Oct 2025 SEAT BELTS My daughter was injured when the third row, far right side seat belt malfunctioned, and she became entrapped. My daughter had fallen asleep in the seat and lay a little sideways resting on a sibling buckled in the center seat. When she arrived home the belt was very tight, and would not undo. She tried to get out and the seat belt auto retractor mechanism was working making it harder to move and then impossible. We reclined the seat a little to try and pull her out and this didn’t help. We also tried removing pieces of clothing and pulling her out to no avail. By this time our child was in pain as the belt was cutting into her waist. Neighbors came out of their homes due to the screaming, this was evening time so felt no option but to cut the belt and release her. This event was very frightening and distressing for our family. We do not understand how the auto retractor and buckle mechanisms malfunctioned but worry it may happen again. We have read similar accounts on the Honda forums. The staff were not helpful or concerned about this malfunction despite a mechanic at Honda acknowledging this has happened before. I would like to formally lodge my concerns over this incident. Sep 2025 FORWARD COLLISION AVOIDANCE,LANE DEPARTURE,BACK OVER PREVENTION Multipurpose camera failure, all advanced safety systems were affected (Adaptive Cruise, Lane Keep Assist, Blind Spot Warning, Rear Cross Traffic Warning, Auto-dim headlights, etc) Also, caused engine light, brake system and transmission system errors to appear but they were cleared by replacing the camera with no apparent damage to the actual systems. Replaced at a dealership for a total cost for parts and labor $2200+.

Sep 2025 FORWARD COLLISION AVOIDANCE,LANE DEPARTURE Front windshield camera failed and available for inspection if needed. All safety features tied to this camera are now disabled. Even basic cruise control is disabled. Problem has been check by a dealer with an estimated cost of $2600 to repair a simple camera module. Not inspected by police but was inspected by the manufactures dealer. I can get it inspected by the police if it helps. There are several warning lights that come up after starting the car. Even dinging when backing out of the driveway causing distractions. This is a known issue for this year Honda Odyssey van. There are lots of posts about it and how to try and fix it without the dealer charging you 10% of the cost of a 2025 model. For something designed for families this is not what I expected. This vehicles safety is impacted by the removal of these features.

Sep 2025 FORWARD COLLISION AVOIDANCE,LANE DEPARTURE Vehicle has code U3000-49 in the MCU module. All vehicle safety systems have shut down and are no longer working until vehicle is repaired (lane assist, collision mitigation, adaptive cruise control, road departure, etc.) Multiplex Camera Unit is on national back order with no ETA available. Vehicle has 5 warning lights and multiple messages that keeping popping up on the cluster.

Sep 2025 ELECTRICAL SYSTEM,STRUCTURE Read Driver side power sliding door, got stuck halfway open. I've had this happen on the passenger side as well, but after turning the car off, putting power doors to manual, and rocking it back and forth, I got it fixed no problem and ended up having to replace a bump sensor on the door and it resolved. I thought that's what was going on when it happened to my driver side door as well. However after I turned the car off, and turned the doors to manual it still wouldn't close. I turned them back to automatic, turned the car back on, and the door proceeded to start going back, it rocked violently then broke the rear window and before stopping again. I read this track roller has been a problem for MANY users of this van and in some cases the entire door has fallen off because of it.

Sep 2025 LANE DEPARTURE,FORWARD COLLISION AVOIDANCE Multi-Purpose camera unit internal electronic failure. Front-facing camera mounted in the windshield. This caused several warning lights including the brake system warning light to illuminate. This failure caused the automatic brake system and other safety features in the car to turn off. The vehicle has been inspected by a mechanic. There were no other warning signs.

Sep 2025 ELECTRICAL SYSTEM,LANE DEPARTURE The forward facing camera has failed. A myriad of systems are dependent upon functionality of camera including collision mitigation braking, lane keep assist, lane departure warning, forward collision warning, cruise control, and automatic lighting and high beams. The failure also prompts a transmission system failure, though it's unclear how/why that system would be related. All issues are prompted by warning lights on the dash and the systems are inoperable. Research of the issue suggests it's a common failure across Odyssey and CR-V models with front-facing cameras and related safety systems.

