| Jul 2026 | POWER TRAIN | The internal transmission Output Shaft Speed Sensor (OSS) and its associated internal circuit/lead frame assembly failed. The vehicle is fully intact, operational, and available for inspection upon request. When the Output Shaft Speed Sensor intermittently loses its signal (dropping to 0 RPM while the vehicle is stopped at red lights or in traffic), it forces the vehicle into an immediate electronic "limp mode." Upon pressing the gas pedal to accelerate, the transmission violently slams into gear with massive mechanical force. This sudden, unpredictable jerking and hesitation while accelerating into active traffic creates an immediate risk of being rear-ended, losing traction, or losing control of the heavy vehicle in an intersection. The problem was officially confirmed via a diagnostic computer scan by an authorized GM dealership (Crews Chevrolet), which pulled and documented history codes P0722 (Output Shaft Speed Sensor Circuit No Signal) and P0700 (Transmission Control System Malfunction Warning). However, the dealership declined physical teardown or sensor replacement because the electronic failure is intermittent and could not be replicated during a brief technician test drive. Yes. The vehicle's external wiring harness was physically inspected by an authorized General Motors dealership technician, who confirmed the external wiring passed inspection. This officially isolates the intermittent electrical drop to an internal transmission component failure. It has not been inspected by police or insurance. Yes. The symptoms appeared suddenly without prior warning. The transmission began shifting erratically and violently, immediately followed by the illumination of the dashboard Check Engine Light (Malfunction Indicator Lamp). The onboard computer triggered diagnostic trouble codes P0722 and P0700. | Crash: No Fire: No | Unknown |
| Jul 2026 | POWER TRAIN | An internal transmission Output Shaft Speed Sensor (OSS) failure causes the vehicle to enter "limp mode" and shift violently, creating a risk of rear-end collisions or loss of control, according to the report. A GM dealer confirmed diagnostic codes P0722 and P0700, isolating the issue to an internal component after an inspection of the external wiring harness. | Crash: No Fire: No | Unknown |
| Jul 2026 | ENGINE | While traveling at night on the highway at excess of 70mph the engine in my 2022 Chevrolet Silverado 1500 6.2L L87 experienced a sudden, complete mechanical failure and loss of propulsion while driving. There were no warning lights, sounds or any other indication the motor would instantly stop working. This created an immediate crash hazard for me, my children and my dog. The vehicle is completely inoperable and I paid $125 to have it towed home. Upon talking to the dealership service ce manager they are refusing to inspect/replace my engine under Safety Recall N252494001. The same dealer is also requiring a large diagnostic fee prior to looking at the vehicle and has stated they will require additional fees as needed. They also refused to provide a loaner vehicle. | Crash: No Fire: No | Unknown |
| Jul 2026 | ENGINE | Intermittent knock on start up. Goes away within 10-15 seconds of continuous run. Same noise would also reoccur when towing a load. | Crash: No Fire: No | Unknown |
| Jul 2026 | ENGINE | Vehicle's replacement motor from GM recall (2nd motor) failed at 6200 miles. Rear tires briefly lock up causing slight swerving before the system shifted to "N"eutral. Upon restart, starter was unable to crank over seized engine. DTC confirmed P0016 crank shaft position out of alignment. | Crash: No Fire: No | Unknown |
| Jul 2026 | ENGINE | We purchased our 2022 Chevrolet Silverado 1500 ZR2 in XXX, it was discovered in the purchase process that the vehicle had an active recall on the engine which was pending a "fix." This was completely downplayed by the dealership as a minor issue that would be addressed. We waited several weeks for the "fix," which was essentially a change in the oil viscosity. On XXX, our [XXX] son, was out of town, driving the vehicle, which suddenly lost all power on the highway leaving him completely stranded. The vehicle would not turn over at all or even shift into neutral. There were no warning signs, no sensors activated, in fact, the Chevy App showed (and still shows) the vehicle as "everything looks good" "no diagnostic issues!" We had the vehicle towed back to our hometown/dealership (Missoula, Chevrolet in Missoula, MT), where it has now been determined, the engine completely seized up/failed, the original "fix" was not adequate, and we are now waiting for a new engine to be put into the vehicle! After reading about other vehicle owners experiences, I am quite skeptical of the new engine as there have been multiple experiences where the new engines had the same issue. This recall is impacting the safety of drivers, most concerning was our [XXX] left stranded in a rural area with very spotty cell service, and the vehicle would not even shift to neutral to get it to a safer area, this needs to be addresses as soon as possible to prevent others from being put into this situation. | Jul 2026 | POWER TRAIN,FUEL/PROPULSION SYSTEM | Truck stutters and idles terrible. Vehicle warning light keeps saying, reduce acceleration and drive safely. Two years ago an injector went bad, and it was fixed. 