Sep 2023 STEERING While driving on the highway, the car suddenly alerted me that the steering assist was affected. The steering wheel became extremely stiff. To turn, it took two hands and quite a lot of strength. I pulled off onto an exit, turned the car off, then started it back. The steering worked properly the rest of the drive. The next day, steering worked fine on my 50-mile drive to work. On the way home, however, it froze up the same way again. Again, I pulled over, turned the car off, then back on and the issue was resolved. The steering wheel was so stiff that I could barely turn it, however, and it is incredibly unsafe. The soonest they said they could check the car is October 2nd. It seems to be a software issue since turning the car off and then back on resolves it. Tesla, however, is impossible to get in touch with other than sending a text through the app and then I'm lucky if I ever get a response.

Sep 2023 VEHICLE SPEED CONTROL,FORWARD COLLISION AVOIDANCE On 9/3/23 we were traveling from Moab UT to Green River UT and the cruise control was engaged at highway speeds. We were not using the "Autopilot" functionality. Without warning, the car suddenly decelerated under what felt like hard braking. There was no warning immediately before the hard braking started and the car had no warning indicators of any kind either before or after the incidents. The hard deceleration happened at least four times. One of those times a raven flew up from the road, but the other times there was nothing in the road. At least three of the incidents occurred on highway 6 traveling north. The last time it occurred was on I-70 westbound after we stopped the cars and cleaned all the cameras and ensured there was no debris on the front bumper which could cause issues with the forward-looking radar. We lost confidence in the cruise control feature and did not enable it for the rest of the trip. This is a safety issue because the car does not just reduce speed, it feels like it is braking hard or perhaps full regeneration of the battery. Either way, on these 2-lane highways, there are often situations when other drivers are driving too close to the rear bumper and a sudden, inexplicable hard braking is very likely to cause collision. A collision at these speeds could push the car into oncoming traffic, leading to fatalities. We asked Tesla service to look into the issue, and said there is likely footage of at least one of the events (because I honked the horn to start recording). The Tesla service person gave several reasons why it deceleration may occur, but Tesla is neither reviewing the footage, nor taking action to remedy the issue. They said "Our remote diagnostics has confirmed the vehicle hardware is operating as expected and there are no repairs the service team can make in regards to this." With service unable to make a fix to the car, this problem remains unresolved and we have lost confidence in cruise control.

Aug 2023 FORWARD COLLISION AVOIDANCE I have had multiple incidents of sudden speed reduction on major hi ways. (us 287 and I 25). Usually these incidents occur when I am traveling north (increasing elevation and later in the day), but that is not always the case. The events appear to be totally random, but result in a speed reduction of 70mph to 40mph rapidly enough that loose items (including pets) fly. on occasion I have had to be very quick to accelerate to avoid being run over by a tailgater. I scheduled a service appointment, but was discouraged from doing so by Tesla Service people who said “the car is still learning”. The fault has not improved. If anything it happens more frequently. At this point, I would say we travel “on edge”.

Aug 2023 FORWARD COLLISION AVOIDANCE The contact owns a 2020 Tesla Model 3. The contact stated that on three occasions while driving at various speeds, the vehicle experienced phantom braking. The contact continued driving. The vehicle was not diagnosed or repaired. The manufacturer was not notified of the failure. The failure mileage was approximately 13,000.

Aug 2023 UNKNOWN OR OTHER Tesla's Full Self Driving was enabled. The car failed to stop at a stop sign, and was actually accelerating through the stop sign. This was a huge error, and I had to slam on the brakes in order to get the car to stop. It was critical that the car stop, as cross traffic did not have a stop sign (2 way stop intersection) and the speed limit for cross traffic was 45mph. Very dangerous. Aug 2023 POWER TRAIN,ELECTRICAL SYSTEM,UNKNOWN OR OTHER On August 2, 2023 at 0400, the Tesla M3 was charging in the garage using a home charger. Smoke indicative of an electrical fire likely originated from the vehicle and eventually triggered a smoke alarm in the kitchen. The driver was unaware the source of the smoke was the Tesla, disconnected the Tesla and moved it out of the house to protect the vehicleb assuming a fire was in the house, and called firefighters who searched the home without success for a source of the fire. The driver now believes the Tesla was the source as the garage access door was cracked open to feed the charging cable from the 240v outlet in the laundry room. The nearest smoke detector from the Tesla is in the kitchen, where the driver smelled smoke from an electrical fire. At roughly 0745, the driver was about to complete a one hour commute to work in this Tesla unaware that the incident at 0400 was related to the Tesla. The driver experienced a sudden vector and velocity change in the vehicle while traveling at highway speeds. This is the driver's last memory of the accident that followed. A witness driving behind the Tesla described the vehicle traveling in clear lanes when it abruptly jerked left, then right into the side of the highway into concrete barricades. The Tesla was destroyed, airbags deployed, and the driver was rescued by first responders,firefighters, and paramedics. The driver was evacuated to the nearest hospital with serious injuries. The driver suffered broken ribs, a pneumothorax, several lacerations to head and chest, several contusions across the shoulder, legs, and body, and a mild traumatic brain injury.

Aug 2023 VEHICLE SPEED CONTROL,FORWARD COLLISION AVOIDANCE Sudden hard "phantom" braking while in adaptive cruise control. This put myself and other drivers in danger of a collision. The problems been reported to Tesla, but as of yet not fixed or inspected.The problem seems to be completely random with no advanced warning of any kind.

