| Jan 2022 | SERVICE BRAKES,FORWARD COLLISION AVOIDANCE | What component or system failed or malfunctioned, and is it available for inspection upon request? On at least 4 occasions, while using the adaptive cruise control, the brakes are suddenly applied hard when no vehicles are anywhere near close enough to trigger an emergency braking situation. I have to manually step on the gas pedal to have the car stop the automatic braking. We can make the vehicle available upon request. How was your safety or the safety of others put at risk? This incident is startling when it occurs and can cause an accident if a vehicle happens to be too close behind our vehicle. We are reluctant to use the adaptive cruise control as a result of these incidents. Has the problem been reproduced or confirmed by a dealer or independent service center? The problem has not been reproduced or confirmed by the dealer or service center. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? No component has been inspected as a result of these incidents. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? There are no warning lights or messages that appear before, during or after the incident. The incidents occur suddenly and with no warning. | Crash: No Fire: No | Unknown |
| Jan 2022 | ENGINE,FUEL/PROPULSION SYSTEM | The Charge Ari Cooler Duct Disconnected from the Throttle Body. When this happens my car immediately goes into lower power mode. This is a safety concern, when going 70 MPH, it will immediately drop down to 50 MPH. This sudden decrease in speed can cause other drivers to wreck into me and my family. This problem has been confirmed by a dealer. This has not been inspected directly by the manufacturer, however it was inspected by a dealer. There were no warning signs prior to failure, only a warning after the incident occurs. The check engine light comes on. | Crash: No Fire: No | Unknown |
| Dec 2021 | UNKNOWN OR OTHER | Sunroof glass "exploded". Sounded like tire blew out. Scaring me into almost a panic situation. Dealer said they never heard of this. Have not gone to insurance yet. No warning signs. | Crash: No Fire: No | Unknown |
| Nov 2021 | UNKNOWN OR OTHER | Sometimes it will go to auto stop but instead completely shut down with no power or anything. You can do anything at all | Crash: No Fire: No | Unknown |
| Nov 2021 | SERVICE BRAKES | While safely driving under normal conditions, I attempted to slow my vehicle down while approaching a vehicle that was making a right turn into a parking lot. Upon trying to apply the brakes, I was met with resistance (Hard brake pedal) which prevented my vehicle from properly slowing down. This was caused by a failed/malfunctioning brake booster/vacuum pump. Since I was driving at a safe distance, I managed to barely avoid colliding with the rear of the car making the turn, however, that miss was extremely narrow. My vehicle barely slowed down when the brakes were applied which, had I been on the interstate or traveling at a higher rate of speed, would have more than likely resulted in a collision. This DEFINITELY puts myself, my wife, and our 2 children's safety at risk. Once I successfully parked the vehicle, a "Service Brake Assist" warning messaged appeared for the 1st time. I began researching/attempting to diagnose the issue. During this time I came across several articles detailing a recall issued by GM describing the exact same issue with the brake booster/pump, ironically however, it only covered vehicle produced through 2018. I then contacted GM customer service to alert the manufacturer and see how they would rectify the situation. I was informed that my vehicle had no open recalls and was out of warranty & there was nothing they could do for me.(handwritten call log can be provided upon request) When I contacted the dealership, they informed me they were unable to see the vehicle until the 1st week in Dec. I was left to take it to my local mechanic who, upon his diagnosis, informed me that it was the vacuum pump that had failed. As previously stated, there were no warnings prior to the failure. The "Service Brake Assist" appeared AFTER I managed to safely get my vehicle to a parking spot. | Crash: No Fire: No | Unknown |