6 months ago, we had it in the shop, of course still under the 60k mile warranty. Said it was fixed, it wasn’t. Just had it back there, and they replaced the the torque converter. Shaking stopped for a month. Now it is even worse. Believe it is faulty injectors. Sadly, we’re not passed the 60k warranty period and feel unsafe driving our truck which we need every single day. Truck is nearly paid off and feel like we got jipped as this truck continues to have problems after problems | Crash: No Fire: No | Unknown |
| Jul 2026 | POWER TRAIN,ENGINE,FUEL/PROPULSION SYSTEM | Truck continues to shutter after having the torque converter replaced and the 2nd injector replaced. Been in the shop several times over the last few years, and still giving warnings on the dash saying, reduce acceleration drive safely. Feel like we just got a terrible vehicle. Almost paid off, and it’s just unreliable. Sadly we need this truck every single day, and the local dealerships have now pushed the problem out so far that we’re passed out 60k warranty period. | Crash: No Fire: No | Unknown |
| Jul 2026 | ENGINE | I was driving on the freeway at 70 mph and my engine totally froze which made my car die and almost caused a huge accident right in the middle of the freeway. Then I had to push it off to 12 mile road gas station in the middle of oncoming traffic. My engine seized and it has been in the dealership since June 3. It is now July 4 2026th so 31 days and I still haven’t even found out if it’s my engine or not they haven’t really told me anything. | Crash: No Fire: No | Unknown |
| Jul 2026 | UNKNOWN OR OTHER | I believe My onstar control module needs to be replaced after trying every option with them over the phone “resets and full battery unplugged reset” this is a major concern for me on the safety aspect of onstars main selling point of instant reports of accidents and is in my mind a major issue. I do not believe I should be on the hook for a $700-1500 part that failed with only 59000 miles. | Crash: No Fire: No | Unknown |
| Jul 2026 | VISIBILITY/WIPER | 2022 SILVERADO ZR2 on second set of headlights due to extreme moisture inside of headlights and at night becomes a saftey issue due to the water distorting the beam of headlights. The headlights in it now are a year old | Crash: No Fire: No | Unknown |
| Jul 2026 | SUSPENSION | The contact owns a 2022 Chevrolet Silverado 1500. The contact stated that while driving on several occasions at various speeds, there was a clunking sound coming from the rear of the vehicle. The contact stated that on other occasions, while driving 65 MPH and above, the vehicle drove over a bump in the road, and the rear of the vehicle lost control and drifted into another lane. The vehicle was taken to a dealer, where it was diagnosed that the leaf spring pads needed to be replaced, and the rust protection was the cause of the leaf spring bushings cracking and separating. The vehicle was not repaired yet. The manufacturer was notified of the failure and offered to cover a portion of the cost of the repair. The failure mileage was approximately 25,000. | Crash: No Fire: No | 25,000 |
| Jul 2026 | POWER TRAIN,ELECTRICAL SYSTEM,ENGINE | 1)The truck experiences an intermittent but complete failure of the Crankshaft Position (CKP) Sensor circuit and multiple chaffing points of the vehicle wiring harness. This issue is a documented manufacturing and electrical defect covered under General Motors Technical Service Bulletin (TSB) #24-NA-012. Yes, the truck is available for inspection. 2)This issue has happened to me on five occasions where I was traveling over 50 MPH and the vehicle violently jerked with my wife and children in the truck with me. This defect poses an immediate, severe safety hazard due to unpredictable, sudden engine/transmission jerk while operating at highway speeds. My main priority is my family! This failure compromises my ability to safely maneuver, accelerate, or steer the heavy-duty vehicle out of active lanes of traffic, dramatically increasing the risk of a high-speed rear-end collision or crash. 3) The problem has been reproduced by Rick Hendrick Chevrolet Norfolk and R.K. Chevrolet Virginia Beach. There are service invoices confirming these reproducible jerks and subsequent engine codes thrown. 