May 2026 WHEELS To Whom It May Concern: I am writing to formally notify you of a serious and dangerous safety defect involving the vehicle I purchased from Carvana on February 2, 2026. On April 9, 2026, while driving on the highway at 70 MPH, my truck stopped abruptly after experiencing a critical wheel failure. Upon exiting the vehicle, I notice that all the lug nuts were missing. I immediately located a tow driver that confirmed what I observed. The tow company towed the vehicles to a Dodge mechanic in Paducah, KY. Upon inspection by a certified mechanic, I was informed that the wheel failure was caused by either a improperly installed loose or too tightened lug nuts, resulting in damage to the wheel hub and wheel assembly, which condition represents a severe safety hazard that could have resulted in catastrophic injury or death. Prior to the malfunction, there were no warning lamps or messages until the actual event. When purchased, Carvana represented that its vehicles undergo thorough inspections and are safe for operation. However, this incident demonstrates a clear failure in inspection, preparation, or mechanical servicing, including but not limited to: •Failure to ensure lug nuts were properly installed and secured •Failure to identify an obvious and critical safety issue •Delivery of a vehicle that was not roadworthy at the time of sale A wheel detachment risk is not a minor and or normal wear and tear as stated by Carvana's warranty company Silver Rock Warranty and Carvana Warranty support team. The malfunction was a fundamental safety failure that any reasonable inspection would have identified and corrected prior to sale.

Apr 2026 POWER TRAIN,EXTERIOR LIGHTING The contact owns a 2021 Jeep Grand Cherokee. The contact stated that while driving at various speeds, the vehicle hesitated while depressing the accelerator pedal and attempting to overtake another vehicle. The RPM was extremely elevated; however, the vehicle failed to respond as intended. The contact pulled over to the shoulder of the roadway and restarted the vehicle before resuming normal driving. The contact became aware that, when shifting between automatic and manual driving modes, the vehicle operated normally without hesitating. The vehicle was taken to several dealers on several occasions; however, the dealer was unable to duplicate the failure or determine the cause of the failure. In addition, the headlights occasionally shut off unexpectedly while driving at night. The contact would pull over and wait for the headlights to return before resuming normal driving. The manufacturer was notified of the failure and informed the contact that there were no recalls associated with the VIN for the failure. The vehicle was not repaired. The failure mileage was approximately 50,000.

NATIONAL HIGHWAY TRAFFIC SAFETY ADMINISTRATION Vehicle Safety Complaint Complainant: [XXX] [XXX] [XXX] Vehicle: 2021 Jeep Grand Cherokee ------------------------------------- SAFETY DEFECT COMPLAINT – POST-REPAIR VEHICLE CONDITION I am reporting a serious safety concern involving my vehicle following collision repairs performed by Gerber Collision & Glass under a claim handled by State Farm Insurance. After the repair, the vehicle began exhibiting multiple safety-related issues, including: - Steering instability and shaking - Engine performance issues and abnormal noise - HVAC malfunction - Persistent chemical and/or coolant odors - Physical symptoms while driving, including dizziness, lightheadedness, and shortness of breath I returned to the repair facility with photographic evidence showing parts with visible wear and damage that appear inconsistent with proper replacement. Despite this, no diagnostic evaluation was performed, and no corrective action was taken. The repair facility has refused to address the condition of these parts, and the insurer has refused to authorize further inspection. Additionally, there are discrepancies between the insurance estimate and the final repair invoice, raising concerns about whether the documented work matches what was actually performed, including work involving structural components such as the cradle/subframe. Apr 2026 ELECTRICAL SYSTEM,ENGINE This submission is an update to NHTSA Complaint #11727024. The vehicle continues to experience recurring engine overheating and misfire conditions. These issues have been previously diagnosed and are consistent with known manufacturer Technical Service Bulletins (TSBs) related to this engine. Despite this: The dealership has not performed the appropriate diagnostic protocol (including STAR testing) The manufacturer, Stellantis, closed my case without resolving the issue No repair or corrective action has been completed The vehicle remains in operation due to lack of manufacturer support. This condition presents a potential safety risk, including: Loss of power while driving Risk of engine failure or shutdown Increased risk during normal roadway operation The vehicle is regularly operated by my 17-year-old daughter, which increases concern regarding driver safety and reliability of the vehicle under these conditions. Mar 2026 LANE DEPARTURE My son’s car drifted to the right past the white line and he ended up hitting a hydrant and continued at least 100 ft before stopping. Our dash cam showed a slow smooth drift to the right without any lane assist being activated to pull the car back. All safety features were activated and turned on the day we bought the car for him. It was the main reason we chose this model. He is very lucky to be alive. After the impact, the car just barely missed a business sign, a telephone pole and a tree. The car was declared totaled. Unfortunately, the total amount the insurance is offering is about 4k short of what we still owe. Words cannot express how lucky we are that our son is okay, but we are very angry that the features we bought the car for, failed him.

Mar 2026 ELECTRICAL SYSTEM,ENGINE I am reporting a repeated safety-related issue involving my 2021 Jeep Cherokee Overland. The vehicle has experienced ongoing engine misfires and overheating, which create unsafe driving conditions. The issue presents as follows: -Engine begins to misfire, causing rough operation and shaking. -The check engine light flashes intermittently indicating active misfire. -Vehicle performance becomes inconsistent, with concern for loss of power while driving. -Vehicle experiencing coolant loss/overheating, concern for engine failure. These conditions create a serious safety risk, as the vehicle may lose power unexpectedly or become inoperable while driving. Multiple repair attempts have been made, including replacement of ignition components (coils and spark plugs) and other related service. The issue continues to recur, and the root cause has not been resolved. Despite repeated attempts to resolve the issue through the dealership and manufacturer, the problem persists and the vehicle remains unreliable. The manufacturer has not provided a resolution despite multiple attempts, and communication is unsuccessful I am submitting this complaint due to ongoing safety concerns and a lack of resolution.