| Oct 2021 | ELECTRICAL SYSTEM,ENGINE | The car was purchased February of 2019. The car has been stalling, car has shut completely down, and the car decrease speed sometimes while on the expressway which can be life-threatening. Not to mention that the radio freezes and the windows also gets stuck sometimes. It was reported to the car lot where the car was purchased as well as GM. the car was taken into Homewood Chevrolet where they stated that it was only a spark plug which is not true. I know that the car is having intermittent problems. They were suppose to an extensive check and I am positive that it was NOT done. There is an electrical and transit engine related issue that is not being addressed | Crash: No Fire: No | Unknown |
| Oct 2021 | SERVICE BRAKES | While exiting the highway I went to put on my brakes and there were no power brakes. It pedal was stiff and I had to push extremely hard to stop and it took a long time to do so. The brake assist light come on as well. I had no prior warning, noise or any reason to believe that my brakes wouldn’t work. We checked the pads, rotors and brake lines and they were all fine so that’s not the issue. It could have been catastrophic if I had been on the highway still when this happened. | Crash: No Fire: No | Unknown |
| Sep 2021 | SERVICE BRAKES | While driving on the freeway, the brakes became suddenly stiff and I was unable to stop the vehicle immediately for oncoming traffic. I had to steer away from the road onto the emergency lane and apply the brakes really hard to stop the car. There were no warning messages of any sorts and while trying to stop and start the car several times, then the service brake assist warning with the check engine light came up. I have to take the vehicle for inspection to the checy dealer to know more detailed cause of the issue. I had my wife, a 4 year old and a 4 month old in the car when this happened. | Crash: No Fire: No | Unknown |
| Aug 2021 | EQUIPMENT ADAPTIVE/MOBILITY | The contact owns a 2019 Chevrolet Equinox. The contact stated while driving 70 MPH on adaptive cruise control, the brakes applied independently. The contact stated no warning light was illuminated. The contact stated he continued driving to his destination. The contact took the vehicle to the local dealer however, the mechanic was unable to determine the cause for the failure. The contact stated he continued to experience the failure. The contact took the vehicle back to the local dealer however, the vehicle was not diagnosed. The contact stated that he continued to experience failure for over 2 years. The contact took the vehicle to a second local dealer, Cantin Chevrolet (623 Union Ave, Laconia, NH 03246: (603) 524-0770) and made the dealer aware of the failure. The vehicle was not diagnosed or repaired. The manufacturer had been informed of the failure. The failure mileage was approximately 10,000. | Crash: No Fire: No | 10,000 |
| Aug 2021 | POWER TRAIN | The contact owns a 2019 Chevrolet Equinox. The contact stated that while driving at various speeds, the check engine warning light illuminated, and the vehicle was hesitant to accelerate while depressing the accelerator pedal. The vehicle was taken to an unknown dealer who replaced the throttle body, but the failure continued to occur. The manufacturer was also notified of the failure, but no assistance was offered. The failure mileage was 14,000. The VIN was not available. | Crash: No Fire: No | 14,000 |
| Aug 2021 | UNKNOWN OR OTHER | We believe that there is a serious issue with the HVAC of this vehicle. My wife actually noticed it soon after buying the vehicle. I wasn't quite sure what it was. Anyway, on August 15, 2021 we drove to visit family and the strong odor of freon or something got so bad that my eyes were burning and began to cough. I took the vehicle to my local dealer and explained the problem. I left the vehicle and the service person called me to say that there was nothing wrong. They did a diagnostic of the vehicle and it came back fine. He mentioned that they is a "Service Bulletin" on 2019 Chevrolet Equinox that stated owners may complain of a strong chemical smell inside the vehicle. He stated that GM said that there is no fix for the problem. This is unacceptable since my wife has allergies and asthma. If this chemical smell is harmful, they should recall all the vehicles affected and correct the problem. It seemed to me that GM's attitude was "tough, live with it". There needs to be further investigation into this by our government and should force GM to correct this problem. I am waiting to hear more from the dealership regarding this problem today. I don't expect to hear anything different than what they told me yesterday, (August 17). | Crash: No Fire: No | Unknown |
