| Dec 2025 | ENGINE AND ENGINE COOLING | The contact owns a 2018 Honda Odyssey. The contact stated while driving 60-62 MPH, there was an abnormal sound coming from the front of the vehicle, and the vehicle lost drive power. The check engine warning light was illuminated. The contact pulled over to the side of the road. The vehicle failed to restart and was towed to Tire Choice Auto Service Centers, where it was diagnosed and determined that one of the engine banks had failed. The mechanic determined that the engine needed to be replaced with a used or brand-new engine. The vehicle was not repaired. The contact related the failure to NHTSA Campaign Number: 23V751000 (Engine and Engine Cooling); however, the VIN was not included. The dealer was notified of the failure and informed the contact that he would receive a call back later. The manufacturer was notified of the failure and informed the contact that a case would be filed, and the contact would receive a call back later. The failure mileage was approximately 132,000. | Crash: No Fire: No Towed | 132,000 |
| Dec 2025 | FUEL/PROPULSION SYSTEM | My 2018 Odyssey has a problem of High Pressure Fuel Pump malfunction. It is come to know that and it can cause stalling the vehicle while running and can cause accident. The same fuel system is shared by 2018 Honda Pilot which had a recall. But Manufacturer did not issue a recall notice for Odyssey's yet. | Crash: No Fire: No | Unknown |
| Dec 2025 | POWER TRAIN,ELECTRICAL SYSTEM,ENGINE | My vehicle repeatedly and unexpectedly shifts into neutral while in Drive, causing a sudden loss of power in traffic. This has happened multiple times without any input from me and has nearly caused a crash. | Crash: No Fire: No | Unknown |
| Nov 2025 | STRUCTURE | A problem with the frame of the van in the rear/above the liftgate not being properly welded created leaking/malfunction of the liftgate and water remediation needed under the headliner. A significant safety issue was created-causing unprovoked opening and closing of the liftgate during operation of the vehicle both while parked and moving. Multiple times were individuals harmed during this process, as well as health issues from riding in the van during long period in an environment with mildew and mold development. | Crash: No Fire: No | Unknown |
| Nov 2025 | ENGINE | At highway speed on cruise control the engine went out. Van only has 118,000 miles on it. | Crash: No Fire: No | Unknown |
| Nov 2025 | LANE DEPARTURE,FORWARD COLLISION AVOIDANCE | Automatic Emergency Braking, Forward Collision Warning, and Lane Departure Warning systems stopped working simultaneously without warning. Upon further investigation, failure of the forward-facing camera is the likely root cause. Based on conversations in internet discussion forums [XXX] , [XXX] ), this is a common failure. There was a recall for the Odyssey to fix a wiring harness issue that could affect the back-up camera. I believe loss of the Automatic Emergency Braking, Forward Collision Warning, and Lane Departure Warning systems is much more serious, with potentially catastrophic consequences. Since evidence suggests failure of the forward-facing camera happens frequently, I believe this issue should be treated with the same gravity as the back-up camera and a recall should be issued. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6) | Crash: No Fire: No | Unknown |
| Nov 2025 | FUEL/PROPULSION SYSTEM | While on a road trip our vehicle began to run louder, same shifting, same gas mileage but louder especially under acceleration. There was no warning and luckily we made it home only to find out it was the High Pressure Fuel Pump in the engine bay that was failing. We had the previous Fuel Pump Recall completed but now at ~82,000 miles the HPFP goes out, these usually last much longer. Took to an independent shop who diagnosed and fixed problem resolved but said we could have easily been stranded or worse the hose with fuel that was violently pulsating in the engine bay could have come unattached and fuel could leak on the hot engine. | Crash: No Fire: No | Unknown |
| Nov 2025 | FUEL/PROPULSION SYSTEM | Was have recall my car fuel pump , I put the car in honda dealership 9am Them I back 3 pm watched my dash cam car park in parking lot no one worked on it them just did paper work When I told the manager the story he told me we did it 4 day ago on 11/07/2025 when you have diagnostic If you did it before why you give me another an appointment and broken my day Then I back home take the seat out See the fuel pump it’s do dirty look no one work on it then the part number in the paper not the one in the pump | Crash: No Fire: No | Unknown |
| Nov 2025 | FORWARD COLLISION AVOIDANCE,LANE DEPARTURE | It seems the front dash camera failed to operate which caused all safety systems that rely on it to fail | Crash: No Fire: No | Unknown |
