After 1 mile from Hoestead Farm, Tesla autopilot hit the side road block and it dragged with scratch and dent on right side of the back door. (That driver can not even control.) The dashboard display is showing the following error message: “front passenger safety restraint system fault”. Tesla informed me that the occupant classification safety system filter module has failed and must be replaced. Tesla has agreed to fix the component under warranty, but they informed me they could not schedule a repair until October 31. It is my understanding that damage to this component could prevent the airbags from functioning properly during an accident, so I am concerned that the car is not safe to operate with passengers. | Aug 2024 | STRUCTURE | I was driving highway speed several miles from home on the highway. Suddenly and spontaneously the hood came up on the car and smashed through the windshield. There were no other cars in that lane, which allowed me to pull over on the right shoulder. However, if there was any other cars and I hadn’t been in the right lane, it would’ve caused a major accident as there was no way for the driver to see and the car is not aerodynamic enough to move at high speeds at that point. After the car was pulled over, and I was able to get out without glass getting everywhere. It was no noticed that the car still thought the front hood was engaged and was allowing me to hit the open button as it was closed. The car also never recorded the event, even though the windshield was smashed with the dash cam that is built into the car that is meant to auto detect incidents. The car never even gave a warning that the hood had any issues. I checked out the front hood and noticed the latch that is supposed to be attached to the hood was still attached to the lock, but not attached to the hood. There was no thread in the holes on the hood meaning that the bolts must’ve had to unscrew as opposed to snap. I only saw one bolt left in the car and even that bolt came out later. Once the car was towed to the Tesla dealership, the Tesla dealership would not help us and did not say that they could fix it. We took a video of hitting the unlock button for the hood, and which the lock can be seen being given up by the locking mechanism and thrown onto the floor. Only at this point did the car even have any safety warning that the hood might be open. The car forces you to acknowledge that the hood is even open at all in order to drive. I tried it on another Tesla and only latched it down partially and even then it still says it’s open. The car came to a complete stop and wouldn’t move forward or reverse. I got out of the car walked around and saw two large bolts laying on the floor. Bent down to look under the car and saw the lower control arm on the right front laying on the ground. Apparently, the two bolts had come loose somehow and the control arm dropped. | Aug 2024 | AIR BAGS,VEHICLE SPEED CONTROL,SERVICE BRAKES | We were about the park in the marked parking lane and the vehicle unintedly accellerated, the brakes did not work and we saw message "Automatic emergency brake not available" and Air bags not available. We tried to control the vehicle to avaoid crashing into the porch of the infront apartment and steared the speeding vehicle and the vehicle hit the pillar of the aparement and finally came to halt few meters away in the parking lot as the front right wheel broke. 1. The ADAS Automatic emergency breaking seems failed 2. Forward collission warning seems not working 3. The safety of the driver and the passenger were at risk while the collission happened 4. I am not sure if the similar incidents happened or can be reproduced by the dealer/manufacturer. 5. Vehicle components inspected by the Tesla collission center but no software logs inspected and they said that they cannot get the device crash/event logs. 6. This can occur on empty roads in excellent weather conditions, but happens every time I have used the cruise control. The rapidity of the deceleration creates a significant risk of being rear ended. I have stopped using cruise control as it is too dangerous. This has been a noted problem on multiple Tesla models for multiple years. There were no vehicles in front, behind, or beside my vehicle. The breaking occurred when it came across a line break where cars from the opposite side of track could turn left onto a service highway but no one was in that divide nor in the turn lane. There was no warning or any obvious indication that the vehicle should harshly slow down for any safety reason, where in fact is there was a vehicle behind me, it could cause a crash. | Aug 2024 | SUSPENSION | A suspension bolt fell off while making a U-turn. It was linked to the lower arm. Hear a loud pop while backing out and made a left input on the steering wheel to complete the turn but the steering wheel will not turn. Check