| May 2026 | VISIBILITY | The contact owns a 2023 Toyota RAV4. The contact stated while the vehicle was parked, the hatch door glass independently fractured. The vehicle was taken to an independent mechanic and repaired. The contact was informed that the failure was a known failure with the vehicle. The dealer was made aware of the failure but provided no assistance. The manufacturer was not made aware of the failure. The failure mileage was approximately 119,000. | Crash: No Fire: No | 119,000 |

| May 2026 | ENGINE AND ENGINE COOLING | Was moving forward when all of the sudden the tailpipe flew off (metal thing in the muffler). No real cause, moves the vehicle normally oyt of the driveway and it just flew off. | Crash: No Fire: No | Unknown |
| Apr 2026 | STEERING | During the day after driving in rain in a 2023 Rav 4 LE AWD with 19000 miles car turned off. I was parked with it idling and with no warning stalled. I turned on multiple times and it kept stalling out. Finally a message appeared on dash "power steering low". After inspecting under hood and seeing all fluids ok I tried car again (maybe 10 min) and it started. Drove around parking lot it was fine. Drove to my mechanic. No error codes, battery was in good shape. This is very concerning. Suspect water in a hard rain somehow got in car. I was on major roads prior to parking, no huge puddles. Others online have mentioned this issue with no real answer and fears this will occur again while driving. NEEDS A RECAL. | Crash: No Fire: No | Unknown |
| Apr 2026 | ELECTRICAL SYSTEM | I have a limited service campaign from Toyota 25TD01 that my local authorized Toyota dealer refuses to repair. The dealer is Classic Toyota of Henderson | Crash: No Fire: No | Unknown |
| Apr 2026 | BACK OVER PREVENTION | When I put the vehicle in reverse the backup camera is displaying a black screen. I looked up recalls and see there is a recall on RAV4’s for this very issue, but my VIN shows no recalls. I called service at Hubler Toyota in Greenwood IN and they confirmed my VIN is not included in the recall. I want to know why it is not included. I am taking it to Hubler tomorrow for diagnostic and repair. I called Toyota manufacturing to report the incident and was advised to contact NHTSA. | Crash: No Fire: No | Unknown |
| Mar 2026 | LANE DEPARTURE,BACK OVER PREVENTION,FORWARD COLLISION AVOIDANCE | System states Pre System Collision Malfunction- Parking Brake Malfunction- secondary collision Malfunction warnings. AWD system malfunctions | Crash: No Fire: No | Unknown |
| Mar 2026 | POWER TRAIN,FUEL/PROPULSION SYSTEM | The vehicle exhibits a consistent and reproducible malfunction in the powertrain system, specifically related to acceleration response and drivetrain engagement. While driving at a steady speed, when the accelerator is pressed gradually (approximately 50–60%), the engine RPM increases but the vehicle does not accelerate accordingly. There is a delay in power delivery. When the accelerator is released, the engine may continue revving briefly, followed by a sudden and jerky engagement of the drivetrain. This issue occurs consistently under the same driving conditions and can be reproduced every time. The vehicle is available for inspection upon request. This malfunction creates a safety risk when merging into traffic, entering intersections, or making turns. The vehicle does not respond when acceleration is required, which can lead to dangerous situations where other vehicles may approach at higher speeds. After the delay, the vehicle may surge forward unexpectedly, increasing the risk of collision. The issue has been reported to multiple authorized Toyota dealerships on several occasions within the first 15 months of ownership. The vehicle has been inspected by dealership technicians, but the concern has not been diagnosed or repaired. Dealerships have stated that the behavior is normal, despite the repeatable nature of the issue. No warning lights, error messages, or diagnostic indicators have appeared at any time. No diagnostic trouble codes have been reported. The vehicle is equipped with an 8-speed automatic transmission, and the behavior appears consistent with a possible transmission, torque converter, or drivetrain control malfunction affecting acceleration response. The condition is not related to driver input and occurs under controlled, repeatable conditions. | Crash: No Fire: No | Unknown |