| Nov 2025 | POWER TRAIN,UNKNOWN OR OTHER,ENGINE | Summary: Engine seized due to internal bearing failure with metal shavings found in oil filter. Full Description: My 2018 Honda Odyssey experienced a sudden engine failure around 116,500 miles. The dealer confirmed the engine is seized and showed significant metal shavings in the oil filter and pan. I have performed all maintenance according to Honda’s recommended schedule using the correct oil and filters. Honda corporate denied assistance based solely on mileage, even though this issue matches symptoms described in current NHTSA investigation PE25-001 (connecting rod bearing failure in Honda 3.5L V6 engines). There were no prior warnings other than a brief check-engine light and rough running before the engine locked up. The vehicle was in motion at the time and could have caused a hazardous stall, serious injury, or death. Given that Honda has issued recalls for similar rod bearing defects in other model years (2015–2020) and this failure mode presents a clear safety risk (loss of power, engine seizure), I request NHTSA include my case in its investigation and compel Honda to extend recall coverage to 2018 Odyssey owners. I can provide dealer diagnostic reports, photos of the oil filter debris, and all maintenance records if needed. | Crash: No Fire: No | Unknown |
| Nov 2025 | FORWARD COLLISION AVOIDANCE | Error code U3000-49 Multipurpose Camera Unit Internal Electronic Failure. Adaptive Cruise Control does not work due to the camera failure. | Crash: No Fire: No | Unknown |
| Nov 2025 | ELECTRICAL SYSTEM | The infotainment center has completely failed. It began with sporadic shutting off and then restarting of the infotainment system as if a wire or connector were loose, along with popping sounds until the unit no longer turns on and appears to have completely failed. This effects ability of back up camera to function because the screen no longer works. There appears to be a recall of other Honda vehicles needing new wire connectors for a similar problem but it does not seem to include this vehicle. | Crash: No Fire: No | Unknown |
| Nov 2025 | BACK OVER PREVENTION,FORWARD COLLISION AVOIDANCE | Suddenly one day in the morning taking my car from my garage, all alarms in my board turned on and stopped working: Automatic emergency braking, blind spot warning, forward and collision warning, lane departure warning, lane keeping assistance and rear cross traffic warning. I bring my car to the McLarty Honda service in Little Rock, AR. After calibration showed that I need a new multipurpose camera code U 3000-49. This is major concern for me. My van is in perfect shape, only 7 years old not so many miles. Doing some research sounds like this is an issue with Honda vehicles. My neighborhood is full of kids ( I have 3 kids too)with these large vehicles you can find a lot of blinds spots and these features help to keep everybody safe. I believe this a safety issue that need to be recalled in Honda's cars. | Crash: No Fire: No | Unknown |
| Nov 2025 | FORWARD COLLISION AVOIDANCE | Camera that controls ACC LKAS, Lane Departure, collision mitigation system, auto high bean, and braking system alerts kick out intermittently and fail to function while driving. | Crash: No Fire: No | Unknown |
| Nov 2025 | FORWARD COLLISION AVOIDANCE,LANE DEPARTURE | I have 6 error lights on my dash board. Road departure mitigation system problem, auto high-beam problem, collision mitigation system problem, lane keeping assist problem, Ada pat I’ve cruise control problem. My car was rear ended from an insured from Utica National insurance group. After my car was rear ended these error message came up. They fixed the car but refused to fix to have the lights removed because they said that these error lights do not come on after a rear end accident. | Crash: Yes Fire: No | Unknown |
| Oct 2025 | FORWARD COLLISION AVOIDANCE,LANE DEPARTURE | All the systems are not working This is a very low mileage vehicle and has not been involved in any accidents. They are many owners reporting the same issue. This is a safety issue as it shuts off many of the safety features of this vehicle while driving. Honda claims it’s normal. | Crash: No Fire: No | Unknown |
| Oct 2025 | FORWARD COLLISION AVOIDANCE,LANE DEPARTURE | One light on dashboard lit up, started as "Blind Spot info system problem", light would go on and off for couple of years. | Oct 2025 | ELECTRICAL SYSTEM,VEHICLE SPEED CONTROL,FORWARD COLLISION AVOIDANCE | Windshield monocular camera overheated. No longer working. | Crash: No Fire: No | Unknown |
| Oct 2025 | FORWARD COLLISION AVOIDANCE,LANE DEPARTURE | The windshield monocular camera is completely non functional causing several dashboard errors to appear. It very disheartening as this is such a known issue within the Honda Odyssey community and very expensive to replace and get calibrated after replacement. This should be recalled since so many are having this issue. Clearly this is a fault of Honda. It seems that the camera can get too hot being in the windshield and just doesn't work. A big reason why I wanted this vehicle was because of these sensors. Now I can't use them. | Crash: No Fire: No | Unknown |
| Oct 2025 | FORWARD COLLISION AVOIDANCE,LANE DEPARTURE | Several of my warning lights came on related to safety features such as brake assist, lane assist, collision mitigation. brake warning. | Oct 2025 | FORWARD COLLISION AVOIDANCE,LANE DEPARTURE | I was unable to utilize my cruise control, forward collision warning, lane departure or any of the features that is related to the Honda sensing program. I was told by the Honda dealership that it was the camera refocusing and continue to refocus which leads for it to overheat. | Crash: No Fire: No | Unknown |
| Oct 2025 | SERVICE BRAKES,FORWARD COLLISION AVOIDANCE | The windshield monocular camera failed, disabling all safety precautions. | Crash: No Fire: No | Unknown |
