| May 2026 | UNKNOWN OR OTHER | Turn Signal starts flashing left after turning right. Happens fairly consistently. | Crash: No Fire: No | Unknown |
| May 2026 | POWER TRAIN,VEHICLE SPEED CONTROL,ENGINE | While driving my 2025 Chevrolet Equinox, the vehicle experienced a sudden power and drivability issue. The vehicle began to decelerate on its own while in motion, even though I was actively driving. A warning notification appeared on the driver dashboard, and shortly after, the vehicle power shut off. This creates a serious safety concern because the vehicle can unexpectedly lose speed and/or power while driving in traffic. The issue appears to involve the transmission, powertrain, or vehicle electrical/power management system. The loss of acceleration and sudden deceleration could increase the risk of a crash, especially at highway speeds or in heavy traffic. This incident occurred while the vehicle was being driven, not while parked. The warning message appeared on the dash before or during the power loss. I am reporting this because the vehicle’s sudden deceleration and power shutdown while driving could be a safety defect. I am requesting that NHTSA review whether this issue is related to a known defect, recall, transmission issue, powertrain control issue, or electrical system failure affecting the 2025 Chevrolet Equinox. | Crash: No Fire: No | Unknown |
| May 2026 | VISIBILITY/WIPER | The front windshield reflects the dash below making it difficult to see out the windshield. Visibility is extremely important while driving. This is obviously on continual problem during daylight hours. | Crash: No Fire: No | Unknown |
| May 2026 | POWER TRAIN | The contact owns a 2025 Chevrolet Equinox. The contact stated while driving at an undisclosed speed, the transmission independently shifted into neutral. No warning light illuminated. The vehicle lost motive power. The contact stated that the vehicle stopped in the middle of a busy road. The contact stated that while attempting to start the vehicle, the vehicle started. In addition, the contact stated that the failure was persistent. In addition, the contact stated that later the check engine warning light illuminated. The vehicle was taken to the dealer, however the vehicle had not been inspected. The manufacturer was not made aware of the failure. The failure mileage was approximately 24,000. | Crash: No Fire: No | Unknown |
| May 2026 | SERVICE BRAKES | While in a parking lot going about 10mph or less, a car pulled out from a parking space on the left. I did a full brake and despite being dry, warm pavement, the car activated abs and released some brake pressure and allowed the car to continue to move. This ment that my braking distance increased by about 5-10 feet more than you'd expect. I have had this happen 3 times, each time a car was pulling out perpendicular on the left. I tried to replicate this in an open parking lot and it never repeated therefore I think it has something to do with the car approaching on the left and the active braking system. | Crash: No Fire: No | Unknown |
| May 2026 | STEERING,SUSPENSION,POWER TRAIN | This vehicle makes a noise that sounds like your driving over rumble strips. The front end shakes and there is a hard vibration. I purchased this vehicle used on 5/5/26 and it’s been in the dealership twice already. The Onstar module is defective and now this noise and vibration. The shaking starts upon accelerating. The dealership, Central GMC/Buick of Jonesboro, AR. told me that GM knew about the problems and that their Engineers did know the fix yet. They say it’s not a safety issue but This is an unsafe vehicle and GM knows it’s unsafe. I’m afraid to drive it. | Crash: No Fire: No | Unknown |
| May 2026 | EXTERIOR LIGHTING,ENGINE | The back lighting for the controls does not work the check engine light is on the dealer said there is no fix for either one of these items at this time | Crash: No Fire: No | Unknown |
| May 2026 | ELECTRICAL SYSTEM | My vehicle OnStar telematic module has shorted out the complete electrical system. This module is backordered. No time frame was told it could be months. Many GM vehicle have the same problem. The vehicle can stop while sitting or driving. This vehicle is unsafe to drive i traded it to the dealership. They will hold on to it until(if & when) they get the part. | Crash: No Fire: No | Unknown |
| May 2026 | ELECTRICAL SYSTEM | Onstar module faulty causing parasitic battery drain unable to get a loaner car | Crash: No Fire: No | Unknown |