the right front wheel and it was toed inward. Called AAA and they where able to re-install the bolt. AAA also has also mention that another bolt was loose and about to fall off. He did not specific which bolt was also loose. Car was driving to a Tesla service center and they inspected the front and rear suspension. When asked why this had happen, the service advicer wasn't clear but stated bolt was loose from factory or at another service center. The car did not have previous work don't on suspension. | Aug 2024 | WHEELS | I bought aftermarket wheel covers for my Model Y from Tesmanian in August 2023. One flew off while driving in July 2024, and it was an extremely scary situation. I did not hit any pothole or strong bump or anything. It just popped right off. When I inspected my other wheel covers, they were in really bad shape. Many of the clips had failed (see pictures of two different wheel covers), and the wheel covers were likely imminently to fly off. I immediately contacted Tesmanian about this unsafe product. Tesmanian has completely ignored my safety concern and failed to protect the consumer - I have not heard from them following the last date I contact them, which was 4 August 2024 (see attached email chain). If one of these flies off a wheel at high speed, it has a legitimate potential to result in an accident. Tesmanian needs to immediately pull this product off the market, alert everyone who purchased this item as to the unsafe nature of the product, as well as provide a full refund to for all orders placed. I had my accident on [XXX] on [XXX] when the car enabled emergency brake for no reason when I was driving in my lane with no objects in its way. I was able to gain control of the tesla back and thankfully nobody hit me as a result. The second glitch happened couple minutes later when the phantom braking spinning my car around causing it to hit the concrete barrier and causing a damage to the car. The contact stated while his daughter was attempting to make a left turn at approximately 5 MPH, another vehicle crashed into the rear passenger-side end of the vehicle. There was cosmetic damage and the vehicle came to a stop in the middle of the road. The air bags failed to deploy. As a result, the driver impacted her head on the steering wheel causing head injuries that required medical attention. The vehicle was towed to a dealer and was not given a diagnosis. The vehicle was repaired however, the failure still occurred; the contact was informed the headliner was damaged due to the repairs done along with several other failures. | Jul 2024 | VISIBILITY/WIPER | I have a complaint about the wiper system. The wiper settings include "Off", "Auto", and numbers 1-4 corresponding to different wiper speeds. The "Auto" detection is based on the front facing camera rather than any rain sensors, and is incredibly inconsistent. It regularly turns on the wipers in completely clear and dry conditions and often fails to turn on the wipers even in heavy rain. This would be more of an annoyance than an issue if it were possible to adjust the wiper speed manually, but it seems that the wipers still utilize some form of auto detection even when set to numbers 1-4. In this particular incident, I was driving in heavy rain, and had the wipers set to "4", the highest setting. Even in this setting, the wiper speed would often slow down regardless of the weather conditions, drastically reducing visibility in an unpredictable manner. At one point, the wipers, still manually set to the highest setting, stopped altogether, and I instantly lost all visibility and I had to emergency brake, as I could not see the road. On the highest setting, the wipers stopped for well over a second, and did not start again until I pressed the wiper button, restarting the wipers. It is one thing to rely on a proprietary and unreliable detection system when wipers are set to "Auto", but I feel it is a major security issue to deceptively allow this unreliable rain detection to override manual wiper settings set by the driver to the extent that the wipers may stop moving entirely in the middle of a downpour on the highest setting. | Crash: No Fire: No | Unknown |
| Jul 2024 | LANE DEPARTURE | Since the recall earlier this year on for the Autopilot, I now have a car that is actually more dangerous as it emits a very jarring audible warning when I engage the Autosteer option. This has actually caused me to jump in my seat and cause me to turn the wheel creating a dangerous situation. Not only is this a potential accident waiting to happen, it is also superfluous as the driver is the one who pulls in the turn signal stall to engage the Autopilot and the Autosteer functionality. I do not need a very loud and startling tone to sound advising me that the Autosteer function is engaged. Please remove this as it is creating a dangerous driving environment.