Sep 2025 FORWARD COLLISION AVOIDANCE,LANE DEPARTURE Got my windshield replaced. Was recalibrated. Within 3 months, 5 dash lights come on. 1. Adaptive Cruise Control Problem 2. Lane Keep Assist Problem 3. Collision Mitigation System Problem 4. Road Departure Mitigation System Problem 5. Auto High-Beam Problem And the Brake System light is on. I was told it wasn’t the new windshield and that recalibration wouldn’t fix the problem. The code that won’t clear is U3000-49 Crash: No Fire: No Unknown
Sep 2025 UNKNOWN OR OTHER,SERVICE BRAKES,FORWARD COLLISION AVOIDANCE Collision Mitigation Braking System (CMBS) My 2019 Honda Odyssey slammed on the brakes while traveling 70+ MPH on a clear freeway. There was no warning and nothing in the road. I have also had issues with the braking warning light flashing for no reason while traveling down the roadway. I called the Honda dealership and inquired about the braking issues and was told unfortunately it can happen, especially if I drive on windy roads.

Sep 2025 FORWARD COLLISION AVOIDANCE The Camera Assembly Monocular (part 36160-THR-A81) is shorted out causing multiple safety system failures from collision warning, lane departure, emergency braking, etc. Confirmed by local mechanic and then Honda dealer (Merrillville, IN). I still have the part, not yet replaced. Quoted $2K to replace. While driving multiple warning lamps went off and cruise control disengaged. No signs of issues prior to this occurrence. Now none of the safety features related to the ADAS system work.

Sep 2025 ELECTRICAL SYSTEM,ENGINE,FUEL/PROPULSION SYSTEM While traveling in my 2019 Honda Odyssey (40,000 mile) the vehicle completely shut off and placed itself into neutral. The warning lights for emissions system problem, brake problem, and parking assist problem lit up on the dashboard. The vehicle went into neutral and would not allow me to drive at all. Then the vehicle shut off all within 30 seconds. The vehicle would then not restart and now i have to get the vehicle towed to the Honda Service Department. After looking through other complaints there is a known recall for manufacturer defected fuel injectors for my year model odyssey but does not appear when i search for recalls. This was an insanely dangerous situation here in Oahu where the traffic is very bad. The vehicle gave no warning or check engine light and we have completed every service for the vehicle. The vehicle just randomly completely shut off and would not restart leaving my family of 5 stranded. Crash: No Fire: No Unknown
Sep 2025 FORWARD COLLISION AVOIDANCE The van's LKAS camera has malfunctioned. This has disabled not only LKAS but cruise control, forward collision warning, automatic emergency braking, blind spot warning, lane departure warning, and rear cross traffic. There is also a brake system failure light active on the digital dashboard. The failure of the LKAS camera has affected numerous Honda vehicles, not only the Odyssey, and across many model years. The part is available for inspection; #36160-THR-A81. The failure of the component has disabled key federally required safety features of the vehicle. The problem has been inspected by a Honda dealership. The repair cost is over $2,000. No warning lights nor error messages appeared before the failure of the component. It's demise presented as an intermittent error for 2 months before becoming permanent.

Sep 2025 FUEL SYSTEM, GASOLINE,POWER TRAIN The contact owns a 2019 Honda Odyssey. The contact stated that while driving at an undisclosed speed, the accelerator pedal was depressed; however, the vehicle failed to accelerate as intended. Additionally, the vehicle stalled with several unknown warning lights illuminated. The contact received notification of NHTSA Campaign Number: 23V858000 (Fuel System, Gasoline). The vehicle was taken to the dealer, and the recall repair was performed. The contact was informed that the transmission needed to be replaced. The contact was informed that the failure was possibly due to a failed fuel pump module. The contact was informed that the transmission needed to be replaced. The vehicle was not repaired. The manufacturer was made aware of the failure. The contact was informed that the repair was not covered under warranty or recall. The contact was advised to contact the NHTSA Hotline to report the failure. The failure mileage was approximately 150,000. Crash: No Fire: No Unknown
Aug 2025 ELECTRICAL SYSTEM,UNKNOWN OR OTHER On our 2019 Honda Odyssey the passenger sliding door is failing to latch closed and stay closed or locked. The door is popped out in the back because the latch mechanism will not engage and electronically lock as part of the normal closing sequence like the other door. From what I can tell, this is the same issue reported in Service Bulletin 18-128, however when I called Honda about it, they said my VIN was not included in that recall. I am concerned because it seems like the same problem and it creates an unsafe situation where the door is not fully latching, even when manually closing the door and trying to lock it. I can hear excessive noise from that side of the vehicle, and the warning message flashed saying there is an issue with the Right Side Sliding door, and the dome lights come on with an audible warning tone. I am unable to put passengers in the back of the van and needed to be very careful when driving it to the dealership for diagnosis yesterday. I'm still waiting to hear from them. I think this issue should be covered under recall work, since it seem like it is the same issue described in the service bulletin 18-128. Crash: No Fire: No Unknown
Aug 2025 FORWARD COLLISION AVOIDANCE,LANE DEPARTURE Multipurpose camera is defective. The camera is available for inspection upon request. A number of safety features do not work when this camera is not operational, including forward collision warning and lane departure warning. Honda dealership performed a diagnostic and confirmed it needs to be replaced. No other parties have inspected the camera. There were no warning messages prior to the camera failing, only after it had failed. This appears to be a common issue affecting many owners of this vehicle. Crash: No Fire: No Unknown
Aug 2025 ENGINE The contact owned a 2019 Honda Odyssey. The contact stated that while driving approximately 65 MPH, the vehicle suddenly decelerated to 45 MPH, and failed to properly accelerate while depressing the accelerator pedal. The contact stated that the failure was intermittent and was self-corrected after each failure. The vehicle was taken to the local dealer on several occasions, but no fault codes were retrieved, and the cause of the failure was not yet determined. The vehicle was not repaired. The manufacturer was notified of the failure, but no assistance was offered. The contact was made aware of NHTSA Action Number: PE25008 (ENGINE). The failure mileage was 33,000.