4) No, I brought this up to General Motors as a safety issue but they have brushed off my concerns. 5) The second time I had the violent jerk I had a message appear on my DTC that stated "174 mi Until 65 MPH Max Speed and Service Emission System See Owner's Manual" that was on 2/21/2026. Extra Note: The vehicle sat disabled at a dealership service center for 21 cumulative days during the initial attempt to repair this specific defect under TSB #24-NA-012. Merely 58 days after the repair, the identical crankshaft position sensor circuit/wiring failure reoccurred while driving, resulting in secondary engine failure and requiring a visit to an independent service facility for diagnostic verification and wiring replacement. Internal vehicle history confirms that prior to my truck ownership the crankshaft position sensor was replaced in December 2025 weeks before I purchased the truck. | Crash: No Fire: No | Unknown |
| Jun 2026 | ELECTRICAL SYSTEM,LATCHES/LOCKS/LINKAGES | The contact owns a 2022 Chevrolet Silverado 1500. The contact stated that while the vehicle was parked, several unknown warning lights were flashing. There was an abnormal ticking sound coming from the vehicle before the tailgate opened independently. The contact stated that while attempting to close the tailgate, the tailgate failed to latch securely. The contact stated that the vehicle was unoccupied and turned off when the failure occurred. The dealer was made aware of the failure, and the contact was informed that the repair was not covered under recall. The vehicle was not diagnosed or repaired by an independent mechanic or dealer. The contact referenced an unknown recall; however, the VIN was not associated with a recall related to the failure. The contact stated that the failure was a persistent failure. The manufacturer was not made aware of the failure. The failure mileage was approximately 40,000. | Crash: No Fire: No | 40,000 |
| Jun 2026 | ENGINE | My company purchased a new 2022 Chevrolet Silverado RST from Bomnin Chevrolet Manassas in December 2022. The vehicle has been properly maintained according to GM’s recommended maintenance schedule, including routine oil changes and tire rotations. On February 10, 2025, the truck began shaking and repeatedly shutting off. Bomnin Chevrolet diagnosed a failed fuel injector and completed the repair. No loaner vehicle was available, requiring me to use Uber transportation until the vehicle was returned on February 12, 2025. On May 19, 2025, the truck again shut off multiple times while traveling approximately 45 mph. The vehicle was returned to Bomnin Chevrolet, where it remained for three days. No loaner vehicle was available, forcing me to rent a vehicle from Enterprise. The dealership ultimately stated it could not diagnose the problem and advised that the failure would have to occur again before it could be repaired. The vehicle was returned on May 22, 2025. On June 18, 2026, the engine shut off while driving and would not restart. The vehicle was towed to Bomnin Chevrolet. After several days, the dealership advised that the engine had suffered catastrophic failure due to a lifter issue and required complete replacement. GM approved the warranty repair; however, the replacement engine is on national backorder. No loaner truck was available. The dealership offered only a $44-per-day rental reimbursement, which is not comparable to the business use of a full-size pickup truck. As of June 29, 2026, the vehicle remains at the dealership, leaving my business without a suitable replacement vehicle and significantly impacting daily operations. I am requesting a voluntary manufacturer repurchase of the vehicle, reimbursement of all rental cars, and a fare financial compensation package for any inconvenience. | Crash: No Fire: No | Unknown |
| Jun 2026 | ELECTRICAL SYSTEM | The power-unlatching tailgate drops open unexpectedly. Opened on freeway at 75mph. Known saftey recall however my VIN isn't listed as an affected vehicle | Crash: No Fire: No | Unknown |
| Jun 2026 | ENGINE | 2022 Silverado with 6.2 l87 engine was taken into a Chevy dealership to have recall check on engine. Pico scope was used to inspect engine and no issues found. Oil was upgraded and oil cap replaced. A week later traveling many hours from home the engine suffered catastrophic failure and has now been at a dealership for the last seven weeks waiting for a replacement. There is no eta for a replacement. | Crash: No Fire: No | Unknown |
