| Jul 2026 | POWER TRAIN,VEHICLE SPEED CONTROL,FORWARD COLLISION AVOIDANCE | While driving in Sport mode on an interstate, during descending downhill, the transmission started down shifting. At that time near the bottom of the hill was standing water on an area. It was suddenly like hitting the brakes & I hydroplaned, but luckily I never touched the brakes. The down shifting at the same time as hitting the water caused the vehicle to loose control of the road. Luckily I regained control by steering and did not wreck. There should be a safety warning on this issue or a correction in the vehicle down shifting | Crash: No Fire: No | Unknown |
| Jul 2026 | ELECTRICAL SYSTEM,EXTERIOR LIGHTING,VISIBILITY/WIPER | Almost all functions connected to the steering column quit working. I do not have working turn signals, high beams or wipers, unless the wipers are on high. None of the functions on the steering wheel work. It seems like a safety issue to not have functioning turn signals or properly finctioning wipers and high beams. No lamps or warnings. | Crash: No Fire: No | Unknown |
| Jul 2026 | ELECTRICAL SYSTEM,UNKNOWN OR OTHER | Vehicle was in Ford dealership’s service bay for an oil change service and reportedly caught fire inside the cab behind the dash due to an electrical system malfunction. The service technician stated that it was on the rack and just spontaneously caught fire inside the cab in the dash behind the infotainment system. | Crash: No Fire: Yes | Unknown |

| Jun 2026 | ELECTRICAL SYSTEM,UNKNOWN OR OTHER | VIN # [XXX] Ford Customer Service Current Case Number: [XXX] Integrated Trailer Module Failure after recall completion. NHTSA Campaign ID Recall # 26C10 was completed at Coconut Point Ford in Florida on 3/26/2026. This recall identified that when towing a trailer, the integrated Trailer Module may lose communication with the trailer possibly causing a loss of brake and turn signal lights or a loss of brake function. On [XXX] in the middle of a seriously steep and dangerous decent, the system reported that the trailer was disconnected from the truck causing, loss of trailer brake control which in turn cause a reliance on the truck. The antilock brake system failed moments later. I had to literally stand on the brakes to slow down to keep from going over the side of the mountain. At the end of the dirt road where it Ts onto the highway we could not stop. We travel with a 23 foot 2019 Vintage Cruiser manufactured by Gulf Stream. That weighs approximately 5200 lbs empty; I have a Cat Scale reading of 4840 pounds from a 12/6/24 weight with my previous truck. We do not travel with any water in our Black, Grey or Fresh Water tanks so even fully loaded we are still well within specs for this trucks towing capacity. The brake control on the truck was set to 5. Faricy Boys Ford in Canon City Colorado, verified that a recall had been completed and worked on the truck for 3 full days, testing many things including tailgate malfunctions and miscellaneous electrical systems trying to replicate the problem, no dealer has been able to replicate the problem. They finally replaced all the brakes and rotors as well as flushing the brake system. I respectfully requested that Ford Motor Company assign the review of this case to a regional Customer Service Manager / Engineer to review the complete repair history of this vehicle, via registered mail. We haven heard anything yet. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6) | Crash: No Fire: No | Unknown |

The dealer was contacted, the contact was informed that the parts were unavailable. The contact stated that he later received another recall notification, stating that the parts were unavailable. The dealer was contacted again. The contact was informed that the parts were on back order. The manufacturer was contacted, and the contact was informed that the parts were unavailable. The contact had not experienced any failure regarding the unrepaired recall repair. | Crash: No Fire: No | Unknown |
| Jun 2026 | POWER TRAIN,ELECTRICAL SYSTEM | 2024 Ford F150 tremor 14,000 miles The gearshift modual locks into position and locks the transmission (light blinking and transmission light shows on dashboard) You cannot shift to park or start the car, it will do this in drive or reverse and locks the transmission. A hard reset does not solve the issue. The First time this happened l was with my wife and two young children in the mountains around 30miles from cell service. This is a major safety issue. The dealership cannot replicate it, and therefore wont replace it even though it is a known issue. tsb 26-2066 I do have a video of this happening. Fortunately the second time this happened I was close to home. The vehicle occasionally leaves us stranded due to an un-replicated transmission lock flaw. | Crash: No Fire: No | Unknown |
| Jun 2026 | ELECTRICAL SYSTEM | The contact owns a 2024 Ford F-150. The contact stated that while driving at an undisclosed speed, the Forward Collision Avoidance system failed to function properly. The contact stated that while driving in traffic, after being cut off by another vehicle, the Forward Collision Avoidance system failed to detect or alert the driver of the issue, or activate the Forward Collision Avoidance Automatic Emergency Braking feature. The vehicle was taken to an authorized Ford dealer on two separate occasions due to the failures. During the first visit, the service department performed a diagnostic test but was unable to duplicate the failure. The contact was instructed to return if the failure persisted. During the second service visit, a technician performed a road test with the contact and successfully verified that the ADAS safety features were inoperable. The technician and the contact performed identical test drives on four brand new 2026 Ford F-150 vehicles at the dealer and experienced similar failures with the Forward Collision Avoidance system. The vehicle was not repaired. The manufacturer was notified of the failure. The failure mileage was 45,000. | Crash: No Fire: No | 45,000 |
| Jun 2026 | ELECTRICAL SYSTEM,ENGINE,FUEL/PROPULSION SYSTEM | While traveling at cruising speed (70 MPH) on Interstate 95 South, the vehicle suffered a catastrophic electrical and mechanical failure, resulting in an immediate engine stall. The vehicle lost all propulsion. Due to the sudden loss of power, the vehicle could not safely clear the active lanes of traffic, coasting to a stop near an exit ramp and dangerously blocking highway traffic.Upon attempting to restart the vehicle to move it out of harm's way, the vehicle emitted a rapid clicking noise and again stalled out. Because this vehicle utilizes an electronic shift-by-wire system, the total power failure permanently locked the transmission into "Park." The vehicle could not be shifted into "Neutral" or "Drive" to be pushed safely off the road. This total vehicle failure created an immediate, life-threatening hazard for the occupants and surrounding motorists on a high-speed interstate, requiring emergency police intervention to manage traffic until a flatbed tow truck arrived. | Crash: No Fire: No | Unknown |

