| Feb 2026 | VISIBILITY/WIPER | We are leasing a 2024 Chevrolet Equinox EV. We like the car, however there is a safety issue that has continually occurring. There is a very strong reflection on the windshield from the materials that make up the dashboard. It can occur in bright sun but also on bright cloudy days. I want to be clear, I am not being blinded by the sun in my eyes but rather the dash reflecting onto the Windshield and obscuring my vision. I’ve had two close calls because of this. I mentioned it to the Chevy service lady when I had an appointment and she said her boyfriend had exactly the same problem with his Equinox EV. He had to put a carpet piece on the dash to stop the obscuring reflection. | Crash: No Fire: No | Unknown |
| Feb 2026 | ELECTRICAL SYSTEM | Vehicle Safety Complaint – 2024 Chevrolet Equinox I am reporting a safety defect involving unintended vehicle movement and improper shutdown behavior. While operating my 2024 Chevrolet Equinox, I was able to shut the vehicle off even though it was not in Park. After shutdown, the vehicle rolled forward on its own and struck a tree. At the time of the incident, the radio remained on despite the vehicle being turned off, indicating the vehicle may not have fully entered ignition-off state. This incident suggests a potential malfunction involving one or more of the following systems: gear position detection system transmission park engagement mechanism electronic shift control module ignition state logic or body control module The vehicle should not allow shutdown unless it is in Park, or it should provide clear warnings and prevent unintended movement. This behavior presents a serious safety risk of rollaway incidents. I am requesting a full diagnostic inspection and documentation of all stored fault codes, including history codes, prior to any resets. Please also check for software updates, technical service bulletins, or known defects related to shift position recognition or ignition state control. This issue resulted in unintended movement and property damage and poses a significant safety hazard. | Crash: No Fire: No | Unknown |

| Dec 2025 | VISIBILITY/WIPER | excessive dashboard glare restricted clear vision of road and associated pedestrians and other vehicles. | Crash: No Fire: No | Unknown |
| Nov 2025 | FUEL/PROPULSION SYSTEM | At 5 am in the morning the car suddenly failed and indicated it was an engine or transmission failure in the middle of a four lane major intersection in a downtown area. It was dangerous due to the darkness, and poor visibility. I had to abandon the vehicle as it was dangerous to stay with it while waiting for police. I could have been injured (as could others) due to its sudden failure in the intersection of a busy roadway. Chevrolet dealership states it is the fuel pump driver module. The car is currently at the dealership for repair. There were no warning lamps, messages or other symptoms of the problem prior to the failure until the car suddenly lost power. Once it lost power the engine light lit up and shortly after I had exited the vehicle, I began receiving messages from "On Star" telling me that there were engine or transmission problems. Meanwhile I stood in the pouring rain in an unsafe neighborhood, leaning out into traffic to try and stop cars from running head on into my car for over 30 minutes waiting for police. | Crash: No Fire: No | Unknown |
| Nov 2025 | VISIBILITY/WIPER | Having issues with the windshield wipers clearing rain when driving and multiple camera malfunctions that I have an appointment to have both checked on 12/2. Wiper stops more than 4 inches from the edge of the window which I could probably just deal with, but when driving it pulls water anywhere from 3-8 inches further across the windshield making it very hard to see when it rains harder or you drive faster. I have owned more than 15 vehicles and never experienced anything making it difficult to see during inclement weather. The other issue is the cameras when backing up or going forward will malfunction for a day or two, then be good for a few weeks and malfunctions again sporadically. Have video of windshield wipers clearing rain issues, but it won’t let me submit, so if you want I can email. Pictures don’t show issue as well. | Crash: No Fire: No | Unknown |
| Nov 2025 | POWER TRAIN | My 2024 Equinox was returned to the dealership on approximately October 1, 2025, due to a check engine light related to the torque converter. I required a rental vehicle during this time. I picked up my vehicle on October 10, 2025, and all repairs were covered under my extended warranty. I was informed that my rental reimbursement would be issued once I submitted the required paperwork. My safety and the safety of my grandchildren were put at risk loss of power and control-stalling, in ability to accelerate in traffic, gear slippage and over heating Given the known history of torque converter issues in the Equinox—dating back as early as 2005—it is disheartening that GM continues to sell vehicles at premium prices while failing to properly support the customers who purchase them. | Crash: No Fire: No | Unknown |