Aug 2025 UNKNOWN OR OTHER,FUEL/PROPULSION SYSTEM Over the last year, recorded 13 incidences where car would not accelerate. (It occurred more times, but I did not record it). I wrote down date, time, weather, gas level, temperature, and what the car would not do. Each incidence was the same - the car would not accelerate, despite pressing down (strongly) on the gas pedal. The car would then jerk forward suddenly. This provided concern and danger as it was often as I was trying to turn off a side road, onto a main road (state highway). This often needs to be done quickly as traffic approaches from both sides. There were a couple of instances when a car was approaching and the vehicle appeared to just stop in the intersection before finally picking up. I have definitely had concerns regarding being hit by oncoming traffic. I brought the vehicle in to Honda to assess in ~May/June 2023. They could not replicate the problem and ended up returning the vehicle without any noted actions. I began taking note of when this occurred starting 6/1/2024, and have 13 recorded incidences. Aug 2025 FORWARD COLLISION AVOIDANCE In August of 2024, I started up my 2019 Honda Odyssey one morning and all the vehicle safety alerts/warnings on the dashboard came on (adaptive cruise control problem, lane keeping assist problem, collision mitigation system problem, road departure mitigation system problem, auto high-beam problem, ABS). I took the vehicle into my local Honda dealership and they ran a diagnostic which determined that the forward looking camera was faulty and needs to be replaced. There were previously no warning lamps, messages or other symptoms of the problem prior to the failure of the camera. The dealer's quote to replace the camera was $1,528.99 at the time. Having since researched this issue further on Honda owner forums, this seems to be a common problem with this generation of Honda vehicles. As evidenced by the corresponding warning messages, the failure of the front camera causes an array of vehicle safety issues impacting the driver and passengers, other vehicles and pedestrians. This defect, which is an extremely cost-prohibitive repair for the consumer, should be addressed and corrected by the manufacturer as soon as possible. The vehicle is available for further inspection upon request. Please note that I was unable to select all the applicable Advanced Driver Assistance Systems (ADAS) under the Problem Parts section of this form. Crash: No Fire: No Unknown
Aug 2025 AIR BAGS On July 1st 2025, while driving in Raton, New Mexico, my 2019 Honda Odyssey EX-L experienced a spontaneous deployment of the driver-side curtain airbag. I was exiting the highway and slowing down to a stop sign (approx. 30 mph) when the airbag deployed. There was no crash, no impact, and no obstacle. This deployment disabled the entire SRS system, leaving the vehicle unsafe. My family — including three children — were in the car. I received a minor airbag burn on my arm, which healed in 24 hours. The SRS light came on only afterwards. There is zero exterior or interior damage. I recorded a video immediately after. The certified collision center at David McDavid Honda (Irving, TX) inspected the vehicle and confirmed in writing that there was no sign of collision. They sent me $11,000 repair (attached). I opened a case with American Honda on Monday, July 7th 2025 and I have not heard back since. The vehicle and components are available for inspection. I’ve since found multiple reports of similar airbag deployments in 2018–2020 Honda Odysseys, including:     •    https://www.odyclub.com/threads/2019-honda-odyssey-elite-random-air-bag-deployment.380737/     •    https://www.odyclub.com/threads/random-airbag-deployment.382243/     •    https://www.odyclub.com/threads/side-curtain-airbags-deployed-w-out-collision.366424/ This appears to be a pattern of spontaneous deployment and SRS failure that poses serious safety risk and warrants investigation.
