While moving my seat back using the electronic seat lever a white clip with two metal bearings was sticking out, I notice the ball bearings were not in place and had came off the white clip. Yes, it is available for inspection. This pose a safety concern as the seat rails will not function correctly when moving seat or using the memory seat buttons for each driver. I have not brought the vehicle to a dealer to inspect as there is a estimate fee involved. I am currently out of my 2 year toyota care warranty. This occurred yesterday with my sister in the car and we almost rear ended the vehicle stopped in front of us. I have complained to the Cumberland Toyota dealership in the past and was told if they cant recreate the issue and it doesn't happen all the time there is no way to fix the issue. Yesterday I was almost in an accident because of this faulty design/anomaly associated with my car. The vehicle intermittently surges, lurches, or creeps forward while I am pressing the brake pedal and attempting to slow or stop. It feels as though the vehicle briefly adds power or braking force momentarily decreases even though my foot remains on the brake. The sensation is not normal creep alone; it feels like a brief unintended forward push while braking. The problem is intermittent, which makes it difficult to predict, but it creates a safety concern because it can happen when slowing, stopping, or maneuvering at low speed. I am concerned this could increase stopping distance or cause the vehicle to move forward unexpectedly. I am requesting investigation of possible defects involving the braking system, hybrid regenerative braking transition, brake actuator, pedal sensing, or related control software. It happens daily. I was able to turn vehicle on after several minutes, but SEVERAL system malfunction error messages (hybrid, accelerator, pre-collision, tpms, airbag, engine…) appeared. The vehicle would not accelerate beyond 10mph. Once I moved it to a safe location, I had the vehicle towed to the dealership. The dealership technician was able to replicate the issue during test drive. The technician apparently had the vehicle towed back to the dealership last night. The car has had other related issues since June 2024 when I bought it. The TPMS system, the accelerator system, and the airbag system have all malfunctioned independently. The accelerator system malfunctioned in Jan. I had to pay for a replacement fuse despite the warranty. The vehicle is modified with Braunability equipment for wheelchair access. These modifications are approved by Toyota and were completed by MobilityWorks. Aside from the vehicle itself being unsafe to drive, the passenger sliding door has had numerous problems. | Dec 2025 | TIRES | Incident Description (Firestone Destination LE3 – P235/65R17) I purchased four new Firestone Destination LE3 tires (size P235/65R17, DOT IW2 8ILE31) and within the first week three of them developed noticeable bulges in the sidewalls. I returned to the tire service center, and they replaced the three tires due to the bulging. The following week, new bulges appeared again in the sidewalls on multiple replacement tires, for a total of six affected tires. Sidewall bulging creates a risk of sudden tire failure and loss of vehicle control, and I do not feel the tires are safe to drive on. One service center stated this was “normal from driving on roads,” but I disagree due to the number of tires affected in such a short time. The condition has been repeated and confirmed by multiple service centers. The tires remain available for inspection upon request. There were no warning lamps or messages before the bulges appeared. The issue began immediately after installation of the first set, and the problem persisted with the replacement set. From the very first day, the trunk door has not functioned properly — often failing to close, randomly opening, or requiring manual force to shut. Despite repeated service visits across multiple states (IL, SC, NC) and investing considerable time, fuel, and lost income as a contractor, this issue persisted for over 18 months, creating safety concerns and inconvenience for me and my family. Key Issues: Trunk door opens on its own – even while reversing, creating serious safety risks. Malfunctioning power close mechanism, requiring manual closure frequently. Unreliable performance: door would close, then immediately re-open. Visited Toyota service centers at least 6 times across 3 states. Each visit took 2–3 hours, costing me work hours as a contractor. Customer service experience has been poor, especially with manager Patte, who: Failed to schedule proper service appointments. Directed me without coordination. | Oct 2025 | SUSPENSION,AIR BAGS,STRUCTURE | Keeps bottoming out suspension is specification is stated at 5.6 inches and I measured 3.3/4 unloaded never get a response on my complaints from my last repair and resolution on what they admitted that suspension was too low and wouldn’t put it in writing only saying that vantage mobility saying that it’s height requirements are as stated by vantage but no mention of ground clearance that I wanted them to verify that it’s specifications are stated at 6.5 inches and i measured 3 3/4 empty then they tell me not to go by specs and I’m a retired from wright Patterson Air Force base in the engineering department with logistics command and I’m like really guys you have to to have a specifications for a engineered system and they can’t even call back after they admit you have to go by specs! | Sep 2025 | VISIBILITY/WIPER | Wobbling vibration on exterior mirror, driver side. It looks like incoming vehicles shake sideways. This happens on any bumpy road and it is easier to observe at highway speeds. Purchased on [XXX]. Vehicle is available for inspection. The distorted image resulting from mirror wobbling affects driver's awareness of surroundings, visibility gets worse at nights/rain due to blurred lights. This can lead to an accident. My regular dealerships in Milford, CT have confirmed the issue but have not been able to provide a fix. They say the vibration is normal. I do not believe so as the passenger side mirror never has issue like that. I have first noticed this issue a couple of weeks when driving on highway. Posting my case online has gotten at least 4 people reporting the same issue with the Sienna since 2021 in multiple trims. | Sep 2025 | SERVICE BRAKES | On rare occasions (about once every 500 miles), when uniformly applying force to the brake pedal, the deceleration suddenly ceases and the vehicle seems to jerk forward. To a passenger, it feels like the driver's foot slipped off the brake pedal, but the driver continued to provide the same uniform force to the brake pedal and lost all deceleration. Typically, the driver is startled and presses significantly harder on the brake pedal jerking all occupants forward. The brakes re-engage and continue the deceleration at a much higher rate. Toyota addressed this problem with TSB 4724 which involved reprogramming the brake actuator assembly. This corrected the problem for about 3 months, but it is now occurring again. When the car was taken to the Toyota dealer again, the technician stated that TSB 4724 was cancelled, he could not duplicate the action (even though he was told it only happened on rare occasions), and claimed that the "brakes function properly as they should." Fortunately these loss of braking incidents have only occurred on dry pavement so that the driver did not lose control of the car when slamming on the brakes to compensate for the loss of deceleration. Coming down a hill on a snow covered road could have caused the vehicle to slide into an oncoming lane. It is my speculation that there is a null spot that exists during certain circumstances which causes there to be a "gap" between when this hybrid vehicle transitions between the dynamic hybrid brake and the friction wheel brake. Both braking systems independently work fine, but the transition between them (on rare occasions) does not occur continuously. This causes the driver to experience an unexpected loss of braking which, under the right conditions, could cause the driver to loose control of the forward motion of the car.