| Mar 2026 | UNKNOWN OR OTHER,VISIBILITY/WIPER | Sat in driver seat, closed driver seat door, and the rear window shattered. There was no impact or other incident to the glass. | Crash: No Fire: No | Unknown |
| Mar 2026 | STEERING,POWER TRAIN,SERVICE BRAKES | The 2023 Toyota RAV4 8-speed transmission is catastrophically defective and poses an immediate safety hazard. It exhibits severe hesitation, slipping, and lurching when accelerating from a stop or low speed, and when attempting to merge into high-speed traffic (I-15/Las Vegas), creating a high risk of rear-end collisions. During these events, the RPMs spike and fall rapidly but the vehicle fails to accelerate. Furthermore, the vehicle lunges forward while the brakes are applied. The engine actively fights the braking system, requiring extreme physical force to stop the car and prevent front-end collisions. This lunging has repeatedly triggered the Pre-Collision System, causing the seatbelts to lock and tension against the driver’s body. Additionally, the vehicle exhibits steering resistance and feedback during turns, making it difficult to maneuver safely. The local dealership (AutoNation Toyota Las Vegas) inspected the vehicle and claimed these life-threatening behaviors are "normal" for this model. | Crash: No Fire: No | Unknown |
| Mar 2026 | STEERING,SUSPENSION | Had safety inspection done when having oil changed on 3/5/26. was informed by dealer that it needed both outer tie rod ends replaced has play in it. this came to a cost of 735.00 with alignment. Of course not covered by vehicle warranty which is useless at this point. I have never had this problem on any vehicles I have owned since starting to drive in 1975. This is a safety and quality issue and I should be reimbursed. I will seriously think about about an American owned company in the future. Having Friend at another dealership TCO 3/18/26 I was away last week and SUV parked in garage. | Crash: No Fire: No | Unknown |
| Mar 2026 | UNKNOWN OR OTHER | rear hatch fails repeatedly to open. it locks up. it failed to shut too and damaged a rear taillight, forcing me to bring it to toyota. i had toyota dealer look at it twice now. from jan up to mar of this year. they had my car 2 weeks. even after replacing hatch struts and hinges,it still happens. it has to be an electrical, mechanical, or computer issue causing it. new vehicle, now under 20k mi. the hatch is widespread in rav4 models and I've found soo many reports online of same problem. this needs to be investigated more and fixed and recalled. | Crash: No Fire: No | Unknown |
| Mar 2026 | STEERING,ENGINE | On a rainy day after driving on the interstate, I pulled into a drive thru. Once there and idling for a few minutes, I tried moving forward and the car rough idled, shows message on dash "Engine Stopped, Power Stering Low" and had reduced engine power and no power steering and then stalls out. I tried turning it back on and it stays on for less than 3 seconds and shuts off. I had it towed to the dealer and they say there are no codes. After reading online, multiple other users experienced the exact same thing when its raining, wipers were on and they were idling after a trip on the interstate. If this would have occurred on the interstate and I lost engine power and reduced steering, it would cause major safety concerns. There were no warning lights prior to the issue and the only thing that flashed for a second when the engine stalled was the power steering, check engine and battery light. Toyota checked the battery and said its at 100%. Dealer cannot identify and issue, but I couldnt drive, had no power steering and had to have the car towed. | Crash: No Fire: No | Unknown |