Aug 2023 FORWARD COLLISION AVOIDANCE The car repeatedly rapidly decelerates when passing semi-trucks on the highway. This creates a problem for vehicles driving behind my vehicle as it creates sudden braking for them and it creates a bit of panic for the driver of my vehicle due to the suddenness of the braking. This problem occurs almost every time the vehicle is driving over 60mph on a freeway and is midway in passing a semi-truck trailer truck. The dealer has replaced the camera but the problem still occurs. Collision lights occur at the time of the error but no other warnings occur.

Jul 2023 VEHICLE SPEED CONTROL,SERVICE BRAKES,FORWARD COLLISION AVOIDANCE While driving on the freeway, my 2020 Model 3 “phantom braked” multiple times while using the cruise control. No autopilot for lane keeping assistance. There were no cars in front of me or any hazards. The pavement was pretty new. I was using this feature earlier in the trip with no issues then it started all the sudden with multiple incidents in a short amount of time.

Only way the rear passengers can get out is climb to the front seats and get out through front doors. This is a serious situation in case of accident when power loss can happen.

Jul 2023 UNKNOWN OR OTHER,FORWARD COLLISION AVOIDANCE My car cameras and audio will randomly stop working. Come to find out I took a recording with a timestamp of 1151 pm where I am on the highway and driving. Have witnesses of me driving and the issue. My model 3 logs, as stated by service technician, say my vehicle is "Parked and not being operated". This means I am driving with potentially dangerous and absolutely unreliable software in my car. It shows the car isn't being operated as I am going 60~ on the highway in traffic. My car cameras will go completely black and the audio warnings will go out including the backup sounds which are a required feature. Have contacted Tesla since 2020 and no action except a blanket statement of "a future update"

Jul 2023 VEHICLE SPEED CONTROL,FORWARD COLLISION AVOIDANCE Phantom Braking occurred almost 40 times on our recent trip from Northern NV to California and return. We left on July 16 and returned on July 21. Our route was from Minden NV to Las Vegas on Highway 95 and then onto Temecula CA on Highway 15 and 215. The return trip was on Highway 15 to CA 138 to Highway 14 to Highway 395. All travel was during daylight hours. Sometimes phantom braking was so severe, the deceleration cause us to be thrown forward towards the dashboard. We wore seat belts to prevent hitting the dashboard. This also happened on a similar trip in 2022 and was reported to Tesla and were told that nothing could be done. The current software version is 2023.20.9

The car was operating on Traffic Aware Cruise Control. Travelling at approx 70mph the vehicle suddenly slowed very quickly below 50 mph before I interceded to resume control of the accelerator to resume my speed before I was rear ended. This occured as I was approaching an overpass in my middle lane.

Able to open trunk and passenger side door only. Drivers side would not open. Touchscreen black no power to Do anything car essentially dead with no warning. There should have been on screen display informing driver of the need to replace 12v battery. There was none. Car needed to be towed. If this has happened while we were in the car we would have been locked in. Tow operator jumped the battery to open hood. Touchscreen restarted but car still unresponsive. Multiple warnings on screen. Tow mode would not work so car needed to be pulled with the rear wheels locked onto tow truck.

Jul 2023 SERVICE BRAKES,FORWARD COLLISION AVOIDANCE While driving on the freeway I5 in central California in the middle of the day there were no cars in front of me and the vehicle went into sudden PHANTOM BRAKING mode. I was driving at 75 MPH and it suddenly went into hard braking. Extremely dangerous. The vehicle behind me had to swerve around us. We could have been killed. I was using the cruise control but NOT the Enhanced Autopilot. This needs to be fixed. We escaped severe injury or death by sheer luck

Jun 2023 SERVICE BRAKES, HYDRAULIC,VEHICLE SPEED CONTROL The contact owned a 2020 Tesla Model 3. The contact stated while pulling into the driveway and depressing the brake pedal, the vehicle failed to respond, accelerated unintendedly, broke through the garage door, and crashed into the vehicle parked inside the garage. The other vehicle moved away and damaged the garage. There was no warning light illuminated. The air bags did not deploy. The contact shifted into reverse (R), pulled out of the driveway and shifted into drive (D). The brake pedal was depressed but the vehicle failed to respond. The contact stated that he hit the curb to stop the vehicle, and the vehicle stopped. The contact stated that all the air bags then deployed. The vehicle failed to unlock. The contact rolled down the windows and cried for help until the neighbors arrived and opened the vehicle from the outside. The contact sustained bruises on both arms, and the contact's wife sustained bruises on the right shoulder and on the left leg. Both the contact and his wife received medical assistance. There was no police report filed. The vehicle was towed to a third party it was sold to. The other damaged vehicle was totaled. The manufacturer was notified of the failure, but no assistance was provided. The failure mileage was 9,600.

Jun 2023 VEHICLE SPEED CONTROL,SERVICE BRAKES on July 19,2022, when I was driving on driveway, at less than 5 miles, I tried to stop to drop my wife and applied brake. Instead of brake, due to computer mal function, it accelerated & hit the garage door, broke the garage door, door fell on the car parked in the garage, damaged that car,and tesla automatically reversed. while reversing brake wasn't working too so I tried to put in drive & brake still was not working so to stop the car for safety, I hit the curb in my front yard All airbags were deloyed, inuring both of us. All doors were locked & we couldn't open any of them. We could open windshield. We shouted HELP and my neighbor came & open the door from outside so we could get out otherwise we both could be dead inside the car