| Nov 2025 | POWER TRAIN,ENGINE,FUEL/PROPULSION SYSTEM | As I was parked with the car running, my 2024 Equinox suddenly shut down and the check engine light came on. I tried to start it, but it didn’t. Someone tried to jump the battery, thinking it was that, but it wasn’t. I had it towed to the dealership and after 2 days they said the Fuel Pump Power Control Module needed to be replaced. Unfortunately, GM did not have the part, but the dealership found one in another state and we had to wait several days for it to be mailed to them. After a week, once the part was replaced, they tried to start the car and realized that the original issue fried the battery, so they had to replace that as well. I was told that they had seen several customers with the same issue and wouldn’t be surprised if GM eventually issues a recall. They also said they couldn’t be sure the replacement part wouldn’t have the same issue. I did some research and found that GM has had this issue with 2019 - 2024 vehicles. Some customers reported that they were driving and almost got hit by other vehicles when theirs shut down. My car only had 18000 miles when this occurred. I no longer feel safe driving my car. | Crash: No Fire: No | Unknown |

There was no warning light illuminated. The contact had taken the vehicle to a local tire shop, where it was determined that all four tires needed to be replaced due to premature wear. The contact stated that the wear marker was visible on all the tires. The vehicle was not repaired. The contact called the manufacturer and was referred to the tire manufacturer, who advised the contact that the tires were manufactured as requested by the manufacturer. The failure mileage was 25,316. | Crash: No Fire: No | 25,316 |
| Jul 2025 | STRUCTURE:BODY | The contact owns a 2024 Chevrolet Equinox. The contact stated that the A/C failed to work as intended. The vehicle was taken to an independent mechanic where a diagnostic test was performed. The independent mechanic informed the contact that the frame on the front passenger’s side near the radiator had been put together with Bondo and was not welded. The contact was informed bottom part of the radiator had fractured and was not intact, and the A/C lines were damaged, and the front bumper was damaged. The mechanic refused to repair the vehicle due to the liability and informed the contact to stop driving the vehicle. The contact took the vehicle back to the private dealer where the vehicle was purchased, and the dealer agreed to repair the vehicle; however, the vehicle was not yet repaired. The contact stated that when the vehicle was purchased a Carfax Report indicated that the vehicle had been in an accident prior to the purchase. The manufacturer was not made aware of the failure. The failure mileage was approximately 17,100. | Crash: No Fire: No | 17,100 |
| Jul 2025 | SERVICE BRAKES | here is the email I sent to GM. My name is [XXX] and I am having a issue with my 2024 Chevy Equinox VIN: [XXX] . There have been about 5 times that the brakes fail. Pushing the brake does nothing car just keeps rolling. Once I can get the car to stop and shut it down and restart the brakes once again work. It was in for service for a few days but there where no error codes and they could not replicate the problem. They did do a update on the car. While my car was in the shop I had the same problem with a Malibu rental. Called the dealership and talked to service and told them of the problem seems to be me somehow. Was asked if I use both feet to gas/brake the car as I was told there is a problem with that. No I don't do that but it seems I can catch both peddles with one foot and this is when the problem occurs. I have to make sure how I brake. Have found out today when it happened once again that if I put the car in neutral and press the gas the brakes start to work but not until then. There also was another issue today as when I did get the car to stop and put it in park I could not put the car back in gear until I shut the car off and restarted. Can someone let me know on how to proceed other then cautiously. I would take the car back in but they can fix a problem they don't see. Any insight would be appreciated. | Jul 2025 | ENGINE | On [XXX], about 400 miles into our trip heading South on [XXX], the Engine Reduced Power Warning came on. We were in the fast lane traveling at 70 MPH,the car immediately slowed to about 30 mph. There was no prior warning or check engine light. Vehicles behind us had to swerved and change lanes to avoid hitting us. The computer went into limp mode, when pressing the accelerator there was no