| May 2026 | UNKNOWN OR OTHER,SERVICE BRAKES,FORWARD COLLISION AVOIDANCE | “The vehicle experiences phantom braking while driving, where the Collision Mitigation Braking System (CMBS) activates without warning even when there are no vehicles or objects in front of me. The vehicle suddenly brakes on its own, creating a major safety hazard and risk of rear-end collisions. The issue has happened multiple times during normal driving at different speeds. Warning lights related to the collision mitigation system and passenger airbag system have appeared intermittently. The blind spot monitoring system has also failed to properly detect nearby vehicles at times. The vehicle was previously taken to the dealership/service department and remained there for an extended period for inspection and attempted repairs, but the problem was not fully corrected. After getting the vehicle back, the same symptoms and braking issues continued. This issue puts myself, my passengers, and other drivers at risk because the vehicle may suddenly brake unexpectedly in traffic. | Crash: No Fire: No | Unknown |
| May 2026 | UNKNOWN OR OTHER | Our sunroof on our Honda 2022 Pilot spontaneously shattered while we were sitting at a stop light. Nothing fell on it and nothing hit, it just suddenly shattered while we were sitting in the stop light. | Crash: No Fire: No | Unknown |
| May 2026 | ELECTRICAL SYSTEM,BACK OVER PREVENTION,FORWARD COLLISION AVOIDANCE | My 2022 Honda Pilot failed to engage Drive after reversing, despite the Drive button being pressed. This resulted in an unintended backward motion & a collision with another vehicle. 1.What component or system failed or malfunctioned…? The Electronic Transmission Shift-by-Wire system (push-button shifter) failed to respond to a gear change request. Additionally, related electrical systems including the dashboard infotainment system & the integrated USB charging ports have malfunctioned. The vehicle & its components are available for inspection upon request. 2.How was your safety or the safety of others put at risk? The vehicle experienced unintended motion. While attempting to move forward in a parking lot, the vehicle remained in Reverse despite selecting Drive. This failure led to a low-speed collision with a truck’s tow hitch. This defect creates a high risk of crashes, injuries to pedestrians, or property damage because the vehicle does not move in the direction the driver has selected. 3.Has the problem been reproduced or confirmed by a dealer or independent service center? The vehicle was previously serviced at a Honda dealership for a radio/infotainment recall. Since that service, the transmission has failed to engage Drive multiple times at random. 4.Has the vehicle or component been inspected by the manufacturer…? The vehicle was inspected during the initial recall fix. And brought back because after the fix because the charging port stopped working/continued screen glitches. Following the recent collision, it is awaiting further inspection by a Honda dealership to determine the extent of the mechanical failure. 5.Were there any warning lamps, messages prior…? Symptoms began after the recall fix & included intermittent electronic failures of the charging ports/screen. There were no specific transmission warning lights prior to the gear selection failure; however, the persistent electrical issues in the dash area suggested a systemic electronic malfunction. | Crash: Yes Fire: No | Unknown |
| May 2026 | FORWARD COLLISION AVOIDANCE,LANE DEPARTURE | I turned my car on one day and warning lights for all of my safety sensing features came up. Intermittently they work for a little bit. I brought it to the dealer, and they said that there was damage to the sensor, but there has been no damage to the front end of the car. Their reply was perhaps a pebble came up from the road while I was driving. It seems to me that that is a fault in the manufacturing if it is that easy for all of the sensing features to stop working. I attempted to sort this out with the dealer and with Honda corporate offices with no luck. | Crash: No Fire: No | Unknown |
| May 2026 | ELECTRICAL SYSTEM | This is the recall information on the Honda site. The failure of the backup camera on our vehicle is what is described in the recall, yet the Honda site does not include our vehicle. I request that our vehicle be included in the covered repair. Quoted from the Honda site: 2019-23 Passport/Pilot MOST FAKRA Service Cables Safety Recall NHTSA Recall Number23V-431 MFR Campaign XXX Campaign Description2019-23 Passport/Pilot MOST FAKRA Service Cables Safety Recall Recall Date06/15/2023 Recall Status17 Character VIN Required Summary American Honda Motor Co., Inc. (Honda) is recalling certain 2019-2022 model year Pilot and 2019-2023 model year Passport vehicles. The Media Oriented Systems Transport (MOST) communication coaxial cable was improperly designed and manufactured, resulting in deformed terminals and potentially causing poor, or lost, connections intermittently between the audio display unit and the vehicle cable connector. Safety Risk If the MOST communication is lost for more than 20 seconds, the rearview camera function will fail when the vehicle is in the reverse gear, increasing the risk of a crash or injury. Remedy Registered owners of