| Aug 2021 | SERVICE BRAKES,FORWARD COLLISION AVOIDANCE | Have been stranded 4 times this summer. When car is parked it has locked in parking and will not shift out of park. Even though the parking brake is released, the car will not shift. After leaving it sitting several hours or overnight, it will start and shift again. | Crash: No Fire: No | Unknown |
| Aug 2021 | POWER TRAIN | The contact owns a 2019 Chevrolet Equinox. While stopped, the vehicle would intermittently stall upon depression of the accelerator pedal without warning. The contact stated that the vehicle would restart soon after failure however, recently the vehicle was towed to a dealer after the vehicle failed to restart after multiple attempts. The dealer was unable to duplicate the cause of the failure. The manufacturer was not notified of the failure. The vehicle was not repaired and remained in the possession of the dealer. The failure mileage was approximately 31,000. | Crash: No Fire: No Towed | 31,000 |
| Aug 2021 | POWER TRAIN | We noticed a reddish liquid on our garage floor. As the car is under warranty, we took it to the dealer who said it was a leak at the drivers side axel seal; they kept the car for one day, said it was fixed, and called me to come pick up the car. The very next day I noticed leak was worse, and so I reported that to the dealer (see uploaded photo). The dealer told me that they would connect with "tech assist" (whatever that means) and get back to me. I'm concerned that if the leak gets even greater while on the road it could be a safety problem. | Crash: No Fire: No | Unknown |
| Aug 2021 | ELECTRICAL SYSTEM,VEHICLE SPEED CONTROL,SERVICE BRAKES | i was traveling on cruise control at 60 mph when i clicked down to 55 mph at speed change. a couple minutes later the brakes slammed on hard enough to have the front wheels chirping i dont know if rear brakes came on but i went from 55 down to under 30 in a hard panic braking. this threw my wife up aganist her seat belt since she has had a stroke her resistanse to the hard slow down was low. i stepped on the gas and the brakes released i cant remember if i re engaged the cruise control or if it never disingaged. the other thing that happened was the warning about not using the info display while driving came up this only happens now and than when you start the car. so i suspect i had a momentary power lose in that circuit and what ever else is tied in to the info center im retired but i worked in mechanical engr all my life so i have some knowledge of systems | Crash: No Fire: No | Unknown |
| Jul 2021 | VEHICLE SPEED CONTROL,ENGINE,FUEL/PROPULSION SYSTEM | I was out driving on a normal errand when my car suddenly seemed to lose all ability to accelerate. When I pressed on the accelerator nothing happened and I could only advance in the car at less than 5 MPH. Fortunately I was just leaving my neighborhood and on a residential street where the speed limit was only 35 MPH anyways, but I shudder to think about what could have happened if I lost the ability to accelerate when I was on a highway or interstate, or even making a turn in front of oncoming traffic on a normal road. As any driver knows, you estimate your ability to safely turn in front of oncoming traffic based on your car's accelerator working. If I had made that judgment, attempted to turn, and not been able to accelerate and instead been left in the middle of oncoming traffic, I could have been hit and seriously injured or worse. There were no warning lights or anything that appeared prior to the incident. After losing the ability to accelerate, I received an email from Chevrolet within 10 minutes stating "An issue with the emissions system in your 2019 Chevrolet Equinox has been detected. Please service your vehicle within 7 days. Please disregard this message if your vehicle is already in for service. Inaccurate notifications can be generated during service." My car ended up being towed to the dealer for service where they confirmed and reproduced the problem. They said something about the emissions system valve(s) had to be replaced, and that this was a relatively common problem they had seen in other Chevrolet Equinoxes too, but were not aware of any recall being issued yet. | Crash: No Fire: No | Unknown |