| Jun 2026 | POWER TRAIN,ELECTRICAL SYSTEM,UNKNOWN OR OTHER | On June 7th, 2026, I pulled into my driveway, and put the truck in park (did not set the parking brake, but left it running). I walked, approximately 25 feet up my driveway, towards my garage, to retrieve my garbage can, to bring it to the road. About the time i got to my garbage can, my truck proceded to roll towards me, as i walked back towards the road. I then noticed it rolling, let go of my garbage can, ran to the truck, jumped in it, hit the brakes, and i had to shift the truck back in gear, then back in park. By that time, it had already struck the support pole to my temporary car port, destroyed it, and caused a small amount of damage to the front fender of my truck (small dent, and some scratches in the front plastic). I know i did not leave it in drive, as i had to shift it into gear, then back into park, as well as the fact, that it stayed still, as i walked normally, 25 feet up the driveway. I tested the truck, after the fact. It starts rolling immediately after taking your foot off the brake, from the same spot. On a side note, my truck had just received an update download, 2 days prior to the incident. There were no warning lights. Luckily, my safety, was the only one at risk. I have not had it inspected, and it has not happened again, since. I did contact General Motors, immediately, but got no response to my email. I do have a short security video, starting when i retrieved my garbage can, to the moment the truck struck the pole. | Crash: No Fire: No | Unknown |
| Jun 2026 | AIR BAGS | I hit a deer. The driver’s side roof rail air bag attempted to deploy. The wiring shorted out and electrical smoke filled the cabin. | Crash: No Fire: No | Unknown |
| Jun 2026 | SERVICE BRAKES, HYDRAULIC | The contact owns a 2022 Chevrolet Silverado 1500. The contact stated that upon starting the vehicle, there was a message related to brakes. The contact stated that while driving at an undisclosed speed, the brake pedal was depressed, and there was an abnormal grinding sound coming from the brakes. There were no warning lights illuminated. The vehicle was taken to the local dealer, where it was diagnosed and determined that the brake pads and rotors needed to be replaced. The dealer informed the contact that the brake sensors had detached. The repair was not covered under warranty. The manufacturer was contacted and confirmed that the repair was not covered under warranty. The failure mileage was approximately 35,000. | Crash: No Fire: No | 35,000 |
| Jun 2026 | FUEL/PROPULSION SYSTEM | On June 18, 2026, my 2022 Chevrolet Silverado 1500 with the 5.3L V8 engine and approximately 41,000 miles suddenly stalled and died while driving in traffic and would not restart. The vehicle had to be towed to a Chevrolet dealership (Starling Chevrolet in Mt. Pleasant, SC). The service manager ran a diagnostic and identified the Fuel Pump Control Module (FPCM) as the likely cause of failure. This appears to be a known and widespread issue affecting newer Silverado trucks. The sudden stall while driving with no warning created a dangerous situation in traffic. The vehicle is currently at the dealer awaiting repair under the powertrain warranty. | Crash: No Fire: No | Unknown |
| Jun 2026 | ELECTRICAL SYSTEM,LANE DEPARTURE | I am having the same issue as the GM Safety Recall N232426400 - where it says it is a common issue in 2020-2024 Chevy 1500's (which my truck is a 2022 Chevy). Where the tailgate will drop by itself. I looked into this recall and my truck is not included. However, it just rained heavy and now my truck's tailgate is dropping on it's own. This is a safety risk due to loading the truck bed and having it open on its own without me knowing resulting in items falling out of my truck onto the roadway. Another thing is my lane keeping assist is no longer working. | Crash: No Fire: No | Unknown |
| Jun 2026 | ENGINE | recall remedy was performed just a few months prior didn't even make it through the life of the oil and the 6.2l l87 is seized up per Chevrolet dealership in Chelsea Michigan | Crash: No Fire: No | Unknown |
| Jun 2026 | POWER TRAIN | Code P0747 the car jerked forward twice and did not accelerate. The engine light came on. GM said my truck was not covered under the special coverage because of my VIN although it’s got the same problem for the same truck listed | Crash: No Fire: No | Unknown |
| Jun 2026 | POWER TRAIN | The contact owns a 2022 Chevrolet Silverado 1500. The contact stated that while driving 10 MPH, the vehicle hesitated and lurched forward while depressing the accelerator pedal. The contact stated that the failure reoccurred while driving from a complete stop. The vehicle was taken to the dealer, where it was diagnosed with transmission valve failure. The vehicle was being repaired. The contact related the failure to NHTSA Campaign Number: 26V083000 (POWER TRAIN); however, the VIN was not included. The manufacturer was not contacted. The failure mileage was approximately 70,000. | Crash: No Fire: No | 70,000 |