| May 2026 | SERVICE BRAKES | I was driving by and received multiple warnings about brakes. Brake assist, one pedal driving, reverse brake assist, general brake warning. After the came on the brake were spongy with a hard brake after applying more pressure to the pedal. I needed to apply more pressure because the car was not stopping. I almost hit various cars on the way home because there is certainly an adjustment needed to my braking habits. Very dangerous and unsafe. I took the truck to the dealership and quoted me around $2,500. This is something I have no control over and see that there is a recall on this issue for a 2025 model, but my vehicle does not fall under this recall. | Crash: No Fire: No | Unknown |
| May 2026 | ELECTRICAL SYSTEM | APIM real‑time clock failure. Dealer refuses repair, states ‘software issue’ for 16 months. Clock resets instantly to 12:00, cannot retain time. Will also blink sometime to current time and then immediately back to 12:00. This displays both on my head unit as well as my heads up display. | Crash: No Fire: No | Unknown |
| May 2026 | ELECTRICAL SYSTEM,SEATS,UNKNOWN OR OTHER | I own a 2024 Ford F-150 Lightning Platinum built in August 2024. The vehicle has been at the dealership for over 45 days for severe, unresolved electrical safety defects. I have attached photographic proof of the main defects. The first three photos show the driver's instrument display screen behind the steering wheel completely blacking out on startup, blinding critical safety gauges like the speedometer. The center screen stays on, but the main cluster is totally dead. Fourth and fifth photos show the driver's seat memory failure. I manually set my lumbar support to a comfortable 3-4-3 setting and save it, but every time the truck sits, the seat completely deflates to a flat zero default on startup. This causes an immediate driving distraction and ergonomic hazard. The factory navigation also randomly shouts driving directions out of nowhere without any driver input. The dealership is currently trying to install a temporary "bypass wire" hack on my factory harness just to see if it fixes the seat memory. However, Ford's internal hotline (SSSC) refuses to authorize repairs for the black screen or phantom navigation until a tech physically catches it glitching on command. Ford’s policy is to return the vehicle unrepaired if it does not fail during active observation. I am requesting NHTSA to investigate Ford's active evasion of safety recall 25V540 (Ford 25S88). The official recall paperwork states the defective supplier software entered production on May 14, 2024. My truck was manufactured in August 2024—squarely within the defective window—yet Ford is using a model-year loophole to exclude 2024 vehicles from the recall population. They are denying owners a permanent factory software remedy and forcing dealers to use unapproved, temporary physical workarounds. The vehicle is at the dealership and available for federal inspection if need be. | Crash: No Fire: No | Unknown |
| May 2026 | POWER TRAIN | The contact owns a 2024 Ford F-150. The contact stated he received the notification letter for NHTSA Campaign Number: 25V512000 (Power Train). The vehicle was taken to the local dealer where it was inspected and informed the parts were backordered. The manufacturer was contacted who confirmed the part status and referred the contact to the local dealer. The contact had not experienced a failure. | Crash: No Fire: No | Unknown |
| May 2026 | POWER TRAIN | The contact owns a Ford F-150. The contact stated that during cold start up, there is a delay when reversing and resistance in backward movement. | May 2026 | POWER TRAIN,ELECTRICAL SYSTEM,ENGINE | On 4/22/2026 while flat towing this vehicle behind my motorhome, it burst into flames. The trip began in [XXX] . Driving south on XXX, I stopped about 120 miles later at a rest stop near XXX. I did a complete walk around inspection. There were no issues, everything looked fine. We continued on and about 20 miles later another vehicle driver was telling me frantically to pull over. I looked at my rear camera and saw the Ford was smoking. This was not due to an improper neutral tow setting. The truck would have burned in the first 20 miles. I have towed this vehicle many times. I know the importance of doing it properly., Thankfully there was a good place to pull over. I tried to extinguish the fire with my extinguisher but it was too hot. I was unable to disconnect from the motorhome. Both are a total loss. I think of all the places this could have happened and how trapped we could have been. By the grace of God, nobody was injured. Yes there is a police report and an insurance claim. There were no warning signs. My braking system runs on its own battery so there was no draw on the Ford battery. Ford did an update on the computer the night prior. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6) | Crash: No Fire: Yes | Unknown |
| May 2026 | SEAT BELTS | Seat belt sensors do not work correctly for back seat passenger. Seat doesn’t register people sitting in it or when seat belt is buckled sometimes. Issue was reported to dealer and taken for repairs u see warranty, but never completed. Causes issues when carrying passengers because i can’t tell if people are buckled. Sometimes driving down road it alerts randomly that seat belt was unbuckled and causes risk that could lead to an accident when I have to adjust it for a child while driving down the road. This is a wide spread reported issue for these trucks and should be recalled. | Crash: No Fire: No | Unknown |
| May 2026 | FUEL/PROPULSION SYSTEM | Dash said look for safe place to pull over. When turned engine off, it would not start again. Had to wait hours to get it towed to dealership. Been there over 5 weeks and they still don't know what's wrong but suspect hybrid battery problem. Only about 21k miles and 18 mo old. No loaner provided. | Crash: No Fire: No | Unknown |
| May 2026 | POWER TRAIN | Very hard down shifting at low speeds and it feels like the drive line is coming out of it!! Has been happening since brand new! I have reported it to the dealership and the said it was a quart low on fluid and they added a quart but the vehicle continues to do the same thing??? | Crash: No Fire: No | Unknown |