power, we somehow made it to the right shoulder without getting hit and limped along to a safe spot and called a tow truck. Car was towed to an independent repair shop recommended by AAA, they ran the codes and came up with a code for High pressure Fuel pump. This vehicle is a 2024 Chevy Equinox with 15,018 miles on it, we bought brand new in April 2024 with 5 miles, always maintained at the dealer.Because we were 400 miles from home and the closest dealer in Bakersfield said they could not work on it for 3 or 4 days, we had to have the independent shop replace the fuel pump. We had to rent a car to continue our trip and picked up the car 2 days later. Replacing the fuel pump fixed the problem but the check engine light came back on about 30 miles later. The mechanic ran the codes and said it needed the computer re-flashed but it would be safe to drive home. After we got home we took it to the dealer, they re-flashed the computer and check engine light went off. After about 30 miles later the check engine light came back on, took it back to the dealer and this time the codes showed a bad catalyst converter, dealer replaced under warranty, only 15728 miles on vehicle at this point. So far 200 miles and light is still off.I I filed a complaint with General Motors customer service to let them know of this problem. I also requested to get reimbursement for the rental car and fuel pump. They refused to reimburse us because we didn't have a dealer do it, regardless of the situation. The public needs to know of this dangerous situation with the model. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552 | Crash: No Fire: No | Unknown |
| Jun 2025 | WHEELS | Report of Dangerous Rental Vehicle and Serious Safety Incident – Rental Agreement #289216922 I am submitting this report to formally document a serious safety issue involving a rental vehicle I received from Avis in May 2024. Several safety components malfunctioned and put the safety of my and my passenger in a life threatening situation. At the time of pickup, the vehicle had a visible dent near the front right tire, which was acknowledged by the rental agent and dismissed as cosmetic. I also noted a faulty rear hatch, which opened unpredictably and struck occupants in the head on multiple occasions as it behaved erratically. Over the course of the rental, the vehicle displayed additional signs of mechanical failure — including instability at highway speeds, loud squealing, and parts of the bumper physically falling off during the trip. On May 30th, while driving at highway speed, the vehicle suffered a sudden tire blowout. Critically, the onboard system misidentified the location of the blown tire, indicating the front right tire had failed when it was actually the rear right. This misinformation delayed proper response and nearly resulted in a rollover. We were stranded on the roadside for over an hour, narrowly avoiding several collisions. A critical system failure. A responding tow truck driver observed signs that the vehicle may have previously been involved in a collision — which aligns with the visible damage present when the vehicle was picked up. Avis has, despite multiple attempts, refused to confirm whether the vehicle was removed from service after the incident or whether any safety inspection was conducted -- we remain severely concerned for the safety of anyone else using this vehicle. This vehicle should not have been rented in its condition. The failure of safety systems, mechanical instability, prior collision damage, and the loss of exterior components created a serious and unacceptable risk to the driver, passenger, and others on the road. | Crash: No Fire: No | Unknown |
| Jun 2025 | ELECTRICAL SYSTEM | Electronics on car entirely go out including: Headlights, Turn signals, Hazard lights, Airbags, Electronic braking, Traction control, Tire pressure monitor, Fuel gage measurement, Speedometer, Odometer, Temperature gage and All Advanced Driver Assistance Systems. The vehicle has been to the dealership on 4 separate occasions from May 16th - June 27th 2025 Various inspections and repairs have been confirmed and completed by Dealership: Empire Chevrolet of Hicksville located at 236 South Broadway, Hicksville, N.Y. 11801. After driving for about 1 day the problems persist. The car is being brought back to the dealer tomorrow 6/28/25 for a fourth service call for the same issue. Driver and passenger safety is at risk due to lack of headlights, turn signals, hazard lights, airbags, electronic braking and speedometer etc... Warning lights appear simultaneously with electronics malfunction | Crash: No Fire: No | Unknown |