The vehicle was taken to the dealer, where it was diagnosed and determined that the front and rear wheel alignment had failed and needed to be repaired. The vehicle was repaired; however, the failure recurred. The vehicle was taken back to the dealer, but was not diagnosed or repaired. The manufacturer was made aware of the failure. The approximate failure mileage was 74,000. | Crash: No Fire: No | 74,000 |

The contact stated the front passenger’s side Occupant Classification System (OCS) sensor failed to detect an occupant seated in the passenger’s seat and was deactivated on the instrument panel. The dealer was contacted and informed of the failure; however, the vehicle had not been taken to be diagnosed or repaired. The contact researched and found information on NHTSA Campaign Number: 22V519000 (Air Bags) and associated the failure with the recall. The manufacturer was informed of the failure, and the contact was informed that the VIN was not included in the recall. The manufacturer referred the contact to the NHTSA Hotline to report the failure. The failure mileage was 57,500 | Crash: No Fire: No | 57,500 |

| Dec 2025 | POWER TRAIN,AIR BAGS,VEHICLE SPEED CONTROL | My wife was going at about 10 - 15 mph coming up about 2 car lengths to a small curve in a residential area when all of a sudden she heard a pop or snap and the Rav4 went to a super high speed by itself. The acceleration could not be stopped even though she had her feet on the brakes. She had both hands on the steering wheel to avoid hitting a house and ended up going through a wall in the yard. There were no warning sounds and no signals indicating a problem. it appeared that the display was blank. There was no pre-collision warning or braking except as my wife was pushing on the brakes. She also hit the right side into side wall. No air bags deployed even though Windshield and all door glass on sides shattered. My wife the driver was okay and not hurt, and the grandchild in the car seat behind my wife was fine also. Neither had injuries. There were checked by emt. The police looked but all would not try to get in except to remove my granddaughter. My wife was removed also because she could not get out the door was jammed against shed and rock, and handles broken off This happened after the recall work was done by toyota dealer on 12/04/2025. | Nov 2025 | ELECTRICAL SYSTEM,UNKNOWN OR OTHER,ENGINE | I purchased my 2023 Toyota RAV4 new back on March 13,2023. 10 days later on March 23 it was raining as I pulled into BJ’s parking lot, I parked and began fumbling around getting my bag and keys together when my car started shaking and sputtering eventually shutting down with an error message engine stopped steering power low. After a call to the salesman I was able to get my car restarted after 15minutes and many failed attempts. Car was bought back to service the following week and I was told car was not properly programmed at delivery, it was now programmed correctly and this should not happen again. April 17,2023 my son pulling into Tractor supply , raining out, pulls into spot and car again shakes, sputters and shuts off, same message engine stopped steering power low. After many attempts car restarts approximately 20 minutes later. Car serviced codes ARE FOUND and cleared, road tested and told car was ok. May 31,2025 I was driving down Rt 80, pouring rain when traffic comes to a complete stop due to accident ahead. Car starts shaking, sputtering and shuts down. Same message code as always. I turned the car completely off as not to kill battery and started to goggle the RAV4. I attempted to restart the car numerous times. Called for a tow, prayed that I didn’t get killed once traffic started to move. Finally 1-1/2 hours later it started. Traffic began moving 15 minutes after I got it started. Took in for service, charged $199.61 only to be told there were no codes. Problem could not be duplicated and in the future to always disable the start/stop button while using the vehicle. October30, 2025 raining, I stopped at Panera drive thru. Placed an order waiting in line and car shuts down, same error code, couldn’t restart car for 20 minutes. Took to Toyota November 5th same response no codes and cannot replicate issue, car is ok. Please help rectify this before someone gets killed! | Crash: No Fire: No | Unknown |

| Nov 2025 | UNKNOWN OR OTHER | We discovered severe mold contamination and water intrusion in our 2023 Toyota RAV4 Hybrid. My wife, our [XXX] child, and I have been experiencing dizziness and nausea for months whenever we were inside the vehicle, but we did not know the cause until the dealer (DLL) showed us extensive mold throughout the cabin and standing water trapped inside the vehicle. The dealer confirmed that this was not normal wear and tear. The mold has spread into the floors, interior panels, and even the steering components. This is a serious health hazard. After doing research, we found many other RAV4 Hybrid owners reporting similar water leaks and mold growth issues. This raises the concern of a potential design or sealing defect in this model. This vehicle is unsafe to drive, especially with a young child. We are requesting a full safety investigation into the water intrusion, sealing integrity, and air quality risks associated with this model. Symptoms: • Dizziness • Nausea • Headache • Strong mold smell • Corrosion of interior components This is a serious safety and health issue, and we are asking NHTSA to investigate. | Nov 2025 | ELECTRICAL SYSTEM,FORWARD COLLISION AVOIDANCE | While driving or idling, the pre-collusion, RSA malfunction, Headlight system malfunction, and LTA system malfunction will go off as well as my navigation on my screen. Its been going on and off for 7 months now. It works, then it doesn't. I don't want to take it in because I know they're famous for making me pay high dollar fees for anything and everything. Toyota is horrible for their services and just want the money. I never had any issues with my car until I got a nail in my tire that was an easy fix (that they also wanted to keep my car overnight for a patch). | Crash: No Fire: No | Unknown |
