May 2026 UNKNOWN OR OTHER Around 11 am on Sat., May 2, 2026, I was sitting in a very long line in Starbuck's drive thru when my 2025 Lexus RX hybrid suddenly and uncontrollably accelerated--like an uncontrollable bucking horse. Brakes didn't work. I couldn't stop my out of control car, and the multiple computer sensors also didn't stop the car. It was like the throttle was engaged because of a computer glitch. In an effort to avoid hitting the car in front of me, I immediately tried to swerve, but still hit the car, then jumped the exceptionally high curb and ended up in the grass below and almost into the busy road ahead of me. It was terrifying. The car in front of me hit the car in front of her. The exact same issue happened to me on June 18, 2025 when my car unintentionally accelerated--also in a very tight space and going at 2 mph. There was no accident at this time because my car's computer stopped me from plowing into the building immediately in front of me after unintentionally accelerating. The next day at the recommendation of the person who sold me the car, I took my new Lexus to the service department. They gave me a loaner and told me to contact Toyota's/Lexus' Brand Engagement department in order to start the review, which I did. The review was conducted and ended around July 9, 2025 . On Mon., May 4, 2026 I took my car back to the Lexus dealer where I purchased, but was not given a loaner, which I requested, and was told I would need to wait several hours to have this inspected. I then took my car to Boyce Body Werks. A State Farm agent examined my car and initially approved almost $24K worth of damages. Boyce Body Werks is saying when they disassembled my car that there was more damage underneath my car. I also notified Toyota/Lexus about the same issue that happened one year prior. They also refused to give me a loaner, and I am now forced to pay my rental fees. Boyce Body Werks--a Lexus certified dealer has multiple pictures of the damage. My car is still under warranty.

May 2026 AIR BAGS,SEAT BELTS The car does not alert when a seat belt is not worn in the back seats. This feature is mentioned in the owner's manual, but the car fails to alert when a rear passenger does not buckle their seat belt. I have raised the complaint twice and proved to lexus service center, that car doesn't warn or alert the safety belt. It was also proven and checked by Lexus with their technician. The problem is accuring right from the time car was purchased, When check with the dealer it was designed , but their manual confirms the seat belt not worn warning light is included in the package. It put the any passener riding mainly children at serious risk

Apr 2026 SERVICE BRAKES,ENGINE,FORWARD COLLISION AVOIDANCE On April 1, 2026, while pulling into a parking space at our church, I was applying the brake when the vehicle suddenly and forcefully surged forward without warning. The unintended acceleration was intense and unexpected, occurring despite continuous braking input. This resulted in the vehicle striking the building and causing significant damage to both the vehicle and the structure. This incident has occurred twice within a 10 day period, both times during low-speed parking/stopping maneuvers. There were no warning lights, alerts, no automatic emergency braking engagement, no air bag deployment, or prior indications of malfunction. I did report this to the police and they filed an Incident Report. Lexus was notified and an independent analysis engineer examined the data from the vehicle's CDR System. This report was sent to me and to Lexus.

Apr 2026 UNKNOWN OR OTHER,VISIBILITY/WIPER I bought a CPO lexus 2025 RX 350 in Jan 9, 2026. This car was lightly used in less than 3 months and right now it's around 7685 mileage. However, just with 3 months I purchased this car, the A/C stops cooling and the dealership said this is due to the condenser was damaged by small road stones where the small road stones can easily pass through the grille. - We have noticed that there are multiple reports of Lexus/Toyota SUVs with condensers easily punctured by normal road debris. - Dealers acknowledge it as a common pattern. - Loss of A/C can affect defogging/visibility and is risky in extremely hot weather, e.g. causing hard breathing, etc for driver and passengers in the car. This issue needs to be fixed by Lexus/Toyota.