| Jul 2021 | ELECTRICAL SYSTEM,ENGINE | same errors codes i received less than 30 days ago when my car stopped while driving. This past 7/2/2021 my car stopped while in the mist of making a right turn. | Crash: No Fire: No | Unknown |
| Jun 2021 | ELECTRICAL SYSTEM,ENGINE | Automatic engine shutoff did not restart at a busy stop light. Car shut down. Had to tow my car to auto repair. Computer code error U0284,B2575. Engine Stalls. Fixed the problem and 10 days later, I am driving and the car stops in the middle of the road again. Diagnosis is the same reason that the computer is not communicating with the car to start the engine. | Crash: No Fire: No | Unknown |
| Apr 2021 | POWER TRAIN | TL* THE CONTACTS WIFE OWNED A 2019 CHEVROLET EQUINOX. THE CONTACT STATED WHILE COMING OUT OF A STANDSTILL THE CONTACT HEARD A STRANGE NOISE COMING FROM THE ENGINE COMPARTMENT AND SHORTLY AFTERWARDS THE VEHICLE BEGAN TO PRODUCE SMOKE AND PROCEEDED TO CATCH ON FIRE. THE FIRE WAS EXTINGUISHED BY CITY POLICE AND A REPORT WAS TAKEN. THE CONTACT STATED ENGINE POWER REDUCED WARNING DISPLAYED WAS ILLUMINATED. THE VEHICLE WAS TOWED TO CONTACTS RESIDENCE WHERE IT WAS DIAGNOSED WITH BEING DESTROYED. UPON INVESTIGATION, THE CONTACT ASSOCIATED THE FAILURE WITH, NHTSA CAMPAIGN NUMBER: 20V668000 (POWER TRAIN) HOWEVER THE VIN WAS NOT INCLUDED. THE MANUFACTURER WAS INFORMED OF FAILURE AND WAS AWAITING A RESPONSE. THE FAILURE MILEAGE WAS APPROXIMATELY 16,199. | Crash: No Fire: Yes Towed | 16,199 |
| Apr 2021 | UNKNOWN OR OTHER | THE VEHICLE WAS PARKED OVERNIGHT (STATIONARY) IN A DRIVEWAY. IN THE MORNING, THERE WAS A LARGE CRACK IN THE WINDSHIELD. I WAS NOT A PARTICULARLY COLD NIGHT (FLORIDA). I DO NOT KNOW THE FAULT THAT CAUSED THE CRACK. IT COULD BE FAULTY TEMPERING, FAULTY INSTALLATION, OR SOMETHING ELSE. IT IS UNLIKELY FOR A WINDSHIELD TO SPONTANEOUSLY CRACK IN A MODERN CAR WITHOUT A FAULT. | Crash: No Fire: No | 9,000 |
| Apr 2021 | FUEL/PROPULSION SYSTEM,LANE DEPARTURE | THE VEHICLE STARTED LEAKING GAS FROM THE TANK AT THE REAR AFTER SITTING FOR 20 MINUTES. THIS PROBLEM HAS ALREADY BEEN RECALLED ON VEHICLES OF THIS CALIBER AND YEAR, JUST THE ALL WHEEL DRIVE VERSION. NOT EXACTLY SURE HOW LONG THIS HAS BEEN GOING ON BECAUSE I TYPICALLY DON'T GO BEHIND THE CAR. AS FOR THE PASSENGER SIDE, THE WARNING LIGHT ON THE MIRROR STOPPED WORKING AT ABOUT 15,000 MILES. VERY POSSIBLY SOONER. | Crash: No Fire: No | 35,000 |
| Apr 2021 | WHEELS | THE ENTIRE AXLE CAME OFF UPON HITTING A CURVE. THE TIRE DID NOT POP BUT THE ENTIRE WHEEL CAME OFF. THE INSURANCE COMPANY IS HUNTING FOR REFURBISHED PARTS TO PUT ON THE CAR. | Crash: Yes Fire: No Towed | 42,000 |
| Apr 2021 | WHEELS | WE WERE TOLD OUR TIRES ARE BAD AND NEED REPLACING. WE HAVE LEASED THIS CAR FOR 2 YEARS WITH ONLY 31,140 ON THE TIRES. THIS IS OUR 4TH EQUINOX THAT WE HAVE LEASED AND HAVE NEVER HAD TO REPLACE THE TIRES BEFORE WE RETURNED OUR CARS ON OUR 3 YEAR LEASES. WE ARE VERY DISAPPOINTED. | Crash: No Fire: No | Unknown |
| Mar 2021 | POWER TRAIN,ENGINE,FUEL/PROPULSION SYSTEM | I WAS SITTING AT A STOP LIGHT. WHEN THE LIGHT TURNED GREEN I TOOK MY FOOT OFF THE BRAKE AND APPLIED THE GAS PEDAL. THE CAR DID NOT RESPOND TO THE GAS PEDAL AT FIRST AND JUST SAT THERE. AFTER A FEW SECONDS I WAS ABLE TO START MOVING. THIS SITUATION ALMOST CAUSED ME TO BE REAR ENDED. THIS IS THE SECOND TIME THIS HAS HAPPENED AND HAD THE VEHICLE IN FOR SERVICE A FEW WEEKS AGO FOR SAME ISSUE. | Crash: No Fire: No | 32,800 |
| Mar 2021 | STEERING,STRUCTURE,ENGINE | I WAS IN THE DRIVEWAY AT MY HOME GETTING READY TO LEAVE FOR CHURCH, I WAS PULLING FORWARD BEFORE I BACKED UP THINKING I MIGHT BE TOO CLOSE TO THE GATES. WHEN I MOVED FORWARD WITH MY FOOT ON THE BRAKE TO JUST CREEP ALONG, THE ENGINE REVVED UP TO AN ASTROMICAL SPEED. I WAS HEAD TOWARD MY SON'S TRUCK. I STEERED AWAY BUT CAUGHT THE BUMPER OF AN F350 . MY CAR IS TOTALLY DESTROYED. I KNOCKED THE TRUCK OVER TWO FEET FROM WHERE IT WAS SITTING AND PUSHED IT WITHIN ONE OR TWO INCHES OF GOING THROUGH MY HOUSE WHERE MY SON WAS ASLEEP IN THE RECLINER. BEFORE I HIT THE BRICK POST HOLDING UP ONE END OF MY CAR PORT, I WAS ABLE TO MOVE THE CAR INTO PARK AND STOP IT. THE HILL ASSIST LIGHT WAS ON WHEN I GOT OUT OF THE CAR. I WAS GOING DOWN A SLIGHT INCLINE AND NOT UP A HILL. MY DRIVE IS ONLY ABOUT 20-25 FEET AND THE CAR WAS GOING AT AN ALARMING RATE. FORTUNATELY, I WAS JUST BRUISED AND SORE FROM THE IMPACT. | Crash: Yes Fire: No | 32,000 |