all affected vehicles will be contacted by mail and asked to take their vehicle to an authorized Honda dealer. The dealer will install an improved MOST cable harness between the existing display audio and vehicle terminal connections and a straightening cover over the vehicle cable connector to properly connect the audio display unit. Owners who have paid to have these repairs completed at their own expense may be eligible for reimbursement, in accord with the recall reimbursement plan on file with NHTSA. | Apr 2026 | ENGINE | On April 3,2026 we are starting back from a road trip to Florida from Kentucky (starting odometer 69,199 miles)oil changed prior to leaving in our 2022 Honda pilot. Stopped to get gas and transmission alert code popped up-it would not go into any type of gear. We had no warning previously to this. We were still 3 hours from home, had to get it towed to nearest Honda dealership where they stated it was an electronic module failure but transmission would have to be replaced entirely. Car was out of standard power-train warranty of 60,000 miles d/t this road trip (odometer now over 70K) and we had no extended warranty on the vehicle. We requested assistance for transmission replacement from American Honda. We filled out the required form on 4/9/26 explaining we are a loyal Honda customer(own an additional Honda-CRV), had good service records, etc. It was assigned to a case manager on Fri 4/10-they called left voicemail that it was under review and asked us to call back. We immediately called back on Friday 4/10 and left message-no call back. The following week we called back everyday to American Honda and finally got our case manager over a week later on Monday 4/20-said it would be 5 more days. Heard back today 4/24/26 that Honda denied our assistance request saying it was out of warranty and there are no known manufactorer defects or recalls at this current time. Made us wait 3 weeks to decide we will have to eat cost of a new transmission in a car that is less than 4 years old and <100K miles on it. Very disappointing in Honda corporation especially since there are numerous known complaints, civil law suits, etc on various online forums and in the national news of same issues and they aren't taking any responsibility leaving owners with very high repair expenses. Our transmission will be over $8000 to replace out of pocket. | Crash: No Fire: No | Unknown |
| Apr 2026 | POWER TRAIN,ENGINE,FUEL/PROPULSION SYSTEM | Upon accelerating from a stop, or at various speeds, the vehicle experiences a significant loss of motive power(pulling backwards) followed by an abrupt lunge forward. This hesitation occurs unpredictably and makes entering traffic or intersections extremely hazardous. This feels like a fuel delivery or transmission engagement failure. The last incident occurred on or around the afternoon of March 23. My vehicle has been examined at the dealership on multiple occasions for this same issue and each time I have been told that they are unable to replicate the issue and can not help me further unless they are able to replicate the issue. | Crash: No Fire: No | Unknown |
| Apr 2026 | SERVICE BRAKES | Each time my battery has died the brake starter has also stopped working. I have had to get the starter replaced three times in 4 years. The starter for the Honda pilot 2022 is faulty | Crash: No Fire: No | Unknown |
| Apr 2026 | ENGINE,FUEL/PROPULSION SYSTEM | While driving on the expressway at speeds above 60 mph and while attempting to pass another vehicle the vehicle shuts down with no acceleration. The vehicle will then after several seconds regain acceleration. Several times it has sounded like the vehicles starter engaged. This is a random occurrence and has happened approximately 10 times. There are no error codes and the dealership states that they can not recreate the issue so there is nothing wrong. | Crash: No Fire: No | Unknown |
| Apr 2026 | ELECTRICAL SYSTEM,VEHICLE SPEED CONTROL,ENGINE | I was operating a 2022 Honda Pilot and had just made a left turn into a parking lot. I was slowly pulling into a parking space directly in front of a dance studio with a large floor-to-ceiling window. There were four children and a dance instructor standing inside near the window. As I was coming to a stop to park, my foot was on the brake. At that moment, the engine suddenly revved loudly and the vehicle accelerated forward unexpectedly without any additional throttle input. There were no warning lights, messages, or prior symptoms before this occurred. Despite braking, the vehicle continued forward and struck the exterior of the building. I was able to avoid the window area where the children were standing. The airbag deployed, and the front passenger side of the vehicle was damaged. The building was not structurally damaged, and no one was injured. There were no obstructions such as floor mats interfering with the accelerator, and there was no unintended pedal input. My foot was on the brake at the time of the incident. This appears to be a case of sudden unintended acceleration. The issue has not been reproduced or confirmed by a dealer or service center, and there were no prior warning signs before the event. The vehicle is available for inspection upon request. It has been towed to the body shop. The problem has not been reproduced or confirmed by a dealer or independent service center at this time. The vehicle was inspected by insurance and they did not total it. It is going to be repaired. The vehicle has been inspected by insurance representatives. The manufacturer has not inspected the vehicle at this time. I did not call police because I was not hurt, the kids were not hurt and the building only had a couple of scratches on the brick. | Crash: Yes Fire: No Towed | Unknown |
| Mar 2026 | ELECTRICAL SYSTEM,ENGINE | When the vehicle is stopped at a light and the auto engine idle engages, it intermittently stalls or shuts off the engine and unable or struggles to restart. The most recent time, the Pilot had to be shut off completely and restarted while sitting at a newly turned green light with large semi trucks behind us. When the auto engine idle is turned off there are no problems. This has happened 5 times over the course of the last 2-3 months. | Crash: No Fire: No | Unknown |
| Mar 2026 | FORWARD COLLISION AVOIDANCE,LANE DEPARTURE | Without any warning or incident, The multipurpose camera has failed and several systems are no longer available. DTC U3000-49 This includes, cruise control not working as well as auto high beams and autobraking | Crash: No Fire: No | Unknown |
| Mar 2026 | ELECTRICAL SYSTEM,FORWARD COLLISION AVOIDANCE,LANE DEPARTURE | The front windshield camera is causing the brake system error light to come on and triggering the safety system to malfunction. This failed camera system disables safety features and causes lane assist, cruise control , lighting features, back-up camera, driving assist, and the collision system to be disabled, causing a major safety concern. This front camera system has failed and has been diagnosed by the Certified Honda dealer that my vehicle was purchased from. My vehicle is 4 years old and has 35k miles, and the system has failed. Each component was inspected and issues diagnosed by the dealer. The warning light first displayed on the dashboard and showed as a "brake system" error light. Under the break system error 6 errors displayed they read : 1) adaptive cruise control problem 2) Lane keeping assist problem 3) collision mitigation system problem 4) Road departure mitigation system problem 5) auto-high beam problem 6) transmission system problem | Crash: No Fire: No | Unknown |

| Mar 2026 | ELECTRICAL SYSTEM | The car failed to start after stop at a traffic light. It happened few times. I replaced old battery with a new one. The problem still persists. Mechanic checked the battery health and found to be good. | Crash: No Fire: No | Unknown |
| Feb 2026 | ELECTRICAL SYSTEM | I have received a rejected emission test due to the computer not updating. Honda demands 200 diagnosis for this. and I have read it reported several times to years 2019 - 2023 | Crash: No Fire: No | Unknown |
| Feb 2026 | POWER TRAIN | Vehicle: 2022 Honda Pilot Elite AWD Mileage at first occurrence: ~30,000 miles Current mileage at worsening/repeatability of issue: ~48,000 miles My 2022 Honda Pilot Elite experiences a loss of propulsion during cold starts, most commonly after using remote start. The issue occurs when the vehicle is cold, shifted into Drive, and remains stationary for approximately 10–15 seconds. Under these conditions, the vehicle displays the message “Stop driving when safe. Transmission problem. See manual,” disengages the transmission, and shifts itself into Neutral. The vehicle will not move until it is shut off and restarted. This condition has occurred multiple times and is repeatable under the same circumstances. There are no other warning lights present, and restarting the vehicle temporarily restores normal operation. I reported this issue to my Honda dealer. The dealer contacted Honda North America technical support, who stated they are aware of this exact issue and the specific sequence of events that causes it. Honda advised that they do not consider this behavior a defect and declined to provide any repair guidance or corrective action. This condition results in an unexpected loss of drive capability and could pose a safety risk if it occurs in traffic or at an intersection. The issue can occur during normal vehicle operation and does not involve misuse of the vehicle. | Crash: No Fire: No | Unknown |
| Feb 2026 | UNKNOWN OR OTHER | Idle engine restart failed. The vehicle went in idle mode at stop light, whe I got green and moved feet off brake to acceleration the engine failed to restart. It happened on 2/3/2026. I turned on hazard light to alert other drivers I was in trouble. I kept getting message on dashboard to put car in park, press the brake and push engine start. It did not work for a while I stepped out of vehicle unlocked stepped back in then I could start the engine. The second occurrence was 2/7/2026. This was a major intersection. Same thing happened, situation lasted longer, traffic built up behind me I had to call 911, I step out the vehicle to open the hood in addition to having hazard light on for my safety. I tow truck came along. When he open vehicle and got in, the engine started. I was at dealership today 2/11/2026 they indicated no fault was found. While waiting for my appointment today I have been turning off the automatic idle feature every time I get in the car. | Crash: No Fire: No | Unknown |