| Jun 2025 | POWER TRAIN | The biggest issue is last year while driving on 74 in busy traffic, the car automatically went into a "low power" mode and limited the power without warning. So now in traffic driving 55 mph, the car drastically slowed to about 35 without warning. It would not go faster no matter what I did. Then the check engine light came on afterwards. Thankfully I made it home safely after turning on my hazards and swerving through heavy traffic. I took it to the dealership and they didn't find any issues... of course. It did it again I took it back. They didn't find anything and then it stopped. Now fast forward today... my daughter drove to the store and the car cut off in the middle of the street. The way she explained it to me it was similar to what happened before. Except it cut all the way off this time. Same error on dash something like, "switched to low power." So I'm panicking. My husband and I go down and I try to start, it wouldn't start a few times. But eventually it did then cut off. Then it cut on when I kept slightly on the gas like I know to do with an older car. So... long story long... I got us home but with limited power and hazard lights. I'm glad we made it home safely and my daughter was shook up but she's safe. I'm terrified to let anyone else in the car let alone drive it. It could've been deadly. | Crash: No Fire: No | Unknown |

| Jun 2025 | POWER TRAIN | To Whom It May Concern, I am writing to formally report a serious and recurring safety issue with my 2024 Chevrolet Equinox, and to request immediate investigation and resolution. The vehicle has, on multiple occasions, unexpectedly gone into “reduced power” mode (also referred to as “limp mode”), drastically limiting acceleration while driving at full speed in traffic. This most recently occurred while my teenage daughter was behind the wheel, causing the car to completely shut off in the middle of the street. The first time this happened, I was traveling at approximately 55 mph on Highway 74 when the vehicle suddenly decelerated to about 35 mph without warning, despite my attempts to accelerate. The check engine light appeared only after the power loss. We narrowly avoided a serious crash. We had to engage hazard lights, swerve through traffic, and coast the vehicle home. The dealership was unable to replicate the issue during follow-up visits, and the issue stopped temporarily—but has now returned. This is more than an inconvenience; it is a critical safety risk to myself, my family, and others on the road. Details: Vehicle: 2024 Chevrolet Equinox Issue: Sudden power loss while driving; engine enters limp mode without warning Occurrences: Multiple (including full vehicle shutdown) over the course of ownership Dealership Response: "No issues found" – despite multiple visits Diagnostic Error: Dashboard reads “Engine Power is Reduced” Concern: Loss of power at high speed, dangerous stalling in traffic | Crash: No Fire: No | Unknown |
| Jun 2025 | SERVICE BRAKES | Driving at abut 10 mph, my brakes did not respond to pedal pressure... I had to stomp very hard on the brake pedal to have any slow down effect.... various warning messages flashed across the dashboard (see attached files)... After stopping, I turned off the car and turned it back on, the problem continued to exist..... then, after about 1 hour, I tried again and all systems were working fine. I took the car to Chevrolet Service, and apparently there were no OBDII codes stored in the vehicle. | Crash: No Fire: No | Unknown |
| Jun 2025 | POWER TRAIN | Traveling on the highway with a speed limit of 70 mph. Vehicle would not accelerate above 35 mph resulting in the potential to be rear ended and cause an accident. After three attempts by the GM dealership to repair the vehicle this condition still exists. I have decided due to safety concerns to not operate this vehicle. | Crash: No Fire: No | Unknown |
| Jun 2025 | POWER TRAIN | The transmission would not engage in any gear while I was in the path of oncoming traffic | Crash: No Fire: No | Unknown |
| Apr 2025 | FUEL/PROPULSION SYSTEM | Started into traffic and vehicle quickly slowed way down and then started to move again. At next light power lost again. Do not remember if warning light came on but it was not there prior to the incident. Towed to dealer. Fuel pump replaced. One month later accelerating to exit in heavy traffic it lost power. Engine light came on saying something to the effect engine power lost. It Started back up quickly and I got safely where I was going. It was running ok so I finished another 400 miles of trip. Dealer service tech on phone said probably stabilizer did what it’s suppose to do and probably avoided something serious. Not too much driving since then but I can tell I don’t have the pick up I should have when accelerating. Afraid each time I give pressure to the pedal that it might stall again as it doesn’t pick up speed as I’ve been used to. Taking to dealer for 30,000 mile service and to check this out. Was told if no dash light warning they could check it out but probably will check out fine. | Crash: No Fire: No | Unknown |
