Aug 2024 FORWARD COLLISION AVOIDANCE: WARNINGS When crossing a one-way road with oncoming traffic from the left, a vehicle that has recently passed might be triggering the forward collision avoidance system which cuts off throttle control to the Sienna, even when the passed vehicle is well out of the way. It is unknown if the collision avoidance feature is being triggered as there are no warning lights or messages in the display. This results in the Sienna having no accelerator response and sitting “dead in traffic” sideways in front of oncoming cars. This this is the second time this issue has occurred and today I was nearly t-boned by on-coming vehicles as there was no throttle response to move the car forward. This issue has been reported in 4th generation Sienna (‘21-‘24) forums on Facebook. This issue puts at risk vehicle occupants as well as other vehicles on the road.

Jul 2024 SEATS The 3rd row seats do not remain stowed when passing over a speed bump or dip in the road. Depending upon the speed of the vehicle, the seats might jump up several inches and almost return to the fully stowed position with a loud thud. The seats then bounce up a few inches from the fully stowed position and remain unsecured. It's possible that the seats might unexpectedly fully open at higher speeds.

Jul 2024 SEAT BELTS My child [XXX] ) engaged the automatic locking retractor (ALR) and as it wound back in it wrapped around his neck. By the time we got to him it was tight enough that it was difficult to move or rotate or slide the seatbelt over his head, and we ultimately cut the seatbelt to free him. In addition to the actual safety incident, I was very surprised to learn that this was even possible. I had never heard of this safety issue, and had absolutely no game plan or awareness about this issue. I saw the SBIR program addressing innovation in ALR systems which is exciting! However I would also strongly encourage some sort of awareness campaign or warning messaging to raise awareness and ensure parents keep belt cutters in their cars. Jul 2024 SEATS Rear foldable seats do not lock down into compartment. They bounce up when you stop just a little hard. Cable has already broken when this occurs. Anything laid on top of these seats when folded will be ejected inside of car. Hitting driver in the head. This is very dangerous. Already brought to dealer and complained and nothing was done. They said that's the way it is. Someone will die if this problem is not taken care of.

Jun 2024 ENGINE I was at a complete stop. There were no obstacles around me or oncoming. When I pressed the accelerator nothing happened for a full 3 seconds, until my vehicle jolted forward at an excessive speed. Brakes were not working.

Apr 2024 VISIBILITY The contact owns a 2024 Toyota Sienna. The contact stated that while driving at approximately 70 MPH, the sunroof exploded into the cover. There were no injuries sustained. The contact stated that there were no previous issues with the sunroof. The vehicle was taken to the dealer where the contact was informed that the failure was due to an unknown foreign object. The manufacturer was made aware of the failure and a case was opened. The failure mileage was approximately 9,255. The VIN was not available.

Mar 2024 UNKNOWN OR OTHER Excessive heat coming from all vents and under steering wheel after long time driving. I am very concerned that the vehicle may be caught on fire and then cause injuries or casualties.

Mar 2024 VEHICLE SPEED CONTROL,FORWARD COLLISION AVOIDANCE Vehicle was driving on a slow traffic local street at roughly 10MPH. Spacing between car in front of me was around 30 ft. I was gliding towards front and ready to brake. Suddenly the vehicle start to accelerate hard by itself without driver’s operation. I tried to engage brake / gas pedal and was not helping. Front collision system did not activate like it suppose to. I ended up turn sharp right towards road curb and hit pole at average speed of 30mph or less. No air bag deployed. I then switch to Park. No warning lights pop up. All safety features on vehicles were turned on before the incident. Whats weird is the car is functioning normal now after incident. I contacted Toyota USA and waiting for feedback.