| Feb 2026 | ELECTRICAL SYSTEM,FORWARD COLLISION AVOIDANCE,LANE DEPARTURE | Lots of warning lights going off in car. Took it to the dealership and was told it’s the front camera and would cost over $2k to fix. Car is new, less than 5 years old. Should not be having this issue on a new car. After researching found there are lots of people with the same issue. Seems to me it’s a manufacturing issue and Honda should be responsible to fix. | Crash: No Fire: No | Unknown |
| Jan 2026 | STEERING | 2022 Honda Pilot - steering wheel / front of car started to make a clanking noise when driving over speed bumps / noise started around 35k miles on odometer. Two different service appointments with Honda pointed to shocks and sway bar links; 3rd non honda service appointment found the issue to be a failing rack and pinion. 3rd service appointment replaced the rack and pinion / sound is gone and vehicle steers tight once again. mileage was 44k when replaced. | Crash: No Fire: No | Unknown |
| Jan 2026 | FORWARD COLLISION AVOIDANCE | It is constantly breaking when cars are coming the other way. It almost caused a vehicle to almost hit me. Also the auto shut off has almost caused accidents because it causes my car to stall and I wish I could keep it off and not have to push the button off every time. | Crash: No Fire: No | Unknown |
| Jan 2026 | UNKNOWN OR OTHER | While driving on I-26 at 60 MPH at approximately 2PM on 01/08/2026 the sunroof exploded. It was not struck by anything, no other vehicle was within a half mile of us at the time. Fortunately the cover was closed because we do not use the sunroof. It has only been opened one time a the delership the day the car was delivered new. This sunroof explosion was loud and frankly scared the heck out of us. Had the cover not been kept closed we would have been showered by broken glass. This could have easley resulted in a driver panic and a crash. I feel this is a manufacturing defect and based on numbers of events through various Honda models and years should covered by Honda recall/warranty coverage. | Crash: No Fire: No | Unknown |
| Jan 2026 | ENGINE | Engine fails to restart on its own from a complete stop at a traffic light or road intersection with the Auto Idle Stop (AIS) function engaged. When I release the brake pedal, the vehicle automatically goes into neutral, and when I try to put into Drive, nothing happens, then it takes multiple attempts to turn the vehicle off. Once it turns off, it does turn back on, and I can continue driving. This behavior is hard to reproduce consistently, it's happening from time to time. | Crash: No Fire: No | Unknown |
| Dec 2025 | ENGINE,FORWARD COLLISION AVOIDANCE | As I was traveling from Houma, La on a major highway to an out of state meeting. I started to hear a slight tapping sound and pulled over to inspect the sound. Upon inspection it seemed as though it was coming from the lower engine, or exhaust near the engine area. I located the nearest Honda dealership which was Community Honda of Lafayette with intentions of dealership inspection. It was approximately a 20 minute drive from the current location. As I was approaching the exit to the dealership the warning system lit up an check engine light. The Manager at the dealership took the vehicle to the back and ran a scan and responded that it sounds like an exhaust leak. I got back on the road for my destination. Approximately an hour later while I was on [XXX] all of the warning lights lit up at once, the engine jerked and shut off. I pulled over to inspect the vehicle again. I restarted the vehicle to work myself to a safe exit. The vehicle would not drive over 20mph. Once at a safe location I made concessions to have the vehicle carried back to my local Honda dealership, Barker Honda in Houma, La Upon inspection by the Honda technicians it was found to have irreparable damage to the engine internals. Technician states this could occur to low oil and/ or lack of maintenance; however, the technician was perplexed by the failure because all maintenance and records in their system showed that the vehicle was properly maintained by their staff. | Nov 2025 | LANE DEPARTURE,FORWARD COLLISION AVOIDANCE | The ADAS camera has failed. It is extremely common. When it fails, all the lights for EVERY system of ADAS comes on. None of them work. And it is not possible to turn off. Furthermore it sets codes for the brakes and turns on the brake warning lights. So none of the brake assist systems work either. Honda refuses to warranty any of these systems when the camera fails. It is across their entire line of vehicles with ADAS. If they can’t build a functional system, it should not have the system. Also, the system functions extremely poorly, when it was operating. It will drive the vehicle, unassisted, down to 20 mph, then just shuts off and lets the vehicle coast, unguided. Also, if the cruise is set and lane keeping on, and it can’t find lane lines, particularly where lanes merge, it will often veer left, into oncoming traffic. I have tested both of these, when the system was functioning “correctly”. The cruise and lane keeping just shuts off, at any speed, and lets the car go where ever it would without input. It is extremely dangerous. | Crash: No Fire: No | Unknown |