Mar 2026 ELECTRICAL SYSTEM,UNKNOWN OR OTHER,FUEL/PROPULSION SYSTEM I was driving my car and the car shut off at 30 mph. It completely stopped and the steering wheel locked up. Started flashing start/stop and told me to put it in park. Glad no one got hurt and hope it doesn’t do it again.

Mar 2026 SUSPENSION,ELECTRICAL SYSTEM I own a 2021 Jeep Grand Cherokee Trailhawk which we purchased new. On Dec 30, 2025 we were driving home and received a service suspension message on the dash. We took it to our mechanic immediately and they diagnosed it and replaced the left front ride sensor. $380.88 In Jan 21,2026 the ride height message was back. They removed and replaced the air pump assembly with a new united. To remove & replace-suspension-rear suspension-ride control compressor, R&R $2,026.31. Feb 26, 2026 Took back to Lithia Dealership . Stability Control light was on and Traction control light are on check on advise. Changed out Module Transfer Case $1,013.33. Upon further research found out other Jeep owners had the same problem with the air suspension system. I feel this should be looked into.

Mar 2026 EXTERIOR LIGHTING The contact owns a 2021 Jeep Grand Cherokee. The contact stated that while the vehicle was parked with the headlight switch set to the Automatic Off/On position, the headlights blinked continuously, the bulb became red, and then the headlights became inoperable. While the headlight switch was in the on position, the headlights operated as designed. There was no warning light illuminated. The vehicle was not taken to a dealer but was not repaired. The manufacturer was not notified of the failure. The contact associated the failure with NHTSA Campaign Number: 21V950000 (Exterior Lighting); however, the VIN was not included. Feb 2026 SEAT BELTS Driver's seat belt retractor doesn't retract all of the time. Hangs up & leaves slack. Bought car & wasn't as bad. Didn't talk to dealer until now. Drove car 800 miles & 4 months since purchase. Dealer service dept says no extended warranty etc. I'm past the original warranty period & the 3 month certified used car warranty. Car was sold on Oct 14, 2025. The equipment checklist form marked that all safety systems were working. Feb 2026 SUSPENSION Over the past year, I've noticed a suspension-related squeak - increasing in frequency - coming from the rear of my 2021 Jeep Grand Cherokee Limited vehicle. I brought this to the attention of the Jeep dealer service dept where I purchased the vehicle, as well as to another independent auto repair shop, and both indicated they could not find anything wrong. Today, I saw news on the internet of the Jeep Grand Cherokee Limited "rear coil spring" issue as described in this Safety Recall report [XXX] and now believe the noise I've been hearing may be related to this recall. Feb 2026 EXTERIOR LIGHTING Car has 28k miles. It is driven by a [XX] year old. It is a Jeep GC Summit which is top of the line for Jeep. Both headlights were blown and the dash warning system never alerted that the lights were out. Car is equipped with the headlight monitoring system. She never drives at night so I cannot give you an exact date but we think it has been out since 2024 which is the last Texas inspection it had and passed. Contacted Jeep Wave with concern and was blown off and sent to a VM which was never returned. You have a recall for the Laredo and Overland models but not the Summit. The DRL lights thankfully are bright but you still need headlights. Car has been set to auto for headlights due to her age. I am hoping you give this a serious look. I posted on Facebook and other people are having the same issues with the Summits. You have no headlights and nothing saying they are out. Jan 2026 STEERING The contact owned a 2021 Jeep Grand Cherokee. The contact stated that while making a left-hand turn, the wheels on the vehicle failed to follow the trajectory of the movement and failed to function as intended, and moved toward the curb. The vehicle came to a stop when it hit the curb. It was unknown if any warning lights were illuminated. The driver-side and passenger-side air bags did not deploy. The contact exited the vehicle, and upon inspecting the vehicle, the contact noticed that the front right and rear passenger-side tires were facing inwards. The vehicle was towed to a collision center. The contact was not injured. No police report was filed. The insurance company was notified and deemed the vehicle a total loss. The failure mileage was approximately 127,000.

Any time temperature drops into negative digits the vehicle will drain both batteries to the point the auto is inoperable. Further jump starting the vehicle in a very low battery state is a futile venture. The issue leaves the vehicle and it's occupants stranded in sub-zero conditions.

Jan 2026 ENGINE I am reporting a potential safety-related defect involving my Chrysler vehicle equipped with a V6 Pentastar engine. During a routine oil change a few weeks ago, I was informed by the service provider that my engine oil level was extremely low, despite the oil change being performed earlier than the recommended interval. This was alarming, as the vehicle had not shown any warning lights or obvious leaks. The service provider advised me that Chrysler vehicles with the V6 Pentastar engine have a known issue related to a defective engine seal that can cause excessive oil consumption. I was told that Chrysler is aware of this issue, that it has been the subject of class action lawsuits, but that it has not been formally recalled. I have since submitted my information to a class action settlement related to this issue but have not received any response. Due to the risk of engine damage, sudden failure, or safety hazards while driving, I believe this issue should be formally reviewed and documented by NHTSA. I am submitting this report to help document a potentially widespread defect that may affect vehicle safety and to ensure appropriate action is taken to protect consumers.