Mar 2026 UNKNOWN OR OTHER 2025 Lexus RX350H purchased new on 09/04/2025. The sunroof portion of the panoramic sunroof exploded, sounded like shotgun blast. Then we heard some wind noise but realized none of the windows were open/affected. I started to open the inside electronic sunscreen but noticed glass shards so I immediately closed it back up. We were greatly startled and I could have swerved/loss control of the car, thankfully I did not. Also, if the interior electronic sunscreen would have been open at the time of explosion we could have been cut from falling glass shards and would have probably lost control of the car. We were crossing the Old Tampa Bay on the Howard Frankland Bridge (I-275) heading to Tampa, FL Airport. There are no overpasses on the bridge so no falling debris was possible and we did not see any air-born road debris that impacted the roof of the car. There were no other dents etc. in the roof or car body indicating an impact. When I took the car to Lexus to get the sunroof replaced the service representative stated it was the third replacement recently. When I picked the car up a couple days later I spoke with the Service Manager about my extended warranty coverage and the lack of evidence of an impact from road debris. He stated warranty would not cover the replacement and Lexus would not cover the replacement even on a 4 month old vehicle.

Mar 2026 BACK OVER PREVENTION On November 11, 2025, while reversing inside a garage, the rearview camera image— which had initially displayed correctly on the vehicle’s screen—suddenly froze while the vehicle remained in reverse mode. At the same time, the Pre-Collision System (PCS) failed to engage the brakes while the vehicle was traveling at approximately 2–3 miles per hour. As a result, the vehicle struck the garage structure and sustained damage. Toyota investigated this incident (Case #XXX) but based its conclusion on the incorrect assumption that I had reported either a brake system malfunction or a complete absence of an image on the display (a dark or blank screen). Because of this, Toyota determined that it could not provide assistance regarding the resulting vehicle damage. This characterization is inaccurate. The issue was not a blank screen. The rearview camera image appeared normally and then froze while the vehicle was still in reverse. My vehicle is currently subject to Recall Campaign No. 25LA06, which addresses reliability issues involving critical safety-related electronic systems. The rear visibility system is a federally mandated safety feature under 49 CFR §571.111 (Rear Visibility Requirements). When the image freezes during active vehicle operation, the system no longer provides the required real-time visibility necessary for safe reversing. Given the existence of an active recall campaign aimed at addressing reliability issues within the vehicle's electronic safety systems, this malfunction should be considered potentially linked to a defect covered by Recall Campaign No. 25LA06. The failure of the rear-view system during vehicle operation directly contributed to the occurrence of the collision.

Dec 2025 ELECTRICAL SYSTEM Car left us stranded 12 V battery completely dead had a lot of difficulty trying to get it started. The unfortunate part is I got rid of a 2023 Lexus RX 350 H for the exact same problem could not get resolved after installing five batteries. Now we’re going down this road again.

Nov 2025 ELECTRICAL SYSTEM,EXTERIOR LIGHTING,ENGINE I bought this vehicle last Thursday, The new Lexus we only drove it three times, but all power down. The first time we were about to go to work, the whole vehicle no power, so I checked the battery, it was 10 Volts, so I used my CRV battery turned on Rx350, it's working, Second time, when I arrived my work place, I turned off the new vehicle, The Whole power down again, After I checked the battery, it showed 12 Volts, after I checked the battery, the power came back. The third time, After working, I got totally locked outside the RX350, I tried so so many times, The Whole power was gone again, so I broke the handle and used the key get into the car, and checked the battery, still 12 volts, after my checking battery, the power back again! So I am sure it's not only the battery problem, but also Power management module something wrong with it, or a module named 89690-0E011 Is damaged , I am very sure the new RX350h needs very very professional technician repair, This vehicle has serious safety hazards and poses a threat to life and safety.

Oct 2025 ELECTRICAL SYSTEM While driving, the instrument panel went blank. I believe it is not safe to drive with no instrumentation or knowledge of your vehicle or speed. The dealer confirmed it needed a software update and performed this yesterday. We were not advised of this needed update, no recall or no communication from the manufacturer. We should be advised when something this critical is needed.