| May 2026 | EQUIPMENT | The contact owns a 2025 Chevrolet Equinox. The contact stated that the vehicle's GPS monitoring system had failed to operate as needed. The vehicle was taken to the dealer on multiple occasions where the contact was informed that a software update was performed on the vehicle; however, each software update failed to correct the failure. The dealer was notified of the continued failure and informed the contact that the part to repair the vehicle was on backorder with no certain timeframe as to when the part may become available. The contact stated that he and his wife are elderly and rely on the GPS monitoring system for both traveling and unexpected medical emergencies. The manufacturer was also notified of the failure where they gave the contact the same information as the dealer and was given a case number. The vehicle was not repaired. The failure mileage was approximately 22,000. | Crash: No Fire: No | 22,000 |
| May 2026 | ELECTRICAL SYSTEM | telematic defective waiting 7 weeks still no eta... safety issue if battery loses power | Crash: No Fire: No | Unknown |
| May 2026 | POWER TRAIN,ENGINE | The contact owns a 2025 Chevrolet Equinox. The contact stated that while driving at 70 MPH, the vehicle unexpectedly decelerated to 10 MPH with no warning lights illuminated. The contact was able to restart the vehicle following the failure. The vehicle was taken to the dealer. The vehicle was not diagnosed or repaired. The manufacturer was made aware of the failure. The approximate failure mileage was 5,700. | Crash: No Fire: No | 5,700 |
| May 2026 | SUSPENSION,POWER TRAIN,ELECTRICAL SYSTEM | While operating the vehicle on both interstate highways and local roadways, an intermittent malfunction occurs involving the All-Wheel Drive (AWD) system. The instrument panel displays a warning message stating, “Service All-Wheel Drive System.” When this warning appears, the vehicle automatically disengages from AWD and reverts to Two-Wheel Drive (2WD). Once this occurs, the system does not allow the driver to manually return the vehicle to AWD. This issue has occurred on more than three separate occasions. Notably, there have been instances where the warning indicator illuminated immediately upon starting the vehicle, prior to any extended operation. When the malfunction is present, the vehicle exhibits abnormal driving behavior, including a noticeable jerking or hesitation that feels consistent with a transmission-related issue. The failure is unpredictable in nature, with no clear pattern or triggering condition identified. This creates a significant safety concern, particularly when operating the vehicle at highway speeds or in conditions where AWD capability may be necessary for proper traction and control. An appointment has been scheduled with an authorized dealership for diagnostic evaluation; however, at the time of this report, the vehicle has not yet been inspected or repaired, and the issue remains unresolved. | Crash: No Fire: No | Unknown |
| Apr 2026 | POWER TRAIN,SERVICE BRAKES,FORWARD COLLISION AVOIDANCE | While driving on the freeway, my vehicle displayed a “Service All-Wheel Drive System” warning and unexpectedly switched into two-wheel drive. This created a serious safety concern due to sudden loss of traction and control. Additionally, on multiple occasions while reversing, the vehicle abruptly applied the brakes without any obstruction present, displaying a message that automatic braking had engaged. My foot was on the accelerator at the time, and this created a sudden and unsafe situation. The vehicle has been taken to the dealership for repair, but due to the unpredictable nature of these issues, I no longer feel safe operating the vehicle. | Crash: No Fire: No | Unknown |
| Apr 2026 | LANE DEPARTURE | Occasionally, the vehicle will jerk the steering wheel when going around a curve (almost like its trying to center the line while driving in a curve) and sometimes it steers out of the lane or simply won’t function correctly at all. | Crash: No Fire: No | Unknown |
| Apr 2026 | ENGINE | This is a 2025 vehicle with 25,000 miles and the Check Engine Soon light stays illuminated due to a faulty PCV Valve or possibly a poorly designed crankcase. The engine code is P04FB and is commonly found as an issue when you do a search. As a safety precaution, the vehicle is programmed to not allow the remote start to work as designed when the engine light is on so this issue affects the overall functionality of the car. Having spoken with the dealership, they report seeing that engine code often and that the part is on backorder. The dealership seems to think it's the crankcase causing the issue and not the PCV Valve as the fix they have been ordering for them is the case. It will potentially go away as the weather is hotter, but if a chilly night or morning comes up, then the light will come back on because there is moisture buildup on the PCV Valve when the temperature drops that causes the light to come on. With all of the complaints I have read online for this very issue, I would like to see NHTSA step up and make Chevy fix their problem. Thank you. | Crash: No Fire: No | Unknown |
