROL,LANE DEPARTURE I was on [XXX] heading towards Chicago and following the traffic. I started changing the line to pass the vehicle in front of me and suddenly my tesla completely lost control and started drifting to the sides. I released the accelerator right away but it made it even worse. It seems that acceleration continues to happen that’s what made car to drift. Eventually car slowed down and I was able to make it to my line. I don’t understand why line assistance didn’t work to keep the vehicle stable, why after releasing accelerator the vehicle continued to gain the speed. It was sunny day and dry road. Jul 2024 SUSPENSION,STRUCTURE,WHEELS My Tesla (dark gray) was t-boned by a 2015 Hyundai (white) while it was passing the intersection on green light. The Hyundai hit my car when it tried to turn right on red light. My car was running at around 25 mph at that moment. About one second before the accident, my car's front collision warning went off, but everything was too late; the Hyundai's left front bumper hit my car. The crash did not cause any serious damage on the white Hyundai; as you can see in the photo, it barely got any dents, except its left front headlight cap was broken. The incident created small dents on my car's right fender panel and front right door. However, even though my car's front right wheel was not directly hit, it fell off during the accident. If my car was running at a high speed, it would have rolled over and caused serious injuries.

Jun 2024 STEERING The contact owned a 2024 Tesla Model Y. The contact stated while driving 60 MPH he began to lose control of the steering wheel which resulted in the vehicle crashing into a ravine and flipping over multiples with no warning light illuminated. The contact stated that three wheels detached the vehicle but was unsure if the wheels fell before the accident. The contact was taken to a local hospital for a trauma evaluation in which the contact suffered a concussion and bruises on his body. A police report was filed. The vehicle was towed to the salvage junkyard. The dealer was not contacted, and the vehicle was not repaired. The manufacturer was not made aware of the failure. The failure mileage was approximately 50.

Jun 2024 SERVICE BRAKES,FORWARD COLLISION AVOIDANCE During my trip, traveling at speeds ranging from ~30 to ~75mph my car suddenly and with no advance warning braked to a full stop, four separate times. I was not using cruise control or any driver assisted features. There were no obstacles in the road. I did not get in an accident because there were no other cars near me on the two different roads I traveled on.

Jun 2024 ELECTRICAL SYSTEM,UNKNOWN OR OTHER,SERVICE BRAKES The Tesla accelerated by itself while I am driving in a heavy traffic. To avoid hitting the front car, I braked several times, which didn’t work at all. It still accelerated, instinctively I steered wheels to the right front and hit a small hill. Airbags are out. I struggled to get off the car and shouted for help. 911 was called and emergency ambulance took me to ER. Luckily I am alive, it could be much worse that I would tremble when thinking of that crash scene again. This happened on [XXX].