| May 2026 | ELECTRICAL SYSTEM | 2024 Ford F-150 Platinum PowerBoost Hybrid. Diagnostic codes for power steering loss and CAN bus communication faults documented at 337 miles, well within the 3-year/36,000-mile new vehicle warranty. The vehicle has been at a Ford dealer since March 10, 2026 for symptoms including the truck randomly turning off while driving and loss of power steering communication. As of this report, the truck has been at the dealer 58 days without resolution. Documented diagnostic findings include: Lost Communication with PSCM (U0131), SCP/J1850 Communications Bus Fault (U1028), permanent code P1A42 Propulsion System Status Signal Performance, hybrid-specific High Voltage System Interlock U3523-00, Steering Position Sensor P060A, no communication with Power Steering Control Module, intermittent PCM communication during self-tests, BCMC fuse 69 with no power, and circuit CBB69 with no voltage. A Ford Field Service Engineer was dispatched April 23, 2026. On April 24, 2026 the dealer located damaged wiring inside a harness behind the BCMC bracket. Prior to that, the dealer escalated through R&I of multiple modules (PSCM, PCM, BCMC, Gateway Module access) over 45 days without locating the actual cause. The vehicle is currently subject to NHTSA recall 26V104 / Ford 26C10 (Trailer Module loss of communication at startup) issued 3/18/2026, status Recall Incomplete. The recall describes the same general failure mode (module communication loss at startup) the customer has been experiencing. Power steering loss while driving is a serious safety concern, particularly given the vehicle's hybrid drivetrain and CAN-bus-dependent safety systems. | Crash: No Fire: No | Unknown |

| Apr 2026 | ELECTRICAL SYSTEM | While driving on a surface road in Columbia, MD at approximately 40 mph, the dash display of the 2024 Ford Lightning Flash went completely blank, leaving no indication of vehicle speed, battery percentage, battery temp, motor temp, cruise control status etc. Zero information was displayed, and the dash was completely cold and dark. This failure, while traveling at speed, represents a gross safety hazard, especially because the same LCD displays automatic emergency braking informaion etc. Immediately, I was able to pull over, and establish an OBD-2 connection to the ECM, and pull error logs. I also took screenshots of critical components and voltages. I cleared the codes, re-checked for codes and saved another error log showing a couple of codes. I then cycled power to the truck several times, hoping for a reboot to clear the error. I also performed a manual reset of the Ford SYNC system. Using the OBD-2 virtual dash on my reader, I was able to safely display speed and battery data, and drive immediately to the nearest Ford Dealer (Koons Ford). On the way, I phoned Crossroads Ford (Apex, NC) who did a remote error check using the vehicle's modem. The dealer may have saved this data. Upon arrival to Koons, service writers and technicians were able to witness and verify inoperability of the dash unit, and cycle the vehicle power, whereafter the problem persisted. They then took it back for service and evaluation. After approximately 4 hours, I was told that the "dash reset itself after a couple hours" and they could not get it to repeat the error after it came back. Before this, there were no warning signs of dash failure, nor since. | Crash: No Fire: No | Unknown |