| Apr 2026 | UNKNOWN OR OTHER | Pulling into our parking lot of apartments. After picking car up from dealership for a different problem. The “service high voltage” light came on. Turned car off restarted. Same issue. | Crash: No Fire: No | Unknown |
| Apr 2026 | ELECTRICAL SYSTEM,UNKNOWN OR OTHER | While driving, a burning odor developed followed by visible smoke entering the passenger cabin from the dashboard/HUD area. We pulled the vehicle over and had it towed due to safety concerns. Dealer required dashboard disassembly to investigate potential electrical/fire condition but was unable to replicate the issue. | Crash: No Fire: No | Unknown |
| Apr 2026 | ELECTRICAL SYSTEM | For the second time the service high voltage system warning has came on first time it was updated the dealership said now it’s back on again A/C not working properly | Crash: No Fire: No | Unknown |
| Apr 2026 | UNKNOWN OR OTHER,BACK OVER PREVENTION,FORWARD COLLISION AVOIDANCE | Sometimes when I am backing up, the breaks will slam on. Back in 2025 first I noticed, but thought it was human error, just wasn’t sure. After it happened more than a couple of times in 2026, I realized it wasn’t me and must the safety warning system? I googled after it happened this month and (phantom breaking) popped up for the Chevy equinox. I Have an appt. With Chevrolet to check it out in 2 weeks. Scary when it happens. I’ve only noticed it so far when backing up. If it were to happen other than in a parking lot, that could be dangerous, especially if I am going faster. | Crash: No Fire: No | Unknown |
| Apr 2026 | ELECTRICAL SYSTEM | wiring harness on PCV Valve causes check engine light to come on as soon as it rains or gets wet. Dealer has tried multiple times to fix issue to no avail. The parts are on national back order and they can not get them. Dealer has told me that this is a know issue with this model year equinox. | Crash: No Fire: No | Unknown |
| Apr 2026 | ELECTRICAL SYSTEM | The contact owns a 2025 Chevrolet Equinox. The contact stated that the high-voltage system message was displayed. The vehicle was taken to the local dealer, where an unknown diagnostic test and repair were performed on the computer, but the failure recurred. The vehicle was taken to the local dealer again, where the AC receiver was ordered; however, the part ordered was the incorrect part. The dealer informed the contact that the part was on back order. The vehicle was not repaired. The manufacturer was contacted and opened a case. The failure mileage was approximately 6,065. | Crash: No Fire: No | 6,065 |
| Apr 2026 | ELECTRICAL SYSTEM,UNKNOWN OR OTHER,SERVICE BRAKES | The navigation system and the one-pedal driving system started malfunctioning. The navigation system began displaying a different location during remote driving, causing it to send me down a street against traffic. This issue has never been resolved and continues unabated. Then, while on the highway, when I needed to slow down, the one-pedal driving system deactivated, causing me to almost collide with another vehicle. This has happened more than three times this month. GM and Chevrolet just keep saying they'll call me back to resolve the issue, but they never do, and they refuse to fix it. | Crash: No Fire: No | Unknown |
| Apr 2026 | POWER TRAIN,ELECTRICAL SYSTEM,VISIBILITY/WIPER | I was driving my car and a "service high voltage battery" message appeared on the screen and a message to reduce charging the car to 68%. The message coincided with the car having no cabin heat and no ability to defrost the windshield. The dealership says that the cabin heat and AC use the same cooling system as the battery and that any failure in the battery cooling system can cause a no heat/AC situation | Crash: No Fire: No | Unknown |
| Apr 2026 | UNKNOWN OR OTHER | OnStar Module failure. This did not trigger any type of notification. I had to make several phone calls to OnStar to determine I would need to schedule a service appointment for a technician to look at the vehicle. After inspection it was noted that the OnStar Module is not working and will need to be replaced. Upon ordering the part I was informed it is on backorder with no delivery time noted. If I were to get into a crash OnStar would not have triggered a call to first responders for my safety - which is a service that I pay for. No warning lights appeared and the the OnStar button still shows "green" as in active service. | Crash: No Fire: No | Unknown |