Oct 2025 FORWARD COLLISION AVOIDANCE I was driving the vehicle up a road I drive every day, when suddenly the instrument cluster showed a picture of a red pedestrian icon, and slammed on the breaks. It threw me forward, but quickly proceeded on like normal. It detected a pedestrian that was not in front of the vehicle. This was only 20 days after I got the car. It proceeded to do this three more times, with similar severity, but at different speeds and different locations. The fourth instance, I was driving home and the vehicle had the same pedestrian detection of an imaginary person, this time breaking me from 40 mph to 12 mph with a car behind me. Thank the lord I was not hit. I made a service appointment asap, and it was there for 8 days. I had it happen to me 5 times in 3300 miles and 2.5 months, and they drove it 12 miles and said they couldn’t duplicate it. Cooperate told me to send it up to another store which I did. It has now been there for 16 days, and they can’t find an issue either. I am not comfortable driving the car because it could have a breaking incident at any time, and is putting me in danger. Cooperate has told me to file a lemon case or quote “wait for it to happen again and try and document it in better detail.” I have given them locations, time and date estimates and weather estimates for every instance, and I’m still getting put back in a car that someone at Hendrick Lexus North considered a “liability.” I am beyond disappointed in Lexus for going against their word, is making me the guinea pig in a effort to try to replicate the issue. This is my third Lexus, and my image of the brand is ruined.

Sep 2025 AIR BAGS,VEHICLE SPEED CONTROL,SERVICE BRAKES The contact owns a 2025 Lexus RX 350. The contact stated that while driving into a parking space, the vehicle suddenly experienced unintended acceleration and drove forward at a high rate of speed before crashing into the brick wall of an office building. During the crash, no air bags deployed, and the brake pedal was hard when pressed and did not stop the vehicle. Also, the manufacturer's APP indicated a malfunction in the engine and engine throttle during the incident. The driver sustained injuries to the knees, arms, and hips, which required medical treatment. An incident report was filed by the facility Patrol Officer, and the vehicle was towed to the collision center. The cause of the failures was not determined. The manufacturer and local dealer were notified of the incident. An engineer from the manufacturer was sent to examine the vehicle and reported no trouble found. The contact indicated that upon delivery of the vehicle, several electronic features did not function properly, including the display and navigation screen. The manufacturer and local dealer were notified of the failure. The failure mileage was 106.

Jun 2025 UNKNOWN OR OTHER I bought my car on May 30, 2025, and on June 3, 2025 while driving, the driver’s side rear window behind the passenger’s seat shattered. It sounded like an explosion. No cars were around at the time of the incident. Luckily, the window stayed intact.

May 2025 SERVICE BRAKES, HYDRAULIC,ENGINE The contact owns a 2025 Lexus RX350H. The contact stated that while coming to a stop, the brake pedal and accelerator pedal were both depressed simultaneously. The contact stated that there was a loud sound coming from the engine compartment. There was no warning light illuminated. The contact forcefully depressed the brake pedal to stop the vehicle. The vehicle was towed to the dealer but was not diagnosed or repaired. The manufacturer was made aware of the failure. The approximate failure mileage was 119.

Mar 2025 LATCHES/LOCKS/LINKAGES 1) Electrical malfunction with warning of locking malfunction transiently appearing 2) Locked inside the car and the manual door release would not work after the above. 3) Finally got out after using key fob and multiple attempts to use the door handle, closed the door, and the car locked and could not open it with either the key fob, Lexus application, nor with the emergency physical key (it would turn but not unlock the door). 4) After 1st malfunction, had a smell of burned electrical component but NO smoke or fire. The hybrid system was fully charged however, it is probable that after speaking to Lexus while inside the car with the ignition on to trouble shoot a charging scheduling issue, the 12 volt battery was partially discharged. The car has 441 miles and after Lexus service arrived, they were able to get it drivable HOWEVER, check engine light remained on and "system malfunction...visit your dealer" was also on the entire 7 mile trip to the dealer with the tow truck following me all the way. I kept the window open despite the rain so that I could get out if something else happened.